‘Witness’ document of Belief & Good Deeds for the Deceased

From Mafatih al Jinan compiled by Shaykh Abbas al-Qummi

Shaykh al-ä}s| has advised that the following should be written on paper as a document of witness of the
belief & knowing only good deeds of the deceased believer.
This document is made & signed by one witness & further attested to by 40 believers (who can sign despite
their knowledge of faults of the deceased).
This document is then folded, put in an envelope and put with the palm-leaf stalk on the right side of the
deceased. It is preferably written with camphor (i.e. water with which camphor is mixed). and non-aromatic
aloe-wood, must be put on the dead person’s forehead
It is said that Allah will forgive the deceased for his faults if 40 believers testify in this way.

BEFORE signing, the following dua is to be recited by the first witness:

 ‫ﺣ‬
 ‫ٱﻟﺮ‬ ‫ﻦ‬ ٰ
‫ٱﷲ ٱﻟﺮ‬ ‫ ْﺴ ﻢ‬
In the Name of Allah; the All-
    
beneficent, the All-merciful. bismill{hi alrraÅm{ni alrraÅ|mi

ٰ ْ ْ
‫ٱﷲ‬
  ‫ﻻ‬  ‫ﻻ إ إ‬ ‫ﺪ أ ن‬   ‫أ‬
‫ﻬ‬ ‫ﺷ‬
I bear witness that there is no
god save Allah, ashhadu an l{ il{ha ill{ all{hu
 
One and Only Lord and having no
 ‫ﻳﻚ‬ ‫ﻻ ﺷ ﺮ‬ ‫ﺣ ﺪه‬ْ ‫و‬
associate with Him. waÅdah} l{ shar|ka lah}
    
‫ـــﺪه‬
‫ـ‬ ْ ‫وأ ْﺷــــﻬﺪ أن ﻣﺤﻤــــﺪا ًﻋ‬
‫ﺒ‬
I also bear witness that
MuÅammad is His servant and wa ashhadu anna muÅammadan            
≥abduh} wa ras}luh}
‫ورﺳﻮ‬
   
messenger,

‫آ‬ ْ
  ‫ٱﷲ  ﻋ ﻠﻴ ﻪ و‬  .ٰ  ‫ﺻ‬
May Allah bless him and his
 
Household. Äall{ all{hu ≥alayhi wa {lih|

ٌ ْ
‫وأ   ن ٱﻟﺠﻨ  ﺔ ﺣ ﻖ‬
[I also bear witness that]
  
Paradise is true, wa anna aljannata Åaqqun

ٌ
‫وأ   ن ٱﻟﻨﺎر ﺣ ﻖ‬
 
Hellfire is true,

wa anna alnn{ra Åaqqun

ٌ ٌ
‫ﻻ ر ْﻳـﺐ‬ ‫ٱﻟﺴﺎﻋ ﺔ ﺣ ﻖ آ ﺗﻴـﺔ‬ ‫وأن‬
         
the Hour [of Resurrection] is true
and is coming; there is no doubt wa anna alss{≥ata Åaqqun {tiyatun
l{ rayba f|h{
‫ﺎ‬8‫ﻓ‬
about it,


ْ ْ ْ
‫ﻮر‬
  ‫ﺚ ﻣﻦ ﰲ ٱﻟ ﻘﺒ‬
 ‫ٱﷲ ﻳﺒ ﻌ‬
  ‫وأ   ن‬
and Allah shall resurrect those wa anna all{ha yab≥athu man f|
who are in graves. alqub}ri
THE INSCRIPTION to be written & kept on the Right side of the dead body with a palm leaf:

servant and messenger, ras}luh}

that he acknowledges all of the wa annah} muqirrun bijam|≥i al-
prophets and the messengers [of anbiy{'i walrrusuli ≥alayhimu
God], peace be upon them,

‫ٱﷲ و إﻣﺎﻣ ﻪ‬    ‫وأ   ن ﻋ ﻠ ﻴﺎ‬
U  ‫و‬
wa anna ≥aliyyan waliyyu all{hi
    
Allah and the leader appointed by
Him, wa im{muh}
ْ ْ
‫  ﻤﺘ  ﻪ‬V  ‫ ه أ‬W  ‫  ﻤ ﺔ ﻣﻦ و‬V ‫ﻷ‬  ‫وأ   ن ٱ‬
that the Imams from ≥Al|’s wa anna al-a'immata min wuldih|

progeny are his leaders, a'immatuh}
ْ ْ ْ
 ‫ﺤ ﺴﲔ‬  ‫ﺤ ﺴﻦ  وٱﻟ‬ ‫ ٱﻟ‬K‫وأن أوﻟ‬
        
that they are in sequence are al- wa anna awwalahum alÅasanu
âasan, al-âusayn, walÅusaynu
ْ ْ ْ
≥Al| the son of al-âusayn, wa ≥aliyyu ibnu alÅusayni ‫ﲔ‬   ‫ ﺑﻦ ٱﻟ‬Y ‫ﺴ‬ ‫ﺤ‬    ‫و ﻋ‬
Y ْ ‫و ﻣ ﺤﻤ‬
 ٍ ‫ﺪ ﺑﻦ  ﻋ‬     
MuÅammad the son of ≥Al|, wa muÅammadu ibnu ≥aliyyin

