For the Month July to September of the Year 2023
I. INTRODUCTION
Civil registration in the Philippines has gone through various significant

developments from the time it was legally instituted as a system with the
ratification of Act No. 3753 (Civil Registration Law) in 1930.
As a state policy, the system mandates the basic and compulsory

registration of all vital events and the accompanying changes in the civil status of
every Filipino citizen.

In terms of the system’s structure, the Philippines presents a robust

mechanism, its legally defined operational and administrative setup, from the
municipality, where the unit of vital registration is fixed to the central depository
of vital documents at the National Statistics Office.
November 1930, Act No. 3753 – the Civil Registry Law of the Philippines
took on February 27, 1931. Significant reforms: – Enumeration of acts and events
concerning civil status of person which shall be recorded in the civil register –
The director of National Library was designated as the ex-officio Civil Registrar
General – Except in the City of Manila where civil registration function was
continued to be performed by the City Health Officer, the civil registration
function in other cities and all municipalities was performed by city/municipal
treasurers.
In the present set up the Municipal Civil Registrar of Sablayan is a

department within the Executive government of the Local Government of
Sablayan performing all the functions stated above and considered as one of
the mandatory and frontline services.
CRVS activities in the Philippines are coordinated by the Philippine

Statistics Authority (PSA). It is up to the Department of Foreign Affairs to register
the vital events of the over 10 million Filipinos currently living abroad and
forward this information to the PSA. Since 1930 it has been compulsory to
register all births, deaths and marriages. Marriages must be registered within 15
days, while births and deaths must be reported within 30 days. Registrations are
performed at over 1634 Local Government Units (LGUs) by resident Local Civil
Registry Offices (LCROs). Registration of vital events is usually free, but some
LGUs charge 20 to 50 pesos. In addition, mobile registrations and mass weddings
are organized on occasion. The LCRO checks submitted registration forms for
accuracy and completeness, then records them into a Registry Book, assigns a
Registry Number and uploads the information to the Philippine Civil Registry
Information System (PhilCRIS), a centralized database. A complete Vital
Statistics Report is published every year but with a two years lag for the data.
The Inter-Agency Committee on CRVS was established in 2015. The following
government agencies are represented: PSA, Department of Health, Department
of Foreign Affairs, Department of Justice, Department of Education, Department

of Social and Welfare Development, Department of Interior and Local
Government, Office of the Cabinet Secretary and the Commission on Population.
II. HIGHLIGHTS
II.1 Submission of Monthly Reports to the PSA –PO Mamburao.
-On August 4, 2023, the LCRO-Sablayan submitted the duplicate copies of the
recorded vital events for July, 2023. For the month of July the Municipal
Civil Registrar of Sablayan registered 123 Certificate of Live Birth (58 Male
and 65 Female); 22 Marriages; 40 Deaths (26 Male and 14 Female) and
prepared 57 legal instruments.
-On September 7, 2023 the LCRO- Sablayan submitted its monthly report for the
month of August, 2023 at PSA Provincial Office. For the month of August the
Municipal Civil Registrar of Sablayan registered 128 Certificate of Live Birth (64
Male and 64 Female); 12 Marriages; 42 Deaths (18 Male and 24 Female) and
prepared 61 legal instruments.
-On October 4, 2023 the LCRO- Sablayan submitted its monthly report for the
month of September 2023 at PSA Provincial Office. For the month of
September the Municipal Civil Registrar of Sablayan registered 118 Certificate
of Live Birth (63 Male and 55 Female); 5 Marriages; 37 Deaths (19 Male and 18
Female) and prepared 60 legal instruments.
II. 2 Submission of PHILCRIS Monthly Report
recorded vital events for July, 2023 via PHILCRIS. For the month of July the
Municipal Civil Registrar of Sablayan registered 123 Certificate of Live Birth
(58 Male and 65 Female); 22 Marriages; 40 Deaths (26 Male and 14 Female)
and prepared 57 legal instruments.
On September 7, 2023 the LCRO- Sablayan submitted its monthly report for the
month of August, 2023 at PSA Provincial Office via PHILCRIS. For the month of
August the Municipal Civil Registrar of Sablayan registered 128 Certificate of
Live Birth (64 Male and 64 Female); 12 Marriages; 42 Deaths (18 Male and 24

