1.0 PURPOSE
The purpose of this document is to detail the requirements pertaining to hot works and its
associated activities for MIE project management, HSSE personnel, engineers, superintendents,
supervisors, sub-contractors and vendors. It also provides for the communication and compliance
with the relevant health, safety, security and environmental information as supplied by the client to
the project management team.
This Hot Work Procedure has been prepared in accordance with the MIE ISO 45001 : 2018
Management System, which is certified and compliant with the said standard and requirements and
the plan will describe the HSSE management system of MIE that will be implemented during the
execution of CIVIL STRUCTURE WORK (Package A) and PIPING, MECHANICAL EQUIPMENT
ERECTION, ELECTRICAL & INSTRUMENTATION, INSULATION & PAINTING WORKS (Package
B) FOR PROCESS AREA FOR PHASE 1 (50 KTPA) FOR PROPOSED 200 KTPA NITRILE
BUTADIENE RUBBER (NBR) LATEX PLANT ON LOTS 2604,2605,2615 AND 2616, MUKIM
TANJUNG DUABELAS, DAERAH KUALA LANGAT, SELANGOR DARUL EHSAN (F44SL NBR
LATEX PLANT PROJECT) FOR TOP GLOVE SYNTHETIC RUBBER SDN BHD.
This procedure applies to work activities and employees under the management of MIE.

TRAINED PERSON A person who, because of qualifications and experience, has the
skills necessary to perform the stated duties.
HOT WORK Defined as “high hazard”, “low hazard”, or “exclusion”. High hazard
includes electric arc, oxy-fuel gas welding/cutting operations, and
heavy grinding. Low hazard includes brazing, soldering using a
flame, light (portable) grinding, TIG-welding, or other similar low-
energy activities. Exclusion activities are those that include electric
soldering, pedestal/small bench grinders, and sanders.
HOT WORK PERMIT A permit, signed by a Hot Work Authorized Permit Issuer, attached to
a Permit to Work (signed by a different Authorized Person) which has
“Hot Work” indicated and allows welding, cutting, burning, or other
specified heat producing activities to be performed.
H.W AUTHORIZED PERMIT A person trained and examined to a knowledge level of, and practical
ISSUER experience in, hot work (welding, cutting, burning) permit procedures.
As a direct result, he/she has been authorized in writing by the

Project Manager to prepare and issue hot work permits and accepts
the responsibilities associated with authorization.

PACKAGE WORK The works, services or supplies, or any or all of the foregoing, to be
undertaken by a CONTRACTOR or PACKAGE VENDOR, as
applicable. "SCOPE OF WORK" may also be used instead of
"PACKAGE WORK" to facilitate the understanding of all the parties.
PACKAGE Means each package of works or supplies forming part or parts of the
PROJECT.
PACKAGE VENDOR Means any Vendor engaged by OWNER to supply Equipment and
Materials or Spare Parts, which may include associated services.
CONTRACTOR Means any Contractor engaged by OWNER to undertake the
PACKAGE WORKS for a PACKAGE.
SUBCONTRACTOR Means any company which CONTRACTOR has engaged or
proposes to engage (as the context may require) to undertake any
part of the CONTRACTOR PACKAGE WORK
4.0 RESPONSIBILITIES
CONTRACTOR is responsible for ensuring that work activities carried out by the CONTRACTOR
and their Subcontractors, Package Vendors and CONTRACTOR employees are conducted safely
and that all personnel of above entities are not exposed to hazards or risk during the execution of
hot work activities.
CONTRACTOR and SUBCONTRACTOR(s) are responsible for performing their work activities
safely and to comply with all the HSE Procedures in force on CONTRACTOR Project, including this
procedure covering hot work activities.
TOP GLOVE have a principle to ensure all activities are carried out in a safe manner with non-
tolerance of non-compliance to these rules.
F44SL 13 Life Saving Rules aim to ensure all high-risk activities are carried out safely and shall be
utilized by MIE to supplement the requirements of the MIE specific HSSE Procedures.
A Hot Work Permit is required for any work which involves use or introduction of heat, sparks or
flame where there is a high fire risk potential.
Hot Work Authorized Permit Issuers shall have sound and thorough knowledge in
matters relating to the preparation and conduct of hot work.
Before issuing a hot work permit, the Hot Work Authorized Permit Issuer must consider
the following:
Equipment isolation requirements, slip plates, physical isolation, locked electrical
switches, etc.
Residual material remaining after isolation
Washing, purging, and testing to verify that equipment is free of dangerous
contaminants
Precautions to be observed
Protective equipment to be worn
Emergency exit route

