01. PURPOSE OF THE DOCUMENT

Purpose of this manual is to define the responsibilities and describe the method and documents
to be used to perform concrete segment remedial, if necessary, during the fabrication and erection
of the precast segmenta[ box girders of the portien of Toluca-Mexico Valley Project (Viaduct
V5 and Viaduct V6).

02. SCOPE

This Manual shall apply to all concrete related remedia[ on Design and Construction of Precast
Concrete Bridges of the portien of Toluca-Mexico Valley Project [Viaduct V5 and Viaduct V6).

03. DEFINITIONS

As-Cast surface
Untreated surface in which the mortar is the principle visible constituent and the texture is
that which is imparted by the farmwork surface.

Bug holes
Voids caused by air that is entrapped against the farm.

Cold Joint
a plan of weakness in concrete caused by an interruption or delay in the concreting operations. lt
occurs when the first batch of concrete has begun to set befare the next batch is added, so
that the two batches do not intermix.

Cosmetic Defect
Any kind of damage affecting the aesthetic intent of the single segment.

Crack
separation of a product or portian thereof which may appear befare or after post-tensioning
and mayor may not cause separation throughout the segment thickness or depth.

Defect (Minor or Mjor)


Any kind of damage affecting the durability of the single segment.

Erection Site Repair (ESR):


Construction discrepancies identified and documented at site. Those repairs are best addressed
post erection of the segments

Honeycombing
voids in the concrete. Loss of fines or other material from between the aggregate particles,
the inclusion of air pockets between aggregates, or Larger volume of Lost material.

Medium Scaling
Defined as the Loss of surface mortar up to 1 cm in depth and exposure of concrete aggregate.

05. GENERAL REQUIREMENTS

To ensure that repairing operations are performed in accordance with the requirements of
this
Manual, the following items shall be verified:

Work Methods, including inspection requirements and ITPs which are to be understood by
all personnel involved.
Equipment has to be checked and trial runs has to be performed where possible.

06. ROLES AND RESPONSABILITIES


Organization chart for production of the precast segments shall be attached to this document on
Attachment A. Sorne of roles defined below max be modified to reflect actual organization chart
for the project.

06.01 Project Manager


Is responsible for coordinating construction operations related to the Design and
Construction. He is responsible for safety, Quality and production of the project within the scope
of work of the Subcontract.

06.02 Precast Yard/Erection Manager


Is responsible for coordinating construction operations related to the bridge deck segment casting
including the start-up and production of the Casting Yard. He is responsible for Safety,
Quality and production of the Precast Yard.

06.03 Precast Yard Superintendent


Is responsible for the day to day management of all work performed at the casting facility including
subcontracted work relating to the start-up and construction of the casting facility. He coordinates
all production activities between Mould Foremen, the Storage Superintendent, the QC Field
Engineers. the Concrete Superintendent, and Grane Personnel.

06.04 Precast Yard Engineer


Is responsible for the start-up and construction of the casting facility. His duties include
coordination between the design engineer and QC engineering, as well as, the approval of all on
site design changes. He will also take on all responsibilities related to Geometry Control, and will
coordinate with the Precast Yard Superintendent any delivery schedule changes.

06.05 Project Quality Manager (PQM)


In coordination with the Precast Yard Engineer is responsible for initiatir:ig-Work Methodsea

06.07 HSE Manager


is responsible far reviewing Safety lnstructions to ensure safe conditions far the work perfarmed
under the work methods. and is responsible far reviewing the method to ensure that work is
planned in such a way as to minimize the impact upan the environment and shall comply with all
environmental legislation. regulations, permits, licenses, agreements, and rules applicable to the
project.

06.08 Precast Yard HSE Officer


Is responsible far implementing Safety lnstructions to ensure safe conditions far the work
perfarmed under the work methods

06.09 Concrete Supervisor


Is responsible far all precast concreting operations. from placing and curing, to repair and
patching operations. He/she is responsible far concrete placement Quality as it relates to this
portian of the work.

06.010 Storage/Finishing Supervisor


Is responsible far directing all Straddle Carrier personnel and operations. In coordination
with the Field Engineers, he/she is responsible far carrying out visual inspections fallowing
movement of each segment into yard storage to identify defects requiring repairs prior to
shipping of segments far erection. He/she shall ensure that appropriate instructions are
given to the Concrete Finishers to attend to and complete repairs prior to shipment. He/she shall
also ensure that all repairs that will be performed fallowing segment erection are properly
recorded within the package accompanying each segment.

