JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, VICTORIA Q. CUEVAS, of legal age, Filipino, single and presently residing at
Purok 3, Brgy. Bonbon, Butuan City, and CLORENDITA B. GALFO, of legal age,
Filipino, widow, presently residing at Purok 4, Brgy. Bonbon, Butuan City, both after
being duly sworn to in accordance with law, depose and state that:

1. We personally know MARIEVIC FLORES SUAZO of Purok 2, Brgy. Bonbon,

Butuan City, being acquaintance for a long period of time;

2. We declare, confirm and affirm that she used MARIVIC FLORES SUAZO in her
government records since childhood;

3. It was discovered lately that MARIEVIC FLORES SUAZO was recorded in PSA
records as her name;

4. MARIVIC FLORES SUAZO and MARIEVIC FLORES SUAZO refer to one and
the same person.

5. We are not related by affinity or consanguinity to the above-mentioned person;

and

6. We are executing this affidavit to confirm and affirm the foregoing facts and for
whatever legal purpose it may serve.
