TITLE: ORIENTING AND ENHANCING LSES TEACHERS ON THE USE OF


THE PHILIPPINE ASSESSMENT UNIFIED E-FORMS

RATIONALE:

The DepEd has established the various forms that has been established from
previous years as part of the Learners Information. Also, the DepEd School Form 1
(SF1) in the time of COVID-19 for the SY 2020-2021, was established. And consistent
with the Basic Education-Learning Continuity Plan (BE-LCP) as stipulated in DepEd
Order (DO) No. 012, s. 2020, the Department of Education (DepEd) has issued the
Interim Guidelines on the Preparation, Submission, and Checking of School Forms last
School Year 2020-2021. Moreover, about the SF9 also known as the School Form 9
(SF9) or Progress Report Card and what we formerly known as Form 138, the DepEd
has now this so called PAU-E Forms, also known as the Philippine Assessment Unified
–Electronic Forms.

At present, due to the outbreak of Covid-19, several institutions have adopted


various flexible ways of assessing the learners. Flexible learning represents a shift away
from a fragmentary approach to meet the learning needs of learners towards a more
systematic and precise interventions appropriately designed to match the needs of
learners and teachers especially in the conduct of assessment and evaluation.

And because of the mandate from the DepEd, this shift going to the virtual way of
assessing our learners have been pursued in a unified and precise manner using the
PAU-E Forms.

Thus the teachers of our school, Lagui Sail Elementary School (LSES), must
have to be adapted with tool since we are now in the digital world and there will be
no reason not to uphold this mandate. The SDOLC have already initiated such
trainings and webinars which became great tools to achieve the goals of enhancing
our teachers in terms of this aspect.
In a nutshell, this proposed plan for the SLAC of LSES will be conducted via
video conferencing and/or through face to face with proper and maximum observance on
health protocol. This will give the teachers the ideas to acquaint themselves this tool as
distant teachers for a more convenient way in assessing and evaluating our learners
using the PAU-E Forms.

I. OVERVIEW

This SLAC aims to orient the 8 teachers of Lagui Sail Elementary School (LSES)
about the various E-forms under the PAU. Furthermore, the teachers will enhance their
capacities with the aid of such online activities. The proposed date shall be on February
___ , 2022. This instructional SLAC shall have engaging activities such as lectures,
orientations, and discussions as well as making of outputs which will lead by the
speaker.

The people involve are the _____teaching staff and one school head. After the 1-
day SLAC, an evaluation form shall be accomplished by the teacher-participants.
Moreover, such reports (i.e. Accomplishment and Narrative Reports) shall be made for
documentation purposes. The performance indicator shall be used as the basis in
determining the success of the In-Service Training.

II. OBJECTIVES

The SLAC aims to achieve the following objectives:


1. To orient and enhance the _____ teacher-attendees with Philippine
Assessment Unified-E forms;
2. To learn how to adapt and undergo with the mechanisms of these digitally
-interactive tool; and
3. To demo this activity from each of the divided groups of participants.

VII. MONITORING AND EVALUATION

The school head will monitor and supervise the hands on lecture and discussions
of the teachers and follow up their gradual learning and implementations of the ideas
they learned from the SLAC

VIII. PERFORMANCE INDICATOR

After the SLAC, the ____ teachers are expected to plan, utilized, implement and
demo using this electronic tool in their teaching and grading system.

IX. EXPECTED OUTCOMES

After our SLAC all the ____ teachers will be able to:

3. demo this activity from each of the divided groups of participants apply the

various online tools based on their choices;

4. write reflections about their learnings from the SLAC;


5. fill-up Evaluation form; and,
6. draft a narrative report with complete documentations.
