I. Title: Purchase of Instructional Materials for the proposed Special Education
Class for Learners with Special Needs

II. Date and Venue: San Miguel Heights Elementary School

III. Target Participants: Learners with special needs

IV. Alignment: PPST Domain 3 Strand 3.1 Learners' gender, needs, strengths,
interests, and experiences

V. Rationale: To address the needs of the learners with special needs, we, the San
Miguel Heights Elementary School will create a Special Education Program that
establishes a positive, self-learning environment to inspire creativeness, improve
social skills and self-esteem of our learners.

Thus, to facilitate this program we would like to request for Special Education
Program Support fund for the following Proposed Educational Materials and Budget

VI. Objectives:

a. Terminal Objectives:

 Ensure students with learning disabilities receive specialized

instruction designed to meet their unique learning needs through the
use materials provided by the school.

b. Enabling Objectives:

 Create a Special Education program that aims to meet the needs of

individual students specifically the Learners with special needs.

 Purchase instructional materials for the proposed Special Education

Class for Learners with special needs.