07. HEALTH, SAFETY & ENVIRONMENTAL (HSE) REQUIREMENTS

This section is generic and could be modified confarm to specific project requirments:

• Safety Vest. Safety Footwear, Hard Hats and Safety Glasses as a minimum PPE [Personal
Protective Equipmentl to be worn at all times.
• Other PPE may be required far specific work.
• Hazard and Risk Assessment [Attachment O) will be regularly reviewed by both
Production and HSE [Health, Safety & Environmentall Department to ensure that all the
hazards have been identified and adequate control measures are in place.
• The repair crew shall clean up and dispose of any excess or waste material into
waste containers which are removed periodically upan the containers being filled

Note:
This list only provides a preliminary overview of resources which can be possibly used
during defects repairing work, so that it does not necessarily prevent personnel to use other
equipment tools.
Only trained operator must work on power tools.
All power tools shall be inspected and sticker shall be provided on it. Power tools must have 'Dead
Man Lock'.

09. CLASSIFICATION OF DEFECTS


09.01 Bug holes

1. Cosmetic bug hole have a depth and diameter less than 1 O mm and are not required to be
repaired.

2. Minor bug hole have an a rea up to 20 cm2 in a rea and a depth up to 3 cm.

3. Major bug hole with a dimension exceeding either of those dimensions will be considered
as a honeycombing.

09.02 Spall

1. Cosmetic spall is a circular or oval depression not greater than 25 mm in depth


not greater than 20 cm2 in a rea. Cosmetic spalls have to be repaired.

2. Minor spall is defined as a spall not larger than 0.2 m2 and no deeper than concrete cover.
Minar spalls have to be repaired.

09.04 Honeycombing

Remove honeycombing in its entirety to sound concrete befare establishing the classification
of the defect.

1. Minor honeycombing is a void no deeper than concrete cover and no larger than 0.2 m2 in
area that results after the removal of unsound material. Minor honeycombing has to be
repaired.

2. Major honeycombing is a void deeper than concrete cover regardless of the surface area,
or shallower but with a surface area greater 0.2 m2 that results after the removal of
unsound material.

09.05 Crack

ldentify cracks by the classifications and locations described below and subject them to the disposition
required by the identified crack.

Regardless of cause, cracks in segments will be identified according to their surface appearance
in accordance with the following classifications:

1. Cosmetic crack: any crack which is less than 0.25 mm wide.

2. Minor Crack: any crack which is between 0.25 mm and 0.6 mm wide.

3. Major Crack: any crack which is greater than 0.6 mm wide.

09.06 Broken Segment Keys

This includes cracking ar damaging of segment shear or alignment keys during the opening the form.

09.09 Cold Joints

As long as the previously placed concrete is still plastic (an internal vibrator will penetrate it), the
next layer of concrete can be safely placed without producing a cold joint.
Whenever vertical lifts of concrete are interrupted or delayed and caused the underlying layer has
stiffened just beyond the point where it cannot be penetrated by the vibrator, the mould will be opened,
concrete will be removed, and rebar will be cleaned.

Oeficiencies are structural and nonstructural defects that require any type of repair or cosmetics to bring
the segment within specification.
Oeficiencies must be identified immediately following removal of the segment from the match cast
position.
Oeficiencies will be recorded and followed upas described on Attachment C - Oefect & Repairs Strategy:

B. NCR [Non Conformance Report) if defect cannot be repaired as specified in this Method
Statement. A repair procedure will be prepared for the specific case deficiency identified.
Note: COR and NCR Numbers will be recorded on Form 2 - Post-pour lnspection Check List
as well.

All materials and the manner in which they are used shall conform to the relevant health and
Safety regulations.

012.04 Material
Use of any repair material is subject to prior approval by the Engineer.
Material data sheets give instruction as to surface preparation and condition prior to application,
material storage conditions, method of mixing, tools to be used, method of application and treatment or
curing after application. Manufacturer' s recommendations shall be strictly followed unless
otherwise mentioned in this Concrete Segment Remedial Method Statement.
Products shall be stored strictly to the manufacturer's recommendations with particular regards
to temperature and relative humidity.
Materials shall not be removed from store until immediately prior to mixing or use.
Materials from damaged containers shall be discarded.

