SUBJECT: Notice of Return to Work

Dear Ms. Dopeño,

The matter of your immediate resignation and without proper turnover has been brought to
our attention. In particular, it appears that you failed to report for work from November 7 to
the present. The record shows that your improper turnover and resignation without the
approval of your immediate supervisor were under Absences Without Leave (AWOL). In this
regard, please be informed that unauthorized Absence or absences Without Leave (AWOL)
constitutes gross and habitual neglect of duties, which is ground for termination under the
Labor Code.

On record, you submit your resignation letter by October 28, 2022, with an effective date of
November 5, 2022. The HR department reached out to you to discuss the proper turnover and
resignation process and agreed that you will have your last day by Nov. 16, 2022, but during
the holiday, you insisted again on your last day to Nov. 5, 2022. Thru email, we serve your
last notice that if you will continue your last day on Nov. 5, 2022, you will be tagged as
AWOL and your clearance will be held along with your last payment which includes the
following:
1. Oct. 26 to Nov. 5 – Payroll
2. 13th Month
3. Oct. 17 to 23 Reimbursement
4. Oct. 24 to 30 Reimbursement

Your clearance that was submitted and signed by Mr. Jenard Jimenez is invalid due to the
without approval and coordination with your immediate superior, Mr. Franco Roque. In view
of the foregoing, you are hereby required to get in touch with HR immediately and expect to
return to work – in Jaro, Iloilo by Nov. 9, 2022.

Please be guided accordingly.
