The following are the identified students who need intervention program:

III. Program Implementers (Who shall carry out the program?) Identify their roles and functions. List the
teachers who will be involved in this program, the skills they will teach.)
IV. Procedures/Mechanics (How will the plan be implemented? This is the basis of percentage of program
accomplishment.)
*Plan to be completed 1-2 weeks after the Periodical Test; Plan to be implemented within 2-3vweeks after
the Periodical Test.
*Attach supporting documents of the conduct of remediation like schedule of remediation classes, the
teacher handling the class; list of students; least learned skill taught for the day; formative and summative
test results.
IV. Monitoring and Evaluation (How shall the program be monitored and evaluated? Who will be
monitoring the program implementation? What are their roles and functions? How will they monitor? How
often will the monitoring be? What tools will the use?)