012.05 Remedial Methods for Cosmetic and Minor Deficiencies: Patch


Repair (RM-3)

012.05.01 Surface Preparation (SP)

12.5.1.1. Bug holes


Remove all laitance, loase material or aggregates until sound concrete is reached.
Wire brush and water wash shall be used to remove dirt and cement laitance and obtain a
reasonable degree of roughness without affecting surrounding area.

12.5.1.2. Consolidation-Related Surface Defects, Honeycombs, chips, spalls,


physical damages and the like.
Cut all feathered edges of the a rea to be repaired back perpendicular to (or slightly undercut from)
the surface to the depth sound concrete orto a mínimum depth of 1cm.
lf more than 50% of the surface of the bars found is exposed, chipping of concrete shall continue
until such bars can be finger hooked (about 2 cm behind the far tace of the rebarl.

SSO condition is not required if repairs can be done within 16 hours of pouring the substrate
concrete. In this case water mist shall be applied prior to application of cementitious repair mortar. The
substrate must be free of forrn oil, curing compound or other debris.
SSD condition is also not required far filling of bug hales. Water mist shall be applied prior to the
application of cementitious repair mortar.
SSD condition is not required when a mechanical bonding agent or epoxy is used.

012.05.03 Repair Procedures

12.5.3.1. Preparation
Surface condition should be read in conjunction with the definitions and descriptions given in this
Concrete Segment Remedial Method Statement.
Where required by the manufacturer, concrete surfaces within the repair area shall be treated with a
bonding agent or primer in accordance with the rnanufacturers instructions.
Priming coats or bonding agents shall be thoroughlyworked into all hollows and cavities in the prepared
surface and around the reinfarcement if required by the manufacturer.
Repair mortar shall be applied "wet to wet" when using a bonding aid or primer unless the manufacturer
of the repair system specifies that the bonding aid or primer must dry out befare the repair mortar is
applied.
lf at any time the primer or bonding aid completely dries out befare over-laying, except as
permitted above, the repair surfaces shall be re-prepared generally by complete removal of the dried
primer or bonding aid oras specified by the material manufacturer.

12.5.3.2. Application
Repair mortars shall be applied in layers not exceeding the thickness specified by the manufacturer
of the mortar.
Each layer shall be thoroughly applied as soon as the preceding coat has become sufficiently stiff to
support the weight of the additional built-up layer but is still tacky to provide bonding. The time between
layers shall be in accordance with the rnanufacturers recommendations. lf sagging occurs the material
shall be completely removed and re-applied ata reduced thickness.
lf at any time the last layer applied completely dries out befare over-laying, the surface shall be pre pared
according to the rnanuíacturer's recommendations.
Unless agreed with the manufacturer, the final build-up layer within a repair shall not be less than 1 mm
thick and shall be leveled off or profiled using troweling technique to produce an acceptable finish.

Saw cut around the affected area to a depth of 2 cm to 3 cm; create an inverted wedge type cavity
by cutting atan angle such that the bottom of the void is approximately 1 cm deeper than the top.
Remove excess concrete with a chipping hammer and ensure that the required roughness is present.
Prepare repaired surface according to SP described in previous.
Prepare the repaired surface to SSD condition before application of repair material.
Apply parent concrete to the repair a rea.
Protect the a rea to avoid a repeat occurrence of surface imprinting, and follow with application of
the curing compound utilized for typical segment pours (oran equivalentl.
The repair area may be ground flush to match adjacent elevations once concrete has achieved 75%
of ultimate strength.

012.06 Remedial Method for Cracks: Sealing of Cracks (RM-1 & RM-2)

012.06.01 General
Cracks wider than 0.25 mm will be sealed using a solvent free low viscosity epoxy-resin bonding system.
Detailed method of sealing will depend on result of NCR/CDR investigation by the Engineer and
QC Engineers. During the investigation width at different depths and extent of cracking will be
inspected.
Sealing of cracks shall be carried out as early as practically possible to avoid accumulation of dust and
debris entering the cracks which may cause obstruction of resin flow.

Prior to pouring of the resin, cracks and area in their vicinity shall be cleaned and kept dry using oil free
compressed air.
Putty dam along either sides of the crack may preve to be effective in providing resin feed to the crack.
Cracks shall be completely filled with res in. In arder to achieve a full penetration. pouring may be
made
several times within the resin gel time [pot life).
All surfaces contaminated with sealing materials shall be removed immediately befare curing takes
place.
When work is completed and the resin has cured, the surface shall be reinstated and cleaned
satisfactorily.

