IV.SURVEY RESULT FOR SY:2021-2022

School head and teachers will lead and provide guidance in developing the modules.
Information dissemination will be done through virtual announcement, call, text, fb messenger and
signage which posted in school and community premises. The school will explore institutional
partnership with the community, PTA members, and Stakeholders.

Teachers will produce modules and worksheet that will be used by the learners with the
guidance of their parents/ guardian at home. Home school partnership will be strengthen through
information dissemination to parents, by sending home the outline of work activities at the
beginning of each week .Teachers will use modular delivery approach as the learning management
and monitoring system.

Orientation will be a great help to the parents in able for them to be guided what they need to do
in providing time and skills in teaching their child. Especially in kindergarten, grades 1-2 that are
considered as dependent learners. In every orientation (every Saturday of the week), the teacher will
show or guide the parents what to do in every module that they are going to get in the time of their
schedule. An example of this is teaching the parents of the sounds of the letters so that they can do
the same sound for their children. Another way is making interactive videos of teaching how to read
and sending it to the parents through whatever gadgets are available. In addition to this matter, the
school will seek an effort coming from the barangay with the leadership of education committee.
Below are the sample programs of modular learning modalities and the process of delivery plan.

Provide guidance on the submission of new mechanisms in the conduct of enrolment, enhanced
Brigada Eskwela and oplan Balik Eskwela.

 Conduct of survey on the teaching modalities preferred by teachers, parents and learners.
 Learners Enrolment and Survey Form (LESF) and Electronic Learners Enrolment Survey
(E-LESF) serves as an instrument of the school to gather information .
 Conduct online enrolment of face to face submission of documents/other innovative ways to
adhere social distancing protocols for Kindergarten to Grade 6.
 Mobilize partnership, engagement and representation with Barangay Official to maximize
support in conducting enrolment.

V.LEARNING MODALITY

One of the government priority is education. Considering, the current situations, the

Department of Education is guided with an official announcement of the Secretary Leonor M.
Briones that education must continue. San Nicolas Elementary School prioritizes learning as it opts
to render its services to the pupils without inhibitions for we value the importance of incalculating
knowledge even in this time of pandemic.

There were fears among parents, learners and teachers that circulated all over the country
particularly in our locality but its not an excuse to continue the learning of pupils.

The school started its remote enrollment this 16th of September until the 31st, to identify
the needs of the pupils and parents. Learners Enrolment and Survey Form (LESF) is one of the tool
use by the school to ensure that the information to be gathered will help us to decide which
learning modalities would be fit to the learners’ need.

For this reason, the school initially adopts the learning delivery modality which is the
Modular Learning. This type of learning modality is one of the flexible option among others.
DepEd mandates 24th August as the start of classes and implemented a distance learning approach
as a safety precaution. Face to face classes still part of the school plan before the end of the year
considering its implementation depends on the local COVID risk severity classification and
compliance with minimum health standards.
