MINUTES OF THE MEETING

AGENDUM No. 1: Webinar

 For the month of October and to inclined with the celebration of World Mental
Health Day, We the Special Educators’ Club will be conducting a webinar with the
theme of Theme: Managing stress and fighting burnout: A Mental Health Webinar
and the Topic: Burn-Out.
 The Target date of the said webinar would possibly be on the 3rd week of the October
2022, but before that we need to accomplish the following documents that we will
be needing to approve the activity.

1. SAS Form 5/ Letter of Approval of Activity

 I, the secretary Anahiem Elfante will create the SAS Form 5 this
includes the activity proposal, activity rationale, the details, proposed
budget, and the work schedules. For the letter of approval, I also be the
one who will be responsible to create the letter that proposed asking for
permission to conduct a webinar which will be facilitated by the Special
Educators’ Club and the Bachelor of Special Needs Education students
with the guidance of Special Educators’ Club Adviser Mrs. Marie
Charisse C. Jeremillos.
2. Invitation Letter for the Speaker
 The President Mr. Lheno Claud, will be the one who provide the
invitation that guest speaker invitation should include information
relevant to the speaker regarding the event and speaking opportunity.
3. Acknowledgement Letter/Curriculum Vitae
 This will be provided by the guest speaker
4. Flow of Programme
 The flow of the programme was given by Ms. Hyacith Joy Tomulto,
and here is the modified flow:

5. Poster & Teaser
 For the poster and the teaser of the webinar this will be provided by the
SEC SMM-Poster Editor &Video Editor. They need to consult their
works first to the club officers and Ma’am Charisse before posting it to
the facebook page of the club.
6. Certificate Templates
 The certificates will be provided by Ms. Christina Maribojo, the
certificates will be given to the participants of the webinar to
acknowledge their presence during the activity.
7. PPT
 I, the secretary will be the one to create The power point presentation
that will be present during the webinar.
8. Google Forms
 The Special Educators’ Club will be providing two google form sheets,
one is for the registration form and the other one is for the evaluation
form this google form will also serve as their attendance in the webinar.
9. Zoom Background and FB Live Template
 The zoom background and the FB Live template will be provided by
Mr. Joben Macapugay, the participants will be mandated to use the
background. Mr. Macapugay also should consult the other officers first
regarding to their opinions for this matter.