6.2 Supervisor
The supervisor’s responsibilities include the following but not limited to:
Identifying combustible materials i.e. partitions, roofs, wood, paper, textiles, plastics,
chemicals and flammable and combustible liquids and gases and hazardous areas at
the worksite and prepare the worksite for job.
Do not allow welding on metal partitions, walls, ceilings, or roofs with combustible
coverings or with combustible sandwich-type panel construction.
If combustibles are present in the work area, then either:
Relocate the job
Move the combustible materials at least 35 feet (10.7 meters) away from the
work, or protect the combustible materials within 35 feet (10.7 meters) of the work
by using non-combustible / FR covers, shields, or blankets or, if appropriate, wet
the materials.
Verify or have openings or cracks in walls, floors, systems, and equipment within
35 feet (10.7 meters) of the work covered or plugged as appropriate to prevent
sparks from reaching adjacent areas or provide a fire watch/standby person
below grade.
Verify that the worksite has adequate ventilation.
Where potential exists for flammable concentrations of gases, vapours, liquids, or
dust in the atmosphere, perform appropriate monitoring.
Ensure that a fire watch / standby person is assigned and dedicated to each hot
work activity and have been trained to understand the following:
The area to be fire watched
Potential fire hazards (to personnel and property)
Appropriate emergency procedures and actions
Received documented fire-watch and hands-on portable fire extinguisher
training
Fire extinguisher with sufficient pressure is available at the job site and
usable
Formally trained on fire-watch and hands-on portable fire extinguisher
training.
Hot work personnel’s responsibilities will include but not limited to:

Verify that hot work equipment used (such as torches, regulators, pressure-reducing
valves, and manifolds) is listed or approved by a nationally recognized testing
laboratory for the intended use.
Verify that oxygen-fuel gas systems (such as oxygen / acetylene welders) are equipped
with listed or approved backflow valves, flash arrestors, and pressure-relief devices.
If the hot work is performed on pipes or other metal, verify that combustibles in contact
with the metal are protected from ignition caused by heat conduction through the metal
or that they are too far from the heat source at risk.
After preparing or verifying the worksite, the supervisor will ensure that the necessary
employees are ready to begin work and have the necessary approvals.
A flammable gas test must be carried out before hot work is allowed in a hazardous zone or
confined space. The test must confirm that the atmosphere has a flammability of less than 10
percent lower explosive limit (LEL). Repeat or continuous gas tests may be needed for some
jobs, as prescribed on the hot work permit.
III. Supervisor
The supervisor’s responsibilities include the following:
Maintain the fire watch after stopping hot work for a minimum 15-30 minutes, as
appropriate for conditions longer, as indicated for each situation.
If, during the job, the shift changes or supervisors, hot worker(s), or fire watch /
standby person are relieved, have each on-coming supervisor, hot worker, and
fire watch / standby person review and initial the hot work permit before starting
hot work on their shift.
After the job is completed, if any fire detectors were bypassed, covered, or
removed for the work, ensure that the fire department restores the detectors to
service as soon as possible.
If suppression systems were bypassed or special precautions were taken to avoid
accidental operation, restore the system to normal service.
The following additional steps are required for hot work in or on confined spaces / areas
(such as tanks, small rooms).
I. Supervisor
The supervisor’s responsibilities include the following:
If the job requires hot work on / in piping, tanks, or similar confined spaces used
for flammable / combustible liquids or gases, proceed as follows
Before starting work, complete a documented permit to work and relevant
certificate.
Ensure that the interior of items is cleaned of residue.
Have the atmosphere monitored to ensure that the concentration does not
exceed 10 percent of the LEL. Purging and ventilation may be required to prevent
ignition of flammable atmospheres.
During the work, perform atmospheric testing at least every 4 hours, or more
frequently as required by the hot work permit or JSA / STA.
Ensure that adequate ventilation is provided where employee entry is necessary.
All hot work activities done above the ground and works being carried out the lower level
simultaneously, area below must be barricaded with a fire watch ensuring no workers enter
the area.
Fire blanket / spark fall catchment must be provided to ensure welding spark does not fall to
the level below or get into contact with combustible material.
HSE personnel must be notified by task supervisor and agree on the safest method in
executing SIMOPS.
Fuel-gas hoses and oxygen hoses must be easily distinguishable from each other. The
contrast is made by different colours or by surface characteristics readily distinguishable by
touch.
Oxygen and fuel-gas hoses must not be interchangeable. Single hoses having more than 1
gas passage will not be used.
Gas welding and cutting equipment will be inspected at the beginning of each shift to identify
any of the following defects:
Clogged tip openings on torches will be cleaned with approved cleaning wires, drills, or other
devices designed for this purpose.
Hoses and other equipment must be kept clear of walkways, ladders, and stairs. Torches are
only allowed to be ignited using friction lighters or other approved devices; do not use
matches, flame lighters, or hot work to ignite torches.

