D.G Breakdown Report 8 - Sept - 2020
01 Executive Summary:
Total of 7 D.Gs (3 were 113KVA rating, one 250KVA, 513KVA, 635KVA &625KVA rating)
had been installed on 05/September/20 at Camp-5 Area to provide temporarily power
supply during the shutdown to the affected customers associated with existing
transformers 5SXF303-1,-2,-3 &-4 and 5SXF304-1,-2,-3 ,-4,-5 & -6 which were intended
for the replacement of old 2KA08/KB08 Sub-Loop cable under existing Outline Agreement
# 7300012205 with P.O.# 7500018803 (D.O.# U205).
1) The D.G. Set (513KVA) associated with 5SXF303-3 (250KVA) transformer was
stopped at around 04:00 pm result in interruption of supply and restore at 4:30 pm
on 06-Sept-2020 after replacement of faulty D.G set.
Note: - The aforesaid affected D.G was re-start again & aging many time due to
frequent tripping (expected problem in generators alternator) before the
replacement.
2) The D.G. Set (167 KVA) associated with 5SXF304-2 was stopped two time
a) First time at around 7:20 am result in interruption of supply and restore at 8.02
am after replacement of damage speed sensor on 07-Sept-2020.
b) Second time the tripping was happened at 1:30 am, 1:55am & 2:15 am result in
interruption of supply and finally rectify & restore at 3.00 am after cleaning
the moisture of speed sensor on 08-Sept-2020.
Given Below are the detail of affected D.G set:
e) Capacity – 635 KVA
f) S.no – FGW RP ES 5KEWA02403.
g) Model – P6 35 P5.
h) Make – F.G WILSON.
02 Introduction:
The cable replacement in Camp-4 & 5 requires D.G. Sets to provide temporary power
supply for the affected residential customers, the aforesaid D.G. Set Cater Pillar Brand &
F.G Wilson with 513KVA & 635 KVA were rented by STICCO from MJI supplier associated
with 5SXF303-3 & 5SXF304-2 respectively.
03 Incident Description:
Detailed description of scene/location:
1) The D.G. Set (513KVA) associated with 5SXF303-3 (250KVA) transformer was
stopped at around 04:00 pm result in interruption of supply and restore at 4:30 pm
on 06-Sept-2020 after replacement of faulty D.G set.
Note:- The aforesaid affected D.G was re-start again & aging many time due to
frequent tripping (expected problem in generators alternator) before the
replacement.
2) The D.G. Set (167 KVA) associated with 5SXF304-2 was stopped two time

Prior to supplying temporary power, below are the sequence of activities which we have
done.
1) The D.G. Set (513KVA) associated with 5SXF303-3 (250KVA) transformer was
stopped at around 04:00 pm result in interruption of supply and restore at 4:30 pm
on 06-Sept-2020 after replacement of faulty D.G set.
Note:- The aforesaid affected D.G was re-start again & aging many time due to

replacement.

Preliminary check was conducted based on checklist below shows all normal.

Immediate causes.
1) The D.G. Set associated with 5SXF303-3 had voltage fluctuations & frequent tripping.
2) The D.G. Set associated with 5SXF304-2 was abruptly stopped.
conclusions:
The D.Gs set used were provide the bad service which result in unsatisfaction to the
Client.
06 Mitigation proposal:
1. STICCO will hire the New D.Gs set on rental basis from the reputed D.Gs supplier
of the Market.
2. The pre-qualification of the D.G supplier shall be submitted along with the date
sheet to Client for review and approval in case if STICCO used again any rental D.G
sets.
3. STICCO also have plan to purchase new D. Gs set of appropriate rating rather than
using the rental one.
4. STICCO will carefully observe the DGs during operation & shall rectify such type of
problem on urgent basis, if occurred.
5. STICCO will increase the frequency of the visits at each & every installed D.Gs.
The above mitigation will help us in reducing risk of undesirable tripping from such type
of occurrence.
