AFFIDAVIT
(re: Late Registration of Death)
I, AUREA C. FAGEL, of legal age, widower, Filipino citizen and presently
residing at No. 03B Abellera St., Middle Quezon Hill, Baguio City after having
been duly sworn to in accordance with law do hereby deposes and says, that:

1. I am the sister of the late MARTIN CAMBIS BALINGIT, Jr., who was
found dead on April 21, 2022 at his house at Datakan, Kapangan,
Benguet;

2. On the said date, April 21, 2022, my brother’s neighbors were passing-by
his house when they smelled a foul odor emitting from his house and
they decided to check it and when they went inside the house, they
discovered the body of my brother lying on the floor on decomposing
state or maggots all over his body;

3. On the same date, April 21, 2022, the said neighbors who discovered the
dead body called me and informed me about what happened, I
immediately went to Datakan, Kapangan and because of the
decomposing state of the dead body, we buried the following day or on
April 22, 2022 near his house;

4. After the wake and burial, I went to the Local Civil Registrar of the
Municipality of Kapangan purposely to register about the death of my
late brother, unfortunately, the personnel of the said office were all under
quarantine. So, I decided to go back home in Baguio City until such time
that the registration was delayed for how many months;

5. I am now registering the facts of death through delayed registration or

late registration of death;

6. I am executing this affidavit to attest to the veracity and truthfulness of

my foregoing statements and to support the LATE REGISTRATION OF
DEATH of my late brother with the Local Civil Registrar of Kapangan,
Benguet, and for all legal intent and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 13th day of

February, 2023 at Baguio City, Philippines.