Ja≥far the son of MuÅammad, wa ja≥faru ibnu muÅammadin

and the Rising Argument-person, walq{'imu alÅujjatu

‫ٱﻟﺴﻼم‬ ‫ﻢ‬8 ْ ‫ﻋﻠ‬

     
peace be upon them all;

≥alayhimu alssal{mu

ٌ ْ
‫وأ   ن ٱﻟﺠﻨ  ﺔ ﺣ ﻖ‬
 
and that Paradise is true,

wa anna aljannata Åaqqun

ٌ
‫وٱﻟﻨﺎر ﺣ ﻖ‬
 
Hellfire is true,

walnn{ra Åaqqun

ٌ
‫ﺎ‬8‫ﻻ ر ْﻳﺐ ﻓ‬
the Hour [of Resurrection] shall
‫ﺔ‬ ‫وٱﻟﺴﺎﻋﺔ آ ﺗﻴ‬
        
be coming; there is no doubt walss{≥ata {tiyatun l{ rayba f|h{
about it,
ْ ْ ْ ‫وأن ٱﷲ ﻳ‬
‫ﻮر‬    ‫ﺚ ﻣﻦ ﰲ ٱﻟ‬
‫ﺒ‬ ‫ﻘ‬ ‫ﻌ‬
        ‫ﺒ‬
and Allah shall resurrect those wa anna all{ha yab≥athu man f|

may Allah’s blessings be upon

‫وﻣ ْﺴﺘﺨ ﻠﻔﻪ  ﰲأ   ﻣ ﺘ ﻪ‬
and the one appointed by Allah’s

  
Messenger to succeed him in his wa mustakhlafuh} f| ummatih|
nation,

ٰ  ‫ﻷ ْﻣ ﺮ ر ﺑ ﻪ ﺗﺒﺎر ك و ﺗﻌ‬
carrying out the command of his
a‫ﺎ‬  ‫ﻣﺆ دﻳﺎ‬
mu'addiyan li'amri rabbih|
    
Lord the All-blessed and All-
exalted, tab{raka wa ta≥{l{

‫ٱﷲ‬
      ‫ﻃ ﻤ ﺔ ﺑﻨ‬
‫ﻮل‬ ‫ﺳ‬ ‫ر‬ ‫ﺖ‬  ‫وأ   ن ﻓﺎ‬
that F{Çimah is the daughter of wa anna f{Çimata bintu ras}li
Allah’s Messenger, all{hi
ْ ْ ْ ْ ْ
‫ﺤ ﺴﲔ‬  ‫ﻦ وٱﻟ‬ ‫ﺎ ٱﻟﺤﺴ‬8‫وٱﺑﻨ‬
her two sons al-âasan and al-
     
âusayn wabnayh{ alÅasana walÅusayna

are the sons and grandsons of

‫ٱﷲ و ﺳ ْﺒﻄﺎه‬ ‫ﻮل‬ ‫ﺳ‬ ‫ر‬ ‫ﺎ‬ ‫ﻨ‬ ْ
‫ٱﺑ‬
Allah’s Messenger, ibn{ ras}li all{hi wa sibÇ{hu

‫  ﺪ ٰى‬g‫و إﻣﺎﻣﺎ ٱ‬
the two leaders to the true
  
guidance, wa im{m{ alhud{

‫ ﺪا ٱﻟﺮ ﺔ‬V‫ﺎ‬  ‫و ﻗ‬
 
and the two principals of mercy, wa q{'id{ alrraÅmati

ً ‫وأن ﻋﻠﻴﺎ ًوﻣﺤﻤﺪا‬

         
and that ≥Al|, MuÅammad, wa anna ≥aliyyan wa

are Imams, leaders, a'immatun wa q{datun

 ‫ﺟ  ﻞ و ﻋﻼ‬  ‫ٱﷲ‬ a ٰ  ‫ودﻋﺎة إ‬
inviters to Allah the All-majestic
    
and All-high, wa du≥atun il{ all{hi jalla wa ≥al{

ٌ
‫ ﻋﺒﺎ د ه‬. ٰ  ‫ﺠﺔ ﻋ‬ ‫وﺣ‬
and arguments [of Him] against
    
His servants. wa Åujjatun ≥al{ ≥ib{dih|

are entrusted with Allah’s
Messenger, mawd}≥atun ≥inda ras}li all{hi

  ‫ٱﷲ  ﻋ ﻠﻴ ﻪ و‬  . ٰ  ‫ﺻ‬
may Allah bless him and his
 
Household. Äall{ all{hu ≥alayhi wa {lih|

ْ ‫وﻧﻘﺮاﻋﻠ‬ ْ
‫ٱﻟﺴﻼم‬ ‫ﻚ‬
 ‫ﻴ‬     
 
We invoke upon you peace, wa naqra'u ≥alayka alssal{ma

Allah’s mercy, and His blessings. wa raÅmata all{hi wa barak{tih| ‫ﻛﺎ ﺗ ﻪ‬         ‫و ر‬
