Accurately!

This module was collaboratively designed, developed and reviewed by educators both
from public and private institutions to assist you, the teacher or facilitator in helping
the learners meet the standards set by the K to 12 Curriculum while overcoming
their personal, social, and economic constraints in schooling.

1 Filling-out Forms Accurately

Have you ever tried to join in a school club or deposit some of your
savings to the bank? What were the requirements that you need to
accomplish? Did the school club or the bank ask you to fill-out some forms?
If you were, then you have an idea of what this module is about.

Forms are documents that you have to fill-out as a requirement for


admission to a school club or to have bank transactions. These are important
because they tell the club or the bank about you.

Before studying this module, look at the following list of forms. Check
the corresponding boxes beside the forms that you are already familiar with
and know how to fill-out.

Notes to the Teacher
Help the learners understand the importance of filling out forms
by guiding them on what to do with the activities that follow.

What’s New

During enrollment time, your parents need to fill-out a registration form


for you. This is a form which asks information about you, like your name, age,
address, and others.

It is always a good idea to read the entire form before we start to fill it
out. Some forms require us to write our last name first. Others require the
first name. Most forms specify that we type or print the information needed.
Others require our own handwriting. Since you are now old enough to know
these, you must be able to learn how to fill out the form yourself.

What is It

Are you ready to learn? Let us discuss the parts of a bio-data in detail.
Some reminders on how to fill-out each item are given. Let’s see how well you
understand and follow directions.

Personal Data

♦ Name- the most important information in any form and refers to your
complete name including your first name, middle name, and family name.
For example, Gani Callos de Jesus.

♦ Sex- refers to your sex that is, either male or female.

♦ Provincial address- refers to your address in the province where you


originally came from. Make sure you include the number of your house,
street, village/barangay or municipality, city, or province. You may indicate
your zip code too. For example, 10 Matiyaga Street, Barangay Kabo, Maryville
Village, Batangas City 4200.

If you do not have a provincial address, you can write n/a which stands
for “not applicable.”

♦ Date of birth- refers to the date when you were born. Make sure you
include the month, day, and year of your birth. For example, October 5, 2008.

♦ Place of birth-refers to where you were born; the city or municipality of


your birth will do. For example, Manila or Calatagan, Batangas.

♦ Citizenship-refers to your nationality. For example, Filipino, American,


Indonesian, etc.

♦ Height-may be expressed in either feet and inches or centimeters. For


example, if you are 4 feet and 6 inches tall, you can write either 4’6” or 137
cm.

♦ Weight- may be expressed in either pounds or kilograms. For example, if


you weigh 105 pounds, you can write either 105 lbs. or 47.73 kg.

♦ Religion-refers to a cause, principle, or system of beliefs you hold on to.


For example, you may be a Roman Catholic, Muslim, Iglesia ni Cristo, etc.

♦ Telephone-refers to the number of your telephone line or the number


where you can be contacted. For example, 064-433-8873. If you have more
than one contact number, you may indicate all of them here.

♦ Father’s name- refers to the complete name of your father. For example,
Ernesto Martinez de Jesus. Make sure you indicate your father’s first name,
middle name, and family name. You may choose not to write his middle name
in full. In this case, you may simply indicate his middle initial. If your father
is already dead, write “deceased” after his name.

♦ Occupation- refers to your father’s job. For example, carpenter, farmer,


etc. If he is not working anymore, write “retired.” Or if he does not have a job
at the time you are filling-out the bio-data, write “unemployed.”

♦ Mother’s name- refers to the complete name of your mother. For example,
Nena Reyes Callos. Make sure you indicate your mother’s first name, middle
name, and family name. You may decide not to write her middle name in full.
In this case, you may simply write her middle initial. If your mother is already
dead, write “deceased” after her name.

♦ Occupation- refers to your mother’s job. For example, dressmaker, cook,


etc.

♦ Applicant’s signature- shows your signature to signify that you have been
truthful in your application.

The following are some guidelines for filling out forms accurately.

1. Make sure that the information you are giving is complete and accurate,
writing an incomplete address for example, may make it difficult for the
company you are applying to contact you by mail.

2. Check if you spelled the words and wrote the numbers correctly.

3. Write neatly and legibly. Do not use pencil. Use only blue or black ink in
filling-out forms. Or better yet, use a typewriter or a computer.

4. Submit forms on time. Find out when the deadlines are and comply with
them.

Keep these guidelines in mind. They will help you fill-out forms
accurately.

