Lucia Ilocos Sur, after having been duly sworn to in accordance with law hereby depose and say thay:

1) I/We am/are the owner of parcel pof lot located at Nagrebcan Sta Lucia Ilocos Sur under TD No./
Tax Declaration No. _2612____

2) That the Local Government Unit of Sta Lucia shall undertake a project in Barangay Nagrebcan
wherein the said parcel of lot under my/our name shall be affected;

3) That I/ we fully understood the significance and benefits of having the said projects not only for
our barangay, but also for the people of Santa Lucia:

4) That I/we am/are executing this affidavit to attest the truth of the foregoing facts and that I/we
interpose NO OBJECTION for the above mentioned project:
