1 PURPOSE, SCOPE Site Engineer,

1.1 PURPOSE

The purpose of this document is to ensure that the procedures and methodologies
required to deliver the CCTV pipe inspection & survey works are in compliance with
the project specification, QCS2014 and manufacturers’ recommendations.

1.2 SCOPE

In accordance to the Main Contractor requirement, FULLBORE Drilling & Lining W.L.L.
shall submit a Method Statement, describing the general philosophy of the approach
taken that comprises pipe inspection works via CCTV to be carried out in the project of
Roads and Infrastructure in North of Mall of Qatar Area and Celebration Road -
Package 2 (DN110-P02) .

1.3 LOCATION MAP


The Proposed project area Roads and Infrastructure in North of Mall of Qatar Area &
Celebration Road – Pkg 2 (DN-110-P02) is 245ha and situated in west Doha
approximately 25km west of the Central Business District within the Qatar Area
Referencing System (QARS) Zone 51 and 53 taking in the Rawdat Al Jahhaniya, Jery
Al Dabi and Rawdat Bu Dabah districts, within the Al Rayyan Municipality as
illustrated in Location Map.

2 Induction
All personnel entering the worksite shall have received the necessary project
induction. The induction will include site layout, site rules, the identified hazards on
site, emergency procedures, first aid procedures, welfare facilities, safe working
procedures, disciplinary procedures. Additional topics will be discussed in the
induction. All personnel attending the induction will sign the attendance sheet. These
inductions shall be given by the HSE Officer.
Personnel without induction will not be permitted in the work site until given the site
induction and they comply with the safety regulations.
2.2 Training
All personnel shall receive the job specific training according to the site HSE plan and
as per the requirements. Job specific training like confined space entry, confined
space supervisor, rigger, lifting supervisor, first aider, Authorized gas tester shall be
conducted by an Ashghal approved 3rd party training center. All the mobile crane
operator, gantry crane operator and boom truck operator shall be certified by an
Ashghal approved 3rd party. Other all necessary safety training regarding the CCTV
inspection shall be briefed by the site in charge/Safety to execute the work safely.
2.3 Instruction
Site work force shall be carrying out their work under direct supervision by their
Foreman and charge hand for activity to ensure all works are undertaken in a safe
manner and approved procedure.
2.4 Supervision
The site Engineer will make sure the Site foreman and charge hand shall be carrying
out their work under direct supervision of site engineer to ensure all works carried out
according to the Quality and Safety procedures.

3.1 Licenses
All drivers, operators shall have valid driving & operating Licenses/permits from a
relevant concerned department/Authority as per Qatar traffic department requirement.

3.2 Traffic Management


Traffic management plan identifies changes in the working environment, reviewed
and updated by the management as per the work progress. It shall provide all the
necessary information of the proposed works, temporary vehicular and pedestrian
path. Access and egress points for site vehicles shall be clearly marked and this shall
be managed in accordance with the approved Traffic Diversion Plan (TDP).
3.3 Disconnection or Reconnection of Services
Damage to Utilities/Services (if any) shall be addressed to concerned Department
and responsible person. All Stakeholder assets shall be protected or removed with
consent of concerned authorities.

3.4 Procedure to be adopted for Multiple (Mass) Approvals


In case the multiple(mass) approval required like from Kahramaa, Ashghal. Stake
holders shall be arranged in advance approval before proceeding the subsequent
activity.

3.5 Permit to Work
All work executed in the project shall be subject to contractor permit to work
procedure.
The Permit to Work shall be accompanied with a Task Risk Assessment (which shall
be communicated to / discussed with the workgroup), and submitted for prior review
to contractor Site HSE Representative. All permits for the activity (general, hot work,
confined space, lifting activities) shall be applied to the contractor permit coordinator,
and the work will be started only after getting permits from the authorized permit
coordinator.

4.1 General Manager Construction
The General manger is responsible for all departments and project site management. Manager,
Ensures the implementation of Quality, Health, Safety and Environmental Environmental
management system within the level of managers including approval of the continual Manager

communications with the client in all area related to the project.

