    Good day!

    I, ELLEN FAYE F. PADUA, a member of ANHS MPC would like to request that my CBU deduction
    amounting P200.00 for the month of September 2021 and onwards will not be billed nor deducted
    in my monthly salary. Instead, I would pay my contribution over-the-counter if possible.
