AFFIDAVIT OF DISCREPANCY

I, ALEXANDER DE LEON DINGLASAN JR., of legal age, single, Filipino and a

resident of Brgy. Mabutag, Buenavista, Quezon, Philippines after having been duly sworn to in
accordance with law, hereby depose and state:

1. I was registered as ALEXANDER DE LEON DINGLASAN JR. and it is the name I

habitually used in all my records and transactions;
2. That when I applied for PUP College Entrance Test (PUPCET), I inadvertently
missed to indicate the suffix “JR” when I filled up the application form;
3. That the omission is not intentional but merely due to inadvertence and it is not meant
to defraud any person or entity;
4. That, I execute this affidavit to explain the omission of the suffix “JR” in my
PUPCET application.
