    SUBJECT: Written warning on Habitual Loafing and always


    out of tune during performances
    REPUBLIC OF THE PHILIPPINES
    PROVINCE OF BUKIDNON
    CITY OF MALAYBALAY This serves as a formal warning for your unprofessional conduct
    and habitual loafing.
    OFFICE OF THE
    CITY MAYOR It has been observed and noted that for more than a year you
    just timed in and thereafter you go out without reasonable
    excuse. Further, no permission was sought to the undersigned.
    Also, during performances you failed to deliver what you ought
    to do as Bassist. You are out of tune due to lack of practice and
    preparation.

    Recurrence of the same act after this notice will force the
    undersigned to elevate this matter to the higher ups of the City
    Government of Malaybalay for a formal legal action.
