    ASSESSMENT PROCEDURES WHAT TO DO FINDINGS


    INITIATION
    1. Greets patient by name Hi Good Day, I am Luzel Lapuz, your
    student-nurse for today. How would
    you like me to address you?
    2. Introduces self Same with greetings
    3. Explains purpose of interview For us to be able to obtain
    comprehensive information to
    provide a better/ effective
    intervention for you, this initial
    interview is being conducted.
    4. Nature of Interview So this will be an initial interview,
    before we proceed, I am just going to
    ask a few information just to make
    sure that I am speaking with the right
    person.
    5. Information Needed Verification of Demographics (Name,
    DOB, Marital Status, Address, and
    Contact Number)
    6. Confidentiality of data And also, You have nothing to worry
    collected about because all of the information
    that you will be providing won’t be
    discussed with anyone including your
    family and friends without your
    permission aside from other health
    care providers that will be providing
    you the services you need.

    7. Time Frame Please do know that this interview


    will take at least 10 minutes of your
    time, will that be fine for you?
    8. Expectation from the patient Do I have your commitment that you
    will be providing factual information
    that would help us in making an
    effective intervention for you?
    9. Note Taking Lastly, just to inform you that all of
    the information that will be discussed
    in this interview will be recorded.
    ENVIRONMENT
    1. Room should be well lighted • Ask the patient if they are
    and well ventilated already comfortable with the
    setting
    2. Provide privacy • Close the curtains to provide
    privacy

    • Check the room if it is well
