1.0 Objective

This lifting plan explains the equipment and safety procedure of lifting the nested UAS from
ground level to rooftop level for PGB-GTR Nested UAS to ensure the safety of assets and
worker.

2.0 Scope

This lifting plan is to carry out the lifting operation of nested UAS in Ayer Putih City Gate.

3.0 Responsibility

3.1 Manager
1. Responsible for all team operations.
2. Ensure the safety, work process and project execution in order.

3.2 Work Leader


1. Responsible for safety of team members
2. Responsible for safety of asset and equipment in possession
3. Prepare Lifting Plan.
4. To liaise with Lifting Supervisor.

3.3 Lifting Supervisor


1. Brief the lifting operation in Toolbox Talk.
2. Supervise overall lifting operation.
3. To ensure the surrounding for lifting operation is safe and secure.
4. To ensure Safe Working Load (SWL) for lifting gear.

3.4 Crane Operator


1. To ensure the crane is in good condition and safe for lifting operation.
2. To operate crane to carry out lifting operation.

3.5 Worker
1. To help lifting supervisor during lifting operation.
2. To monitor and track blind spot areas during lifting operation.
3. To assist placement of nested UAS.

4.0 Lifting Condition

4.1 Operation Condition


1. The Work Leader and Manager will provide details of all items to be lifted. Including
weights, dimensions, and any specific requirements.
2. The Work Leader must ensure safe access to cranes and workers and ensure the
working area is clear of any obstructions to allow safe lifting operation.
3. No lifting operation during nighttime.
4. Lifting supervisor to ensure the area for crane parking and operation is firm and level
and able to withstand the loads.
5. Lifting operation should not involve the traffic area.
6. Non-authorized person should not enter lifting area and lifting area are barricaded.

4.2 Weather Condition


1. In the event of rain and extreme weather, lifting operation should be suspended until
safe to do lifting.
2. A lifting supervisor and Crane Operator to determine the safe workable wind speed
to ensure safe lifting operation.
3. Lifting operation must stop when there is risk of lightning strikes.

4.3 Ground Condition


1. Lifting supervisor shall inspect the ground condition of lifting area visually and advise
any necessary actions. (Placing mat or plate for stabilize crane etc...)
2. Work leader to enquire any underground service, or vault in lifting area.
3. Lifting operation will be use combination hand sign and walkie talkie as a
communication to operate the lifting.

6.2 Site Condition and Ground Check

1. Overhead cables are located near to fencing. There is ample space for lifting operation
and should not be hazarded to lifting operation.

2. Overhead cables that crossing road are 30 meters apart. Should be enough for
mobilizing mobile crane and jib movement.

3. Area are industrial area, no public crowded. Lifting area are located outside the Ayer
Putih City Gate compound.

4. The entrance road is not the main road. No traffic observed, no need for traffic closure.

Notes for the rated lifting capacity chart
Precautions
1. The rated lifting capacities are the maximum load guaranteed on a firm level ground and include
the mass of hook block and other lifting equipment. The capacities enclosed with bold lines are
based on the structural strength of machine and the others are based on the stability of machine.
2. The working radii as given in the table are the actual values including the deflection of the boom.
Therefore, operate the machine based on the working radius. However, the working radii shown
for jib operations are based on the values obtained when the boom is fully extended (43 m).
Jib operations should be performed on the basis of boom angle only, regardless of boom length
when the boom is not fully extended.
3. The rated lifting capacities for the rooster sheave are equivalent to the rated lifting capacities for
the main boom to a maximum of 4000 kg. At all times the mass of all lifting equipment in use
(including main hook block suspended from boom head)forms part of load and must be
subtracted from the rated lifting capacity.
4. If the boom length exceeds the specified value, the rated lifting capacities for the boom length
above and below the present boom length should be referred to, and the crane should be
operated within the smaller lifting capacity.
5. When using the main boom with the jib installed, 4000 kg plus the mass of hook block and other
lifting equipment, etc., should be subtracted from the rated lifting capacities.
When performing the above operation, do not use the rooster sheave.
6. Critical boom angles for each boom length are shown on bottommost line of lifting capacity table.
If the boom angle is lowered to less than the critical boom angle, the machine will tip over without
load. Therefore, never lower the boom below these angles.
7. The standard number of parts of line is shown in the rated lifting capacity table.
If you work with a non-standard number of parts of line, take 39.2kN (4tf) as the maximum load
on any part of the wire rope.
8. Over front lifting performance without front jack is inferior to over side and over rear lifting
performance. Great care should be taken when transferring from over side to over front since
there is a danger of overloading.
9. Crane operation is permissible up to a wind speed of 10m/s.
Even in relatively light wind conditions, extra care should be taken when handling loads
presenting large wind catching areas.
10. The machine will tip over or be damaged if operated with a load exceeding that specified in the
rated lifting capacity table or not conforming to correct handling.
If such trouble occurs, the machine will not be guaranteed.