Below is a blank form of a bio-data sheet. Fill it out with the necessary
information with the help of an adult in your family. Make sure that all data
are entered accurately. Refer to the scoring rubrics given to you.

Activity 2: Filling-out a Bank Deposit Slip and Withdrawal
Slip
In the previous activity, you practiced how to fill out a bio-data form
neatly, completely, and accurately. You also learned what forms are and
how important they are in your daily life. Aside from bio-data, there are
many other kinds of forms. These include bank deposit and withdrawal
slips.

Read the following and find out about the other kinds of forms we
usually fill out.

Pedro’s Life Lesson

Pedro is the son of vendor who just turned 11 a week ago. His father
was considering teaching him how to fill-out a deposit slip from a bank so
he can help in depositing some of their earnings and learn the value of
saving. So, one day…

Father: Do you want to help me in the store, Pedro?

Pedro: Of course, Father. How can I help you?

Father: See, iho, my sight is not as good as it used to be. I need


someone to help me deposit our store’s weekly profit. So, I’m
planning to teach you how to fill-out a deposit and withdrawal
slip.

Pedro: Sure, Father. I am excited to help you out in filling the forms.
Someday, when I am old enough and have my means of living,
I will work hard and save for my future too. Just like you.

After a few days, Pedro learned how to fill-out deposit slips. His father
no longer needs to write on these forms. Pedro does this for
him.

Do you know what a deposit slip is for? A deposit slip is a written order
to your bank telling it to put funds into your savings account. You can use a
deposit slip to put money such as cash, checks, and money orders into your
account.

Deposit slips vary from bank to bank. Here is an example:

To fill-out a deposit slip, here is what you need to do:

•Fill out the complete date of the day you deposit in the deposit to the
bank.

•Fill-out the name and account number.

•Write the amount of cash you are depositing on the top line.

•Calculate the total deposit. Take the slip and the money you want to
deposit to a teller at your bank.

Withdrawal slips

A withdrawal slip is a written order to your bank instructing it to


withdraw funds from your account. Withdrawal slips vary from bank to bank.

Here is an example:

To fill-out a withdrawal slip, here is what you need to do:

•Fill out the complete date of the day you withdraw in the bank.

•Fill-out the name and account number.

•List the amount of money you want to withdraw.

•Sign the withdrawal slip.

•Take the slip to a teller at your bank.

Identify the missing parts of the deposit slip and withdrawal slip below.

Activity 3: Filling-out a School ID Application Form

Do you have a school ID? Do you remember how the information sheet
for a school ID looks like?

What information are not included in the two previous forms we have
discussed before?

Let’s take a look at them in detail.

♦ Person to be notified in case of emergency—refers to the person you


want to be told in case something bad happens to you. He/She may be your
father, mother, or legal guardian.

The School Opening

It’s the first day of school and I am so excited to meet my new classmates
and teachers.

In this school like any other schools, learners must wear IDs for their
safety and identification. This is a school requirement that learners have
known every year since the first day of class. Administrators and teachers also
always wear IDs around their necks .

“We have to wear IDs so we will be identified, and the teachers will know
whom to contact whenever it is needed. School IDs can also be used to avail
learner privileges when riding in a public transportation such as busses.”

The teacher started to hand over a piece of paper with spaces to write on.
Suddenly I remember my nanay filling-out forms when she enrolled me for the
school year. But I am not so sure if I remember all the information the form is
asking me write.

“Excuse me Teacher Ann, can you help me in filling-out my form. I am


not sure how to do it.”

“Sure, all you have to do is to answer all the details that the form is asking
you, like your name, birthday, address, the name of your parents and their
contact numbers. Just make sure, that the form is filled-out neatly, and all
the texts are readable.”

“Thank you, Teacher Ann.” She sure is helpful. This school year is going
to be so much fun!

Activity 4: Filling-out an Election Ballot

The election for Learner Council


Officers are held each year in all
schools. The types of election vary from
school to school, depending to a large
extent upon the size of the learner
body. Oftentimes, election is done
through secret voting or using a secret
ballot.

Have you ever participated in an


election before? If you have, then you
know how a ballot looks like. If you
have not, then look at a blank ballot
below to know how it looks.

An election ballot is very easy to


fill out. You just must remember the
names of the candidates that you will
vote for. Then just follow the
instructions the election inspector will
give you.
