THE MASTER & C/E SHOULD ENSURE THAT THE FOLLOWING SAFETY PRECAUTIONS ARE STRICTLY
ADHERED TO DURING ALL DIVING OPERATIONS
Prior to the commencement of diving operations, has the diving team’s foreman been advised about?

1. Ballasting/de-ballasting operations

(Time, PS/SB, pump capacity, service pumps in use, over-boards, etc)
2. Expected time of barges coming alongside

(For bunkering, water supply, commercial operations, etc)
3. Communication between the E/R, deck & diving boat tested. A deck Officer with walkie-talkie is
positioned above the dive location on deck whenever the diver is submerged. It is agreed that if ☐
communications fail, diver will surface and operation stalled
4. All overboard discharges and suctions, which are not required, are closed. The diver is informed on

the active discharges and intakes positions
5. All valves within the repair system are lashed and tagged ☐
6. The diver must be ready to enter the water at all times when the E/R is open to the sea chest ☐
7. Port authorities have been informed and granted permission for the dive ☐

ANY CHANGES IN OPERATIONS WHICH MIGHT AFFECT SAFE WORKING CONDITIONS SHOULD BE BROUGHT
TO THE ATTENTION OF THE DIVING TEAM’S FOREMAN IMMEDIATELY AND BEFORE STARTING SUCH
OPERATIONS

Have the following safety precautions been observed?

1. International signalling code flag “A” hoisted by day and the relevant warning lights displayed at night ☐
2. Fuses removed from steering gear system, bow and stern thrusters ☐
3. Main Engine’s turning gear engaged and fuses removed ☐
4. “Divers at Work” Posters displayed in vessel’s control room, near Main Engine control panel, near ☐
starting air valves and gangway
5. If fitted, cathodic protection (electrical) system switched off. It must remain switched off until the ☐
operation is completed and the diver will not re–enter the water
6. All bilge pumping equipment are tested and available ☐
6. In case a seachest needs to be blanked the method is approved by the Company’s Superintendent ☐
Engineer
7. The integrity of the overside blank is verified before removal of the repair item. (If drain valves are not ☐
available, testing by cautiously slackening flanges back bolts is acceptable)
8. Inboard blank flanges are prepared, fitted with drain cocks and ready for installation before the repair ☐
item is removed
9. All valves of the system under repair are closed lashed and tagged ☐
10. The C/E is attending the repair location during removal / refitting of repair / spare item. He has ☐
verified the inboard blank flange is watertight
11. Entries are made in the bridge log book regarding operation progress ☐
12. The Master, the senior Officers and Company Superintendent / Port Captain attending the operation ☐
have reviewed the preparations and safety precautions with the diving team and all procedures
including the identification of the work site have been agreed before work commences
13. Any vessel approaching has been warned to stay away from the diving area ☐

After the dive

1. Confirm with diving team’s foreman that all divers are out of the water ☐
2. Make a general broadcast on ship’s PA: “DIVING OPERATIONS FINISHED” ☐
3. Remove posts and signs in bridge, engine control room and ship’s office ☐
4. Remove signs on suctions and re-start pumps as necessary ☐
5. Remove Code Flag “A”, switch restricted lights off ☐