On the 4th of August 2023, the LCRO Sablayan, submitted its 2nd Quarterly
Report for the year 2023 at the PSA Provincial Office at Mamburao, Occidental
Mindoro;
II.4 Implementation of PSA Batch Request Entry Query System

(BREQS)
For the month of JULY 2023, the Municipal Civil Registrar of Sablayan
endorsed 324 Batch Request Entry Query System (BREQS) at Philippine
Statistics Authority CRS Calapan, Oriental Mindoro. For the month of JULY,
there are 279 BREQS request for COLB, 4 BREQS requests for COD, 16 BREQS
requests for COM and 25 BREQS requests for CENOMAR;
For the month of AUGUST 2023, the Municipal Civil Registrar of Sablayan
Statistics Authority CRS Calapan, Oriental Mindoro. For the month of
AUGUST, there are 228 BREQS request for COLB, 2 BREQS requests for COD,
14 BREQS requests for COM and 14 BREQS requests for CENOMAR;
For the month of SEPTEMBER 2023, the Municipal Civil Registrar of Sablayan
SEPTEMBER, there are 545 BREQS request for COLB, 6 BREQS requests for
COD, 101 BREQS requests for COM and 25 BREQS requests for CENOMAR;
II.5 Implementation of RA 9048 and 10172
For the month of JULY the Municipal Civil Registrar processed 6 petitions
for RA 9048 and 4 petitions for RA 10172. For this month the MCR
received 6 affirmed petitions for RA 9048 and 4 affirmed 10172; and 8
affirmed finality under RA10172;
For the month of AUGUST the Municipal Civil Registrar processed 11 petitions
For the month of SEPTEMBER the Municipal Civil Registrar processed 4

petitions for RA 9048 and 2 petitions for RA 10172. For this month the
MCR received 5 affirmed petitions for RA 9048 and 1 affirmed 10172; 3
affimed for RA 9048 and 1 affirmed finality under RA10172;
III. OVER AND ABOVE ACTIVITIES
lII.1 Conduct of Kasalang Mangyan at Sitio Lindog, Barangay San

Agustin, Sablayan. Occidental Mindoro
On July 4, 2023 the Municipal Civil Registrar of Sablayan conducted Kasalang

Mangyan at Sitio Lindog, Barangay San Agustin, Sablayan, Occidental Mindoro.
Said event was conducted upon the request of IP Members before the Office of
the Municipal Mayor. The activity wedded 6 IP Couples. Also in attendance are
Board Member Atty Tejoso, Jr., and Barangay Officials of Barangay San Agustin

Sablayan, Occidental Mindoro
On July 19, 2023, the MCR represented by Admin Assts Jeremiah Rjhay
Magday, Deson Tapil, Leslie B. Mercado, Judith Cortado, and LLSE Rhoda
Gabres, Phil ID Team, conducted mobile registration at Sitio Duhatan,
Barangay Sto. Niño Sablayan Occidental Mindoro. During the conduct of the
Mobile Registration the MCR were able to cater 22 members of the Indigenous
People's Community whose birth are not yet registered.
lII. 3 Conduct of IEC and Mobile Registration at Barangay Lagnas,

On July 21, 2023, the MCR represented by Admin Assts Jeremiah Rjhay Magday,
Admin Asst Deson Phepps Sy Phepps, Admin Asst Judith Cortado, and LLSE
Rhoda Gabres, conducted mobile registration at Barangay Lagnas Sablayan
Occidental Mindoro. During the conduct of the Mobile Registration the MCR
were able to cater 24 clients who requested PSA Negative and COLB, BREQS
Request.
lII. 4 Conduct of IEC and Mobile Registration at Barangay Victoria,

On July 31, 2023, the MCR represented by Admin Assts Jeremiah Rjhay Magday,
Deson Phepps Sy Phepps, Leslie B. Mercado, Judith Cortado, and LLSE Rhoda
Gabres, conducted mobile registration at Barangay Victoria, Sablayan Occidental
Mindoro. During the conduct of the Mobile Registration the MCR were able to
cater 29 clients who requested PSA Negative and COLB, BREQS Request.
lII. 5 Conduct of IEC and Mobile Registration at Barangay Tagumpay,

