1. To perform excavation on Near A-26/ M-1 16” leak flow line near Manifold Area.
2. To insert blind disk at Near A-26/ M-1 Manifold Area and isolate 16” flow line.

Equipment: Tools:

1 Leak point lies on the desert open access area; leak pressure formed crater along the
area which is dangerous to motorist day or night time.

1 Work permit must be provided and signed; properly coordinated with Oil Center
Control Room for any additional precaution and advice.

extension, socket cable connections fixed, installed tight; in good working condition.

connector; and between cold cutter pressured hose connector.

8 Double check all lifting equipment to be used, must be free from any defects; check
corresponding SWL, making sure using higher rating than lifted load; use tag lines.

9 Crane must be positioned with free 360-degree rotating access; outriggers fully
extended; on equal footing with pads; with certified operator.

10 Nobody is allowed to stand, walk across the boom or lifted material during lifting
operations; everyone involved in the work must be aware every time.

contaminated sand and put farther away from work area.

12 Pipe cold cutting is required; put enough bentonite on both end-pipes before
grinding and welding; MOG-HSE personnel will do inspection before any hot works.

13 Proper containment of work area by covering fire blanket is a must, top to bottom
especially when doing welding and grinding.

14 Warn all working personnel within work area and keep them away to avoid hitting
by sparks, flying debris and projectiles coming from grinding and cutting.

16 Nobody is allowed to take shelter below the Crane or any equipment be it working
time or rest time to avoid any untoward incident.

18 Enough lighting must be provided for this job that requires 24-hour work activities;
safety signs must be put or barricade the area to warn all vehicles, motorist.