4.2 Operations Manager


Reports to General Manager and he’s overall in charge of the project. Day to day
coordination with the contractor/client and responsible for Quality, Health, Safety and
Environmental procedure execution.

4.3 HSE Manager


Responsible for implementation and maintaining the project HSE plan and
procedures. Identifying all potential hazards, performing HSE inspection and
implementing required checklist. Ensuring all compliance with safety regulations and
requirement.

4.4 CCTV department manager


Supervises the inspection process from the conceptual stage through to final stage
through planning and managing all on-site activities. He ensures adherence to the
project plans, including quality, safety standards and the defined time scales. He is
accountable for the planning, organizing and utilization of resources on site.

4.5 Project Environmental Manager


▫ Ensure that the environmental requirements are established, implemented and
maintained, in line with the Construction Environment Management Plan
(CEMP), across the Project site activities, specifically including: Identification of
environmental aspects, Implementation of environmental objectives, targets
and environmental management program.
▫ Undertake weekly site inspections
▫ Develop and maintain all environmental documentation associated with site
works, this includes but is not limited to the Construction Environment
Management Plan (CEMP), control plans and procedures, and site records.
▫ Monitor and verify closeout of actions arising from environmental audits.
▫ Establish procedures to ensure that all personnel, including those who are new or
transferred to a new assignment, are given environmental training relevant to their
position and work duties.
▫ Ensure that all managers and supervisory personnel, including those who work for
subcontractors, are trained in the requirements of the Construction Environment
Management Plan (CEMP) and environmental management system.
▫ Ensure environmental accidents/incidents, non-conformances, environmental issues
are managed, and appropriate corrective actions are undertaken in a satisfactory
time frame.

4.6 Public Relations Officer
The Public Relations Officer(s) is responsible for the implementation of the
Stakeholder Management Plan at the project. He has day-to-day management
responsibility for community and stakeholder engagement.

4.7 CCTV Engineer/Operator


Reports to the CCTV dept. manager and in charge for monitoring all inspection
activities. Executes activities by covering all technical functions during the execution
of all activities. Shall ensure that MS/RA are produced, and relevant information is
passed on to front line workers. Shall ensure that all employees whether temporary or
permanent has received the full induction training.

4.8 Surveyor
Surveyor shall make sure and verify all the points are given as per the updated
drawings and if a conflict/deviation shall be reported to the management. And shall
maintain daily records and approved drawings with the latest revision.

4.9 Workforce
Carry out their work under direct supervision by their Forman and charge hand and
under instruction from the site engineer to ensure all works are undertaken in a safe
manner
The workforce to attend any pre-scheduled tool box and morning task brief prior to
starting any new works
The workforce shall receive a full project briefing and site rules induction prior to
commencing any work on site.

4.10 HSE Officer


The Safety Officer is responsible to conduct workplace inspections, to ensure that
safe working methods and safety measures are in place. Rules and regulations are
maintained and adhered with authority to introduce alternative arrangements for the
improvements when applicable and if considered necessary. He will identify and
ensure HSE resources to ensure no harm to people, no property loss or damage to
the environment during project execution.

4.11 Short, Medium- & Long-Term Planning


The Planning Team is responsible for all planning activities on the project in order to
comply the project Short, Medium- & Long-term planning towards the progress of the
work in advance.

4.12 Project progress, monitoring and other issues reporting regime;


Planning team is the responsible for monitoring of the project progress and reporting
as per project requirement.
Progress activity shall be monitoring by daily, weekly and monthly basis to adhere the
contract specification and submission to Engineers for information.
External and Authority related issues shall be communicated to the project
management in advance to circumvent the delaying of procedures complying to the
specific activity at site including inspection plant and methodology towards the
project.

- Scanning System will be the IBAK Panoramo 2 & IBAK Argus 5 Camera system
which are meeting the requirements of the Contract Specification. It can to
inspection, pan & tilt with continuous angle of rotation.

System components:
- BS 7 control system in control room of service vehicle.
- Cable winch in winch compartment of service vehicle.
- Camera cable with cable deflector.
- Camera tractor with or without lift.
- Ancillary light.
- IKAS evolution software CEN 13508-2 compliant or equivalent.
- ARGUS 5 Conventional Pan and Tilt CCTV camera, crawler unit and accessories etc.
- PANORAMO 2 spherical scanner with crawler unit.
- Electric height adjusting unit.
- Camera winch with cable and boom
- Safety tape, sufficient road cones, road barriers, sign boards, flashing lights
- Sufficient manhole lifting keys and proper small tools
- Generator and associated cables
- Blower fans and associated portable ducting
- Safety equipment to include; PPE, BA, vertical lift harness, lifting frame, multi gas
detectors (calibrated), communication.
- First aid and washing facilities.
- A digital camera with a minimum of four mega pixel resolution shall be available in
CCTV inspection van

5.2 Precautions prior to start

- Should any man enter workspace be required to position or retrieve the CCTV
camera units then the confined space entry will be undertaken.
- The CCTV equipment and survey work controlled from self-contained van.
- The CCTV van consists of two sections; one compartment for the camera operator
(Air Conditioned) and the other to house camera, winch and workshop with washing
facilities.
- The power supply will be by means of a generator, supplying 220 Volts for
computer equipment, and power to the CCTV units which is supplied at 48-72 volts.
- The camera operators’ cabin is air conditioned and contain the camera control unit,
monitor, computer hardware with applicable software interfaces, recording
equipment and seating facilities so that the CCTV operators can be viewed by the
Engineer or his representative on a color monitor screen in real time.
- The CCTV inspection period will be based on traffic permits.
- All survey equipment to be checked and calibrated as necessary prior to the
execution of works. All PPE to be checked and where necessary that
calibration/testing certificates are valid, and all equipment is working correctly.
- Pre-site visit to identify and locate the manhole and sewer line to be surveyed.
- All required Permits from Authorities & Traffic Police to be obtained.
- Confined space entry permit to be obtained.
- Traffic management to be implemented, ensure access for emergency services is
available and maintained for the duration of the works
- Sewers to be pre-cleaned refer to cleaning method statement.

5.3 Safety

- All safety measures and precautions shall be taken as defined in the Project Safety
plan prior to commencing the work and shall be maintained during the course of
work. Fullbore will ensure that the operatives whose activities may bring them in
contact with sewerage are adequately vaccinated and that proof of this is made
available to the Engineer upon request.
- All lifting gear unload/load equipment with suitable tackle and lifting gear (approved
shackles, chains, hooks, etc.) and fastened safely lifting equipment, e.g. ropes, to
be checked regularly for visible damages.
- All lifting works will be carried out from experienced and approved personnel.
- All personnel to be at safe distance during lifting processes and ensure that there
aren’t any persons below the lifted object.
- All lifted material will be stored safely on a suitable platform and secured against
unwanted movements.
5.4 CCTV Inspection survey via IBAK PANORAMO 2 system:

CCTV must be done after manhole cleaning & flushed pipes via pipe jetting.

Procedure:
- The PANORAMO 2 will inspect pipes with diameters 200mm and larger.
- The pipe should be pre-cleaned and dry prior to inspection works.
- Survey works will start from upstream manhole and proceed downstream.
- The camera, umbilical cable, winch drum, associated equipment, the
communication interface & operating console will be made ready and tested on site.
- The camera will be lowered by winch into the start manhole and aligned towards
the face of the pipeline.

