Unsafe conditions

Place: - waiting area for vehicle, opposite

to SBI atm, Sakchi main gate (date: -
21/July/2022).

Reason: - Some part tree bush is growing

on pedestrian area. Result: - During peak
hour it is inconvenient for the pedestrian
to travel in line, they cross the barricade
which arises unsafe condition.

Action: - action taken after 3 days

(24/July/2022), the bush was cut off.
Place: - Grease barrel storage area
in New Bar Mill, (21/July/2022).

Reason: - Empty and half-filled

drum is outside the storage room.

Result: - It was an example of poor

house keeping.

Action: - Immediate mitigation.

Place: - near water box area in
new bar mill (22/July/2022).

Reason: - spillage off water.

Result: - which may lead to slip

fall hazard.

Action: - immediately cleaned.

Place: - looper area, new bar
mill (22/July/2022).

Reason: - The wire guard is

damaged, and the rubber pad
covering is not on it.

Result: - It may lead to trip fall.

Action: - Immediately mitigated

by covering it.
Place: - Path to canteen near
staircase (26/July/2022).

Reason: - Spillage of oil on

path.

Result: - It can cause slip fall

hazard.

Action: - Immediate cleaning

of spillage is done.
Place: - near horizontal

Reason: - Spillage of water,

which accumulate dirt around
itself.

Result: - It can cause slip fall

hazard.

Action: - Immediate cleaning of

spillage is done.
Major problem
PLACE: - NEAR OIL/ GRESS BARREL
STORAGE AREA.
REASON: - As the empty barrel were
checked, for its leakage or and other
deformation by filling water in it, due
to this the water get spilled out and
accumulated near then gate of
storage area.

1. It sets an example of poor

housekeeping.

2. It increases the risk of slip & fall

hazard, and also increase the risk of
incident or near miss during the
loading and unloading of barrel.
 1. Regular site visit is done by contractor in charge, safety
representative, supervisor.

2. Proper housekeeping.
GOOD 3. Workers uses proper PPE inside the New Bar Mill.
OBSERVATION 4. Regular cleaning.

5. Following SOP during performing their work.

6. Adequate warning sign and symbol provided.

7. Proper barricade is given near the running machine.
