hereby allow my son/daughter to participate in Project BUILD
(Building and Improving Literacy Development), a school initiative to improve my child’s
literacy skills, from the period of December 1, 2021 to June 17, 2022 to be conducted in
school or any chosen Barangay center.

I am aware of the learning and other benefits my son/daughter will get from
participating in the activities and will not hold the teachers, school, school authorities and
the barangay liable for any untoward incident that may happen beyond their control.