- Rollers or guides will be provided to ensure that the winch cable and camera cable
are suitably protected from damage at the intersection of the access/manhole and
pipeline.
- Prior to the commencement of the survey, the camera will be positioned at the face
of the sewer line intersecting with the access manhole, any slack in the cable will be
taken up and the camera meterage counter will then be set to zero.
- Once the camera is correctly positioned, and prior to the commencement of the
actual survey the CCTV operator will start recording the video image from the
camera and will enter the following header information onto screen.
- When this has been completed, the operator will advise the CCTV assistant that he
will commence the survey works.
- The CCTV assistant will ensure the smooth operation of the cable drum, and cable
feed supplying the camera unit.
- The header information to be recorded prior to the commencement of the actual
survey will include all the following information:
• Contract Number
• Client
• Date of survey
• Start manhole/end manhole references
• Street name/area/sector
• Survey direction
• Pipe diameter/size and usage
• Pipe material
• Pre-post survey
• Camera operator
• Attendants
- Once the survey starts, the metering information will be displayed on screen in a
position that doesn’t affect the image of the pipe.
- The survey is finished when the camera unit has travelled the length of the pipe to a
position at the juncture of the pipe and the recipient manhole. The survey for pipe is
now complete.
- While proceeding to the recipient manhole, a recording of the internal configuration
and condition is recorded for documenting purposes.
- The recording of the survey is stopped by the camera operator and once the
operator has confirmed that all the necessary data has been recorded, the
inspection work can be considered complete.
- The camera is withdrawn back through the pipeline, the cable operator will ensure
that the cable is not snagged and is fed neatly onto the cable drum.
- The camera is left running during the withdraw stage to ensure that I any problems
occur a record can be made for reference purposes.
- Once the camera has been retrieved the power will be shut down and the unit
replaced in the CCTV vehicle.
- The manhole covers will be replaced, and all tools and equipment cleaned.
- The site must be left tidy
- In the event that the camera cannot proceed to the recipient manhole, due to an
obstruction, blockage, hole in the pipe invert etc. The Engineer will be informed, who
may give instruction to abandon the survey and provide further instruction to
complete the survey for that pipe in an upstream direction commencing at the
downstream manhole.

5.5 CCTV Inspection survey via IBAK ARGUS 5 pan & tilt camera:
CCTV must be done after manhole cleaning & flushed pipes via pipe jetting.

Procedure:
- The conventional Pan and Tilt CCTV camera system will be used for inspecting of
pipes with diameters 400mm to 600mm.
- The pipe should be pre-cleaned and dry prior to inspection works. Survey works
will commence at the “upstream manhole” and proceed ‘downstream’.
- The camera together with its umbilical cable, winch drum and associated equipment
together with the communication interface with the operating console will be made
ready and tested on site. the camera will be lowered by winch into the start manhole
and aligned towards the face of the pipeline. Rollers or guides will be provided to
ensure that the winch cable and the camera cable are suitably protected from the
damage at the intersection of the access /manhole and pipeline.
- Prior to the commencement of the survey, the camera will be used to record the
internal configuration of the manhole for the record the internal configuration of the
manhole for the record purposes. To avoid the potential for the image distortion the
camera will be aligned so that its axis coincides with the ‘centerline’ of the pipe to
be inspected. It will then be positioned at the face of the sewer line intersecting
with the access manhole, so that the circumference of the pipe fills 75% of the total
image to be recorded. Any ‘slack’ in the cable will be taken up and the camera
meterage counter will then be set at zero.

- As the camera proceeds through the pipeline, the operator will pause the camera
at, but not limited to the following:
• Structural / Service operational Defects
• Obstructions
• Longitudinal and circumferential cracks and fractures • Defective pipe
joints
• Changes in pipe diameter and pipe material
• Occurrences of infiltration and exfiltration
• Deviation of the pipeline
• Junction and connection
• Any other defects that affect or have the potential to affect the structural or
service condition of pipe
- The observations will be recorded; photo capture of significant defects will be
taken.
- On arrival at finish manhole, the camera will be paused so that the end of the pipe
fills 75% of the image recorded, the actual length of the pipe will be recorded. The
camera will then proceed into the recipient manhole so that internal condition can
be observed.
- The recording of the survey is stopped by the camera operator and once the
operator has confirmed that all the necessary data has been recorded, the
inspection work can be considered complete.
- In the invent that the camera cannot proceed to the recipient manhole, due to an
obstruction, blockage, hole in the invert etc. the Engineer will be informed, who
may give instruction to abandon the survey and further instruction to complete the
survey for that pipe in an upstream direction commencing at the downstream
manhole.
- The camera is withdrawn back through the pipeline, the cable operator will ensure
that the cable is not snagged and is fed neatly onto the cable drum. The camera is
left running the withdraw stage to ensure that if any problems occur a record can

