CONTENT OF THE INTEGRATIVE PAPER
II. Agency Profile/ Community Profile - provides background information of the agency
or community where the student is placed which appropriately gave a context to the
work of the student
Consists of all the following: agency/ PO’s vision, mission and goals; brief history
and nature (type of org/affiliations, etc.); brief structure; framework used/
demonstrated; and programs and services
III. Framework - clearly and thoroughly presents a substantive, inclusive, and appropriate
model developed or applied which guided the whole FI experience
Consists of all the following: perspective/s, theory/ies, purpose and steps or process
accdg. to the SW helping process
IV. Semestral Plan - shows the agreed plan which presents the activities, that correspond
to the objectives of the FI level enrolled in;
Consists of all the following: general goal/ objective; specific objectives; activities
per objective; time frame; needed resources; and success indicators with introductory
narrative explanation
V. Activities and Accomplishments - presents and explains thoroughly and clearly the
accomplished activities with their corresponding outputs and outcomes that met the
objectives of the FI level enrolled in
VI. Problems Encountered and how you resolved them - explains clearly and in detail the
difficulties faced by the student in trying to perform tasks and activities related to FI