PENGURUSAN DAN PENGENDALIAN SELAMAT KREN BERGERAK

Pada menjalankan kuasa yang diberikan oleh subseksyen 27(1) Akta Kilang dan Jentera 1967 [Akta 139], Ketua Pemeriksa Kilang
dan Jentera mengeluarkan perintah khas yang berikut:

1. Dalam Perintah ini, melainkan jika konteksnya menghendaki makna yang lain:

(a) Pengurus adalah orang yang diambil kerja dalam mana-mana perkhidmatan yang melibatkan pengurusan atau
pengendalian bagi, atau penjagaan ke atas, atau kedekatan kepada, mana-mana jentera atau proses yang
dijalankan dalam mana-mana tempat kerja;
(b) Operator kren adalah operator yang disahkan untuk mengendalikan kren yang memiliki sijil perakuan yang
dikeluarkan oleh Jabatan Keselamatan dan Kesihatan Pekerjaan di bawah subperaturan 26(1) Peraturan-Peraturan
(Pemberitahuan, Perakuan Kelayakan dan Pemeriksaan) Kilang dan Jentera 1970.

2. Pengurus hendaklah memastikan sebelum atau semasa pengoperasian, pengendalian dan penyelenggaraan kren bergerak:

(a) kren bergerak mempunyai sijil perakuan kelayakan yang sah;

(b) operator kren mempunyai sijil kelayakan yang sah;

(c) mempunyai penyelia mengangkat (lifting supervisor), juru isyarat (signalman) dan juru tali (slinger/rigger) yang
mempunyai pengetahuan, pengalaman dan kemahiran yang berkaitan;
(d) sistem permit menjalankan kerja (permit-to-work) dilaksanakan;

(e) pelan mengangkat (lifting plan) diadakan;

(f) segala kelengkapan peralatan mengangkat (lifting gear) diperiksa dan diselenggara secara berkala mengikut
spesifikasi pembuat dan amalan kejuruteraan yang baik;
(g) semua peranti keselamatan diselenggara supaya berfungsi dengan baik pada setiap masa dan tidak dikacau
ganggu;
(h) kerja-kerja memeriksa dan menyelenggara kren dijalankan mengikut spesifikasi pembuat serta amalan
kejuruteraan yang baik; dan
(i) rekod-rekod berkaitan penggunaan, pemeriksaan, penyelenggaraan, pelan mengangkat dan permit menjalankan
kerja disimpan dan diselenggara untuk tujuan pemeriksaan pada bila-bila masa.

3. Operator kren bergerak yang dilantik hendaklah semasa pengoperasian, pengendalian dan penyelenggaraan:

(a) mempunyai sijil kelayakan operator kren yang sah;

(b) kren bergerak mempunyai sijil perakuan kelayakan yang sah;

(c) memastikan tindakannya tidak membahayakan dirinya dan orang lain;

(d) menjalankan kerja-kerja memeriksa dan menguji peranti keselamatan pada kren.

(e) menjalankan pemeriksaan terhadap peralatan mengangkat (lifting gear) dan memastikan ia berada dalam keadaan
yang baik;
(f) merekodkan pemeriksaan rutin ke atas kren ke dalam satu buku daftar kren; dan

(g) memastikan kren bergerak diletakkan di atas tapak yang kukuh dan stabil.

Penalti
Mana-mana orang yang melanggar perintah khas ini adalah melakukan suatu kesalahan dan boleh didakwa di bawah seksyen 8(g)
Akta Kilang dan Jentera 1967 [Akta 139] dan jika disabitkan kesalahan boleh didenda tidak melebihi dua ratus ribu ringgit atau
dipenjarakan selama tempoh tidak melebihi lima tahun atau kedua-duanya.

Penalti
