The Nationals PHI is insistive in getting our company and make us part of its conglomery, albeit
dissuade to our genuine corporate nature I sustain you know, what is good and right to my
mind!

The first two letters of our noun perhaps is the reason with their acts and if this two letters is
rely the motive; we should stop tolling this two greek letters.

In our first years we are rely tolling ASF/ASS yet inity of our company and advocates of service
being our legal noun albeit we enter the Greek letters noun to SEC dissuade it is first letters of
advocates and severe and I want a Greek noun.

To protect our company corporate status we ought yet toll the complete noun with our
fraternity/sorority yet Advocates of service on our latin noun advocatus serviri
fraternity/sorority bicol university and our unity vet ASF/SDD in our logo oridol,on tarp, and to
every gig we got which our noun is being paraded.

We have to do it to protect our company and inspy despite this is our FRATERNITY/SORORITY
to avoid the ALE NATIONALS we’ve got and avoid the shade of hitler.

3. Certificate of Registration issued by SECURITY EXCHANGE COMMISSION (SEC)
4. List of Current Officers and Members; (please include their contact information
5. Original Sworn Statement stating the CSO is an independent, non - partisan organisation
and that it will retain its autonomy while pursuing the advancement of the people’s
interest through its membership in a local special body, after satisfying all the
requirements and set criteria , as stated in DILG Memorandum Circular No. 2019-72
dated May 22,2019, and after securing a Certificate of Accreditation from the concerned
Sanggunian;
6. CY _____________ Annual Accomplishment Report;
7. CY _____________ Financial Statement;
8. Profile indicating the purpose and objectives of our organization
9. Profile of the Minutes of CY ____________ Meeting of the Organization ; and
10. Photocopy of profiles of at least three (3) individuals in the organization that will verify
their involvement in the health or educator sector. {for CSO applying to be member of
the Local School or Health Board}
