How to deal with blockage

1-use a short file ( that can reach blockage)
full working length‫مش كتير هاممني اوصل لل‬
blockage ‫ لل‬path‫بهمني اعرف ال‬
concentrated in the apical part ‫ اكتر وبتكون‬force‫ اقصر بكون متحكم بال‬file ‫كمان كل ما كان ال‬
‫ اكتر‬stiff ‫ بتكون‬shorter file ‫كمان ال‬
2-use file size 6,8,10
Size 6 & 8 need experience
We have a file C-file ( for size 6& 8) that have a tip ( extra stiff) so that you can deal
with th broken inst.
so we will use file 10

3- we need a lubricant
debris‫لحتى يخفف ال‬
gel ‫ بس بشكل‬EDTA ‫ وهدول عبارة عن‬Glide or RC preparation ‫منستخدم‬

‫؟؟؟‬liquid form‫ليش ما نستخدمها بال‬

file ‫ رح يوخد اسهل طريق ال‬liquid ‫ عبيناها‬canal‫ بالتالي ازا ال‬dentin soft‫ رح يخلي ال‬EDTA‫النو ال‬
perforation ‫ وبتالقي حالك ب‬dentin‫ويمشي بال‬
final irrigation‫ بس لل‬liquid EDTA‫ال‬
canal‫ منحط شوية وبعدين منحطها بال‬tip of file‫ او عال‬file‫ يا منحطها عال‬lubricant ‫منحط شوية‬
‫‪4-we bend the file‬‬
‫وهاي ال‪ key‬هون‬
‫وبتكون ال ‪ bend‬باتجاه ال‪ Stopper‬عال‪file‬‬
‫كيف طب نعمل ‪ bend‬؟؟‬
‫عندي خيارين يا بستخدم‬
‫‪ file bender-1‬هاي ميزتها بتحدد الزاوية اللي بدي اياها‬
‫‪ortho plier -2‬‬
‫‪ Never‬انك تعمل ‪ sharp right angle‬رح يكسر ال‪file‬‬

‫ايش طب فائدة ال‪bend‬؟؟‬

‫بال ‪ molar‬عادة فيه عندي ‪ curvature‬من االساس صار ‪ blockage‬بسببها لهيك بعمل ال‪ bend‬باتجاه‬
‫ال‪ ( distal‬هيك عادة ال ‪ curvuture‬بال‪ )mesial root of molar‬او تراكمت ال‪debris‬‬
‫معلومة انو ال‪ distal root‬نادرا ما يكون فيه ‪ curvature‬وازا كان فيه بكون ‪ distally‬رايح ولهيك بما انو‬
‫بالعادة هاد ال ‪ root‬بكون ‪ straight‬نادرا ما يصير فيه ‪blockage‬‬
‫ال‪ debris‬ازا تراكمت بس ب ‪ curve canal‬ممكن تعمل ‪blockage‬‬
‫لهيك ال ‪ bend‬فائدتو يمشي مع ال‪curvature of canal‬‬
‫‪5-we need irrigation‬‬
‫انا بدي استخدم ‪ sodium hypochlorite‬هاد رح يوقف تأثير ال‪ EDTA‬وحيطلع عندي ‪ bubbles‬داللة ع‬
‫انو صار ‪ reaction‬وهاد ال‪ reaction‬هو الي رح يفتت ال‪ debris‬عندي‬
‫بدنا نعمل كتير منيح ‪ irrigation‬ونعيد مرة ومرتين وتالت لحد ما نبلش نحس ب ‪catch or resistance‬‬
‫عال ‪ file‬وبصور بعدها ‪ x-ray‬حتى اتأكد اني ماشي مع ال‪ Canal‬ومش بعمل ‪ ledge‬او ‪perforation‬‬
‫ازا ما حسيت ب‪ catch‬اغسل منيح ب ‪ Sodium hypochlorite‬وبعيد الشغل من اول وجديد‬

‫‪Note‬‬
‫هاد ال‪ file‬بتستخدموا بس ‪ 3‬مرات وانت تشتغل بعدها خلص بتكبوا وبتجيب ‪ file‬جديد‬
‫يعني وانت تشتغل خلي علبة الفايالت حدك‬
‫مع الشغل بتبطل ال ‪ tip‬ال ‪ active‬وال ‪sharp‬‬
‫لهيك ال ‪ new file‬هون ‪mandatory‬‬