012.06.03 Repair Method 2 - lnjection for Sealing of Cracks


(RM-2)

Sealing of cracks appearing in locations other than top of horizontal surfaces shall be made by resin
injection technique. lnjection method is as per following: adhesion ports will be glued directly en
the crack at spacing to be determined by trials and will be dependent en crack width, depth, material &
equipment used and method of application.
In arder to prevent early seepage of the resin during the process of injection, especially in cases of "Direct
lnjection" and overhead application, spaces between the injection ports shall be temporarily surface
sealed along the crack. These seals shall be applied en properly prepared surfaces fer optimum
adhesion.

012.07 Repairs to Broken Shear Keys (RM-4)

012.07.01 General
The QC Field Engineer will review the location and magnitude of the damaged keys and/or the
severity of the spalled area and notify the repair crew of the type of repair, if any, according to
the number of shear keys in the a rea and stress calculation.

012.07.02 Procedure for broken shear keys (shear and alignment keys):
Less than 20% of damaged keys (No spalls
beneath)
Broken shear keys do not need to be repaired at the precast yard. The damaged portian of the
keys should be grinded removing any sharp broken edge prior to erection to facilitate application of
the repair. lf the gap is small, the epoxy glue shall be applied thicker to fill the small gap.
Greater than 20% of damaged

012.07.03 Restore methods


lf the damage at the joints happens prior to match-casting, both on the webs and the slabs, a high-
early strength repair mortar shall be used such that at least 20 MPa strength is achieved befare
the joint is used as a match cast.
lf the damage happens after match-casting. and the damage exceed 20% and the remaining keys
cannot effectively transfer the shear, the keys can be repaired as per the following:
Option 1 - "double female" method: the broken "rnale" keys are dug to become "fernale". Then
the gap between the two fema le keys is scrupulously filled with epoxy (refer to the sketch belowl
after segments erection;

012.08.01 Post-tensioning ducts misalignment [RM-3-6)
In case of post-tensioning ducts misalignment during segments pouring, the possible scenarios are the

Pipe deviation from the correct position is minar or equal to the clearance between the strand and
the duct internal diameter, i.e. 30 mm [refer to the sketch below]. In this case the correct alignment of
the cable is still guaranteed and no repairs are needed;

013. INSPECTIONS AND TEST PLANS


All segments are to be inspected by the QC engineer soon after stripping of the form. All deficiencies are
to be recorded on the Post-pour lnspection as appropriate.

Deficiencies which are classified as major defects are recorded in NCRs or CDRs form by QC
Engineer shall be submitted to QC Manager. The QC Manager will submit NCR together with
proposal to the Engineer for approval.

Upen completion of NCR/CDR repairs. the NCR/CDR shall be signed off as complete in the verification
section of the NCR/CDR by QC Manager/ QC Engineer.

Repairing done without informing the consultant engineer or not in accordance with this Method
Statement will be subject to NCR.

014. FINISHING
014.01 Removal of de-bonding agent
The de-bonding agent shall be a material which self-degrade and thus may not require any
removal. Trace of de-bonding agent remaining, shall be removed by high pressure (300 bar)
water jet. lf not sufficient, use wire brush to remove the residual de-bonding agent.

014.02 Cleaning of surfaces


lf necessary, each segment surfaces shall be cleaned from any traces of mould oil
or dirt.

014.03 Cleaning of PT duct and anchorage


Corrugated PT duct and anchorage of each segment shall be checked. Any slurry shall be
removed.

015. SEGMENT ACCEPTANCE - GREEN DOT PROCEDURE

Each segment will be marked with a Green Dot when below conditions are met:

1. 28 days concrete cube strength verified;


2. Deficiencies repaired [if any];

3. NCRs closed [if any];


4. Erection Site Repair flagged [if any];
5. All checks completed.

Green dots will be applied on segment inner faces only to avoid any potential blemish.

Segments will be inspected by the J&P for final acceptance only after being Green-Dotted by authorized
ROE plant personnel.
Note:Segment final inspection for acceptance will be performed at least a week prior the scheduled date
of shipment to the erection site. This is to allow sufficient time to correct any identified deficiency without
delaying erection activities.