On August 1, 2023, the MCR represented by Admin Assts Jeremiah Rjhay Magday,
Deson Phepps Sy Phepps, Judith Cortado, and LLSE Rhoda Gabres, Phil ID Team,
conducted mobile registration at Barangay Tagumpay Sablayan Occidental
cater 34 clients who requested PSA Negative and COLB, BREQS Request and 2
clients under PBRAP.
lII. 6 Conduct of IEC and Mobile Registration at Barangay Ibud, Sablayan,

Occidental Mindoro
On August 23, 2023, the MCR represented by Admin Assistant Jeremiah Rjhay
Magday, Deson Phepps Sy Phepps, Stella Mae S. Nacor, Judith Cortado, and LLSE
Rhoda Gabres, Phil ID Team, conducted mobile registration at Barangay Ibud
Sablayan Occidental Mindoro. During the conduct of the Mobile Registration the
MCR were able to cater 52 clients who requested PSA Negative and COLB,
BREQS Request and 2 clients under PBRAP.
lII. 7 Conduct of IEC and Mobile Registration at Barangay San

Francisco,Sablayan, Occidental Mindoro
On August 25, 2023, the MCR represented by Admin Assistant Jeremiah Rjhay
Magday, Deson Phepps Sy Phepps, Stella Mae S. Nacor, Judith Cortado, and LLSE
Rhoda Gabres, conducted mobile registration at Barangay San Francisco Sablayan,
Occidental Mindoro. During the conduct of the Mobile Registration the MCR
were able to cater 30 clients who requested PSA Negative and COLB, BREQS
Request.

On September 8, 2023, the MCR represented by Admin Assistant Jeremiah Rjhay

Magday, Stella Mae S. Nacor, Judith Cortado, and LLSE Rhoda Gabres Sadural,
conducted mobile registration at Barangay San Vicente Sablayan, Occidental
cater 19 clients who requested PSA Negative and COLB, BREQS Request.

On September 15, 2023, the MCR represented by Admin Assistant Jeremiah Rjhay
Magday, Judith Cortado, and LLSE Rhoda Gabres Sadural, conducted mobile
registration at Barangay Ligaya Sablayan, Occidental Mindoro. During the conduct
of the Mobile Registration the MCR were able to cater 93 clients who requested
PSA Negative and COLB, BREQS Request.
lII. 10 Attendance to 2nd Philippine Identity Summit and 3rd National

Convention on Civil Registration
On September 26-28, 2023 the Muncipal Civil Registrar of Sablayan headed by Atty.
Melvie S. Dacayanan, Admin Aide VI Dessa Gregorio and Admin Aide IV Rio Tulaylay
attended the 2nd Identity Summit and 3rd National Convention on Civil Registration
held on Bacolod SMX Convention Center, Bacolod City, Negros Occidental. On the said
event, the LCRO Sablayan competed on the top 3 Search for Most Innovative Local
Civil Registrar and won first runner up for the competition.
lII. 11 Conduct of Kasalang Bayan at Siburan Hall, Barangay Buenavista

Sablayan Occidental Mindoro.
On September 26-28, 2023, the LCRO Sablayan headed by Atty. Melvie S.

Dacayanan, Rio Tulaylay Admin Aide IV and Desa Gregorio Admin Aide VI
participated in the 2nd Philippine Identity Summit and 3 rd National Convention
on Civil Registration held at Bacolod SMX, Bacolod City, Negros Occidental.
In the said event the LCRO Sablayan was nominated for Top 3 Most Innovative
Local Civil Registrar. LCRO Atty. Melvie S. Dacayanan presented the Innovation
of MCR Sablayan and was hailed as First Runner Up.

On September 29 2023, at about 2 pm the MCR conducted Kasalang Bayan at Siburan

Hall. LGU Building. Barangay Buenavista, Sablayan Occidental Mindoro. The Municipal
Mayor solemnized the marriage of 35 couples. VM Edwin Mintu and SB Members,
Alfredo Ventura, Marfinn Dulay, Mark Anthony Legaspi and Conchita Dimaculangan
stood as witnesses to the marriage.