Torches and hoses must be completely depressurized (bled) before storage or at the end of
each shift.
Torches and hoses must not be stored in enclosed areas (such as gang boxes or lockers)
while connected to cylinders.
Compressed gas cylinders must be in good condition, not suffering from corrosion and be
properly colour coded to the following requirements:
Oxygen: Black
Acetylene: Maroon
LPG: Red
Argon: Blue
Cylinders and to be individually identified and have a cylinder head cover attached.
When compressed gas cylinders are hoisted, they must be secured on a cradle,
cylinder truck / dolly, sling-board, or pallet; cylinders must not be hoisted using slings
choked around the body nor with unapproved devices or attachments.
When cylinders are moved with powered vehicles, they will be secured in a vertical
position, regulators will be removed, and valve protection caps put in place.
When cylinders are manually handled, unless cylinders are firmly secured on a cylinder
truck, regulators will be removed and valve protection caps put in place before cylinders
are moved in any fashion or by any means.
Regulators must be removed and, when provided for, valve-protection caps put back in
place before moving cylinders.
Exposed current-carrying parts of electrode holders will be insulated in a manner that provides full
protection against electrical shock for arc-welding machine operators.
Arc-welding cables will be flexible, completely insulated, and capable of handling the maximum
current requirements of the work.
Cables will be kept free from repairs and splices for a minimum distance of 10 feet (3 meters) from
the electrode holder.
Exception: Cables with standard insulated connectors, or splices with insulating quality equal to
that of the cable, may be permitted.
If it is necessary to splice lengths of cable, insulated connectors will be used that are equivalent to
that of the cable. If cable lugs are used, the resulting connection will be fastened securely to
provide a good electrical contact; completely insulate exposed metal parts of the lugs.
If electrode holders are left unattended, the electrodes will be removed, and the holders will be
placed so that they cannot make electrical contact with employees or conducting objects.
The power supply to the equipment will be turned off whenever the arc-welding machine operator
leaves or stops work for any length of time, or when the arc-welding machine is moved.
Faulty or defective equipment will be reported to the supervisor and, until repaired, will be tagged
out-of-service.
Hot work will not be performed on piping, ductwork, empty drums, barrels, tanks, or other
containers until they have been thoroughly cleaned to ensure that there are no flammable or
combustible materials present or substances such as greases, tars, acids, or other materials that,
when subjected to heat, might produce a hazard; disconnect or blank off any connection to the
drum or vessel.
Hollow spaces, cavities, and containers will be ventilated to remove gases before hot work
commences. Purging with inert gas may be required as advised by HSE Department.

In enclosed spaces, all surfaces covered with toxic preservatives must be stripped of all toxic
coatings for a distance of at least 4 inches (10.2 centimetres) from the area of heat application, or
the employees must be protected by airline respirators.
In the open air, preservative coatings will be removed a sufficient distance from the area to be
heated to ensure that the temperature of the unstripped metal is not substantially raised.
(Artificial cooling of the metal surrounding the heating area may be used to limit the size of the area
required to be cleaned.) Employees will be protected by an appropriate respirator, if / as needed.

GENERAL
Fire watches / standby persons assigned to hot work duties have a responsibility towards their
fellow employees with regards to minimizing the risks associated with performing hot work.
Before work begins, the fire watch/standby person must be aware of the following responsibilities:
Discuss the job with the supervisor assigning you to fire watch/standby duties.
Confirm that you are clear about:
The work to be done
The likely hazards that exist
The precautions to be observed
Personally read and understand the hot work permit and any other permits associated with
the job, including a Permit to Work, if used on the project/site.
Make yourself known to employees doing the hot work job.
Establish radio contact with the client, if required/applicable.
Assess whether flash protection (such as screens) is adequate for yourself and others.
Position your fire extinguishers in accessible locations.
Be aware that:
If in doubt on any HSE-related issue, stop the hot work and seek advice.
You, or a relief assigned by the Hot Work Authorized Permit Issuer must be in attendance for
the duration of the hot work. If this is not possible, hot work must cease.
If you do stop the hot work due to changes or conditions, you cannot allow it to be restarted
without the agreement of the Hot Work Authorized Permit Issuer who is responsible for the
hot work. This is because conditions may have changed during the time that the work was
stopped.
COMPLETION OF WORK

Confirm that:
All welding sets / generators are stopped and oxygen / methane / acetylene / MAPP
equipment is turned off.
A thorough check is made for fire at job completion.
Advise your supervisor if you use your fire extinguisher and arrange for it to be recharged. If
not used, return it to its original position.
Remain at the site for 15-30 minutes (1/4 to 1/2 hour) — as instructed or on the permit - to
check for a possible fire outbreak.
