I, RAMON T. OMAPAS , of legal age, married , Filipino and a resident of Brgy.

Poblacion , Inopacan, Leyte after having duly sworn to in accordance of law do hereby
depose and state that:

1. I am the President of Maljo and Esperanza Irrigators Assn. , Inc. ;

2. On November 12,2022 the said Association received an ORDER dated 31 August
2022 regarding unpaid annual water charge ;
3. The Maljo and Esperanza Irrigators Assn,. Inc. already tendered its payment on
June 28,2022 through UCPB , a copy of the payment slip is hereby attached and
made an integral part of this Affidavit by way of reference ;
4. The above mentioned Association can not be cited in violation of any order,
rules or regulations ;
5. I am executing this Affidavit to attest to the truth an in compliance with the
Order dated 31 August 2022 and for whatever legal purpose it may serve.