‫‪File 10 if it’s under a high stress it will broke & you will end with a broken inst.‬‬
 How to deal with ledge??
broken inst. ‫نفس الطريقة اللي منتعامل فيها مع‬
confirmative x-ray ‫ وبضل اعمل‬bending of file ‫ بضل اغير زاوية ال‬file ‫ لل‬prebending ‫بعمل‬-1
‫ االصلية‬canal ‫لحد ما اتأكد اني صرت جوا ال‬
canal‫ النك ما بتضمن ترجع جوا ال‬file‫ ما تسحب ال‬canal ‫ انك صرت جوا ال‬once-2
‫ لسببين‬quarter & pull ‫مش‬short stroke ‫ ب‬filing motion ‫ ومنعمل‬k-file 10 ‫احنا هون منستخدم‬-3
‫ اللي بعده يكون اسهل الشغل فيه‬file ‫ لحتى ال‬canal ‫بدي اوسع ال‬-
copious irrigation ‫ما ننسى انو بدنا‬-4
20 ‫و‬15 ‫ بفايالت اكبر‬filing motion ‫بعدها منصير نعمل‬-5
‫ اخر اشي جملة من الكتاب‬-6
If the file is sliding easily, it is turned CW upon withdrawal because this motion will
tend to straighten the apical one-third of a stainless steel file and allow it to rasp,
reduce, smooth, or eliminate the ledge
During these procedures, try to keep the file coronal to the full working length so the
apical foramen is handled delicately and kept as small as practical.

2mm ‫ منيح كمان منقصر الطول‬irrigation
‫ ما حيصير‬canal wall‫ مع ال‬friction ‫ بهاي الطريقة وما بعمل‬canal ‫ داخل ال‬loose ‫ هون‬file ‫بما انو ال‬
ledge ‫عندي‬
 ‫ازا ‪ coronally‬بشتغل ‪phase 2 then 1‬‬
‫يعني حبلش ب ‪ H-file‬بس شرطي هون انو يكون ال‪ file very loose‬وما عليه ‪ stress‬النو ازا صار عليه‬
‫‪ stress‬حيعمل ‪ledge‬‬
‫طول الفايل بكون لحد ما بصير عندي ‪ resistance‬وبقلل ‪ 1mm‬بس رح نعمل اكتر ‪strokes‬‬
‫كيف اعرف انو خلص هاد ال‪ size of file‬خلصت منو؟؟‬
‫‪-1‬بصير مثال ال‪ k-file‬عندي‪loose‬‬
‫‪ H-file-2‬بعرف اني خلصت من انو بطل يطلع عندي ‪ remnant of pulp‬اللي لونها ‪ dark debris‬بال‬
‫‪ infected pulp‬وبصير يطلع معي ‪ yellow , bright dentin‬هيك بكون خلصت ‪C&S‬‬
‫فيه طريقة تانية انو بستخدم ‪ file 10 , 15‬ويكون ‪ H-file‬وبعمل ‪ filing motion‬وعادي بوصل‬
‫لل‪ working length‬وهيك بوسع لحد ما اتخلص من ال‪ coronal resistance‬بالنسبة لل‪ apical area‬ما رح‬
‫تتوسع من فايل ‪ 10‬او ‪15‬‬
‫وبزيد احجام الفايل لحد ما اتخلص من ال ‪ coronal resistance‬وبعدها برجع لل ‪ step back‬العادية‬

‫كمان اخر ‪ note‬من الدكتور فايالت ‪ 60-30‬انسوهم بعملوا ‪blockage‬‬

‫كمان شغلة بتعمل ‪ blockage‬اللي هي لما نعمل ‪ quarter‬ونسحب الفايل نظف ال‪ tip‬قبل ال ترجع الفايل واال‬
‫حيصير ‪blockage‬‬
‫??‪How to deal with broken inst‬‬
‫هاي طبعا االصعب‬
‫‪ Never‬تحاول تشيلها وانت مش شايف‬
‫ازا مش شايف اعمل ‪bypass‬‬
‫ازا حاولت تشيل ‪ broken inst.‬وانت مش شايف حتعمل كوارث مثال ‪ledge or perforation‬‬
‫يعني الخالصة انت بحاجة ل ‪ microscope‬هون‬
‫‪If the broken inst.‬‬
‫‪1-coronally‬‬ ‫‪it’s better to be removed( canal wide so you can see & you have‬‬
‫‪enough dentin thickness) & removed easily may be by a surgical plier or use‬‬
‫)‪masseran kit (has 12 sizes‬‬
2-if it’s on the middle it depend on the case either bypass or retrival
3-in the apical best choice is bypass
apex‫ برا ال‬broken inst. ‫النك هون ازا حاولت تطلعها رح تطلع ال‬

‫؟؟‬broken inst.‫طب كيف نطلعها لل‬

1-good radicular & coronal access
2-using ultrasonic expose the inst. ( we need to expose 1.5-2mm of the broken inst)
If you beyond curvature you can’t see so don’t try tr retrieve inst.
3-creat a platform by Gates Glidden drill ( be careful bc. This inst. Can cause ledges)
‫ لحتى ما تضعف السن‬dentin thickness‫انتبه عال‬
4-either you see by a microscope or you depend on the tactile sensation
5-using a suitable size of masserann kit you engage the broken inst. Then a CCW
motion then withdrawal

If we will use ultrasonic

Prior to performing any radicular removal techniques, it is wise to place cotton pellets
over other exposed orifices, if present, to prevent the nuisance re-entry of the
fragment into another canal.
Take x-ray to see where the inst.
Be sure you can see & expose inst. Then CCW with ultrasonic tip E7
