PATIENT:   Is this Doctor Valenzuela’s office

CA: Yes ma’am

PATIENT: Hi, my name is Jessel Tapia, I'd like to make an appointment.

CA: When was the last time you saw the doctor ma’am?

PATIENT:   I've never been to your office before.

CA: Is this to make an appointment for you or your child, and may I know who
referred you to our office ma’am?

PATIENT:  For mine. It’s my friend Alice Smith asked me to call you.

CA: We just love Alice, she referred so many patients to our office! Have you
been to any other doctor for this condition?

CA: Were x-rays taken? Or any lab tests done?

PATIENT: Not yet.

CA:   Okay ma’am, would you prefer today or tomorrow?


PATIENT:   Tomorrow

CA:   Morning or afternoon?

PATIENT:   Afternoon

CA:   Terrific! We have an opening for a new patient tomorrow at 4:00 or 6:30
pm. Which would be better for you?

PATIENT:   6:30.

CA:   Let me take some information from you right now ma’am so we can have
a file prepared in advance to save you some time when you come in.

CA:   Thank you for your cooperation, Ms. Tapia. We look forward to seeing
you tomorrow, Tuesday, March 20, at 6:30pm

(Pediatric)

Tina: Good afternoon. Green Valley Pediatrics. This is Tina. How


may I help you?

Joecelle: Yeah, hi. This is Joecelle Tapia. My son, Tyler, is a patient


of Dr. Panzano. He’s breaking out in hives, and I think he may be
having some sort of an allergic reaction.

Tina: Okay. Dr. Littman is on call tonight. Let me get a hold of her.

Joecelle: Okay, great.

Tina: Okay. And you said your first name is Joecelle. And would
you be able to spell your last name for me?

Joecelle: Sure. It’s JOECELLE, J-O-E-C-E-L-L-E

Tina: Okay that’s Joecelle, J-O-E-C-E-L-L-E

Joecelle: Correct.

Tina: Okay. And your son’s name is Tyler?

Joecelle: Yes.
Tina: Okay. And how old is he?

Joecelle: Eight years old.

Tina: Okay. And he is a patient of Dr. Panzano?

Joecelle: Yes.

Tina: Okay. And is he in any pain, or is he having any other


difficulties?

Joecelle: No, he seems fine. I’m just a bit concerned about the
hives.

Tina: Okay. And can I have a number that the doctor would be able
to reach you at, Mrs. Joecelle?

Joecelle: Sure. It’s, area code 610, 265-1714.

Tina: Okay. I have 610-265-1714.

Joecelle: Yes.

Tina: Okay, I will get a hold of Dr. Littman right away, and I’ll have
her get back to you as soon as possible.

Joecelle: Thank you very much.


Tina: Thank you. I hope Tyler feels better.
