Simple Risk Assessment Tool:Exercise #1
Risk Assessment
Practical task: _______________________ Date: 17 January, 2023

Name: _____________________________
MIDN 4CL LIMSON, ANDRE JARED C. Section/Batch: PLUTONIUM 2026
Objective:
The purpose of this exercise is to identify and generate a prioritized list of hazards,
specific to the problem under review. This purpose is achieved by employing standard techniques to
identify hazards which can contribute to accidents, and by screening these hazards using a
combination of available data and judgment.
Direction: Identify at least ten (10) unsafe conditions present in the area assigned to you. Tick the
possible level of risk (5 – severe risk, 4 – major risk, 3 – medium/moderate risk, 2 – minor or low risk,
and 1 – insignificant risk) and the likelihood of occurrence of its consequence(s) (5 - almost certain, 4 -
likely, 3 - possible, 2 - unlikely, and 1 - rare to happen). Hazard interpretations and recommendations
will be discussed in class.
