1. Respectfully request for conduct of medico-legal examination on the

accompanying specimen purposely to determine whether the submitted specimen are identical
and came from the same person.

a) NATURE OF CASE: Alleged Rape with Homicide

b) VICTIM: ERIN CABAUATAN y BARTOLOME, 21 years old, single and a resident
of Purok 7, Poblacion 2, Santa Maria, Isabela.
c) SUSPECTS:

1. EDGARDO SANTIAGO y TADAS;

2. CLARK,ESPINOSA y RULLODA Alyas BULI;

3. JOMAR GODOY y BRAVO alyas SUOB; and

4. FRONSTONE LIWALIW y CAMPILIS alyas PONKAN

d) TDPO: 10:00 AM of March 3, 2023 at Purok 7, Poblacion 2, Santa Maria, Isabela

e) SPECIMEN SUBMITTED :

e1.One (1) piece of blood stain (swab) recovered from the crime scene
and marked as "ABW 1 '
e2. One (1) piece of dried blood stain recovered from the crime scene
and marked as "ABW 2".
e3. Three (3) pieces of blood swab (standard) taken from the victim place in a
paper bag marked as "ABW 3", "ABW 4", and "ABW 5";
e4. Three (3) pieces of blood swab (standard) taken from the suspect place in a
paper bag marked as "ABW 6", "ÁBW 7", and "ABW 8".