be made, and the unit replaced in the CCTV vehicle. The manhole covers will be
replaced, and all tools and equipment cleaned. The site will be left tidy.
- Quality Control Procedures for CCTV Survey and Inspection works will include:
• Inspection request
• Linear meterage check
• Calibration of camera and deflection measurement device
• Calibration of meterage recorder
• Camera setup position according to pipe diameter
• Focusing
• Camera speed control
• General photo quality
• Full description for any survey abandonment (SA)
• Circumstances that require the survey to be repeated
• Coding Procedure in accordance with EN 13508-2
• Recording/marking CD procedure as specified
- The recorded data will be transferred to the project office by portable memory unit
where it will be reviewed, and a report will be generated, and recommendation will
be written up and presented to the Engineer.
- The reporting will be carried out by staff that have training from the software
author (IBAK) in the use of the IKAS32 reporting system in addition to WRC
coding system certification.
- Fullbore is implementing an internal QA system to check the quality and
consistency of the coding works.

5.6 TRAPPED CAMERA ACTION

- Preventive action is the first step in order to avoid such an occurrence.


- The camera operators are trained to look out for potential hazards such as
collapsed sections, reduction in sewer diameter, and acute change of direction
and will consider abandoning the survey due to the potential risk of the camera
becoming stuck.
- If any of the above situations are observed during camera operations, the operator
will liaise with the Engineer with a view to withdraw the crawler camera. The
engineer, who may give instruction to abandon the survey and provide further
instruction to complete the survey for that pipe commencing at the opposite
manhole.
- In the unlikely event that the camera becomes trapped in the sewer whilst working
and the camera is unable to forward or reverse, then a proposes solution / action
will be submitted to the Engineer which will be based on the individual
case/situation.

5.7 REPORTING

- If using a conventional CCTV system, the CCTV operator will manually code the
defects observed as the survey progress and codes to be recheck in the office.
- When using the spherical system, the scanner provides a 360-degree picture in
digital format, which is recorded and then transferred back to the site office for
interpretation, coding and preparation of reports.
- The reports will be compiled by trained person to the standard.

All design approval related to stakeholders to be coordinate and approval in place

alterations in project requirements as work progresses. The CEMP and the health and Manager, CCTV
safety plan will take into account, the following minimum requirements: engineer, CCTV
• Effluent from works shall not be harmful, or cause deposits contamination in Operator
drains and sewers.
• Existing services shall not be damaged and are to be maintained in good order.
• Effective action is required to prevent the general public and occupiers of
adjacent land and buildings being affected by any danger, disturbance,
discomfort, trespass or nuisance.
• All warning signs, fences and safety precautions will be implemented before
starting the work.
• All applicable environmental regulations and standards in this regard shall be
strictly followed by QBS.

MANDATORY PPE FOR THE WORK ACTIVITY Engineer, HSE
PERSONAL PROTECTIVE EQUIPMENT officer
9.1 Shall provide required Personal Protective Equipment to the worker, which is
required by legislation, and additionally where it has been evaluated as
necessary for the safe conduct of work where no other more effective control
measures are appropriate.
9.2 The Personal Protective Equipment will be provided to all workers at no charge to
the employee.
9.3 Shall ensure that workers are trained in the proper use adjustment, limitations,
maintenance and storage of the issued PPE. The instructions shall include
details of the method of obtaining replacement for lost or damage items.
9.4 Approved safety helmets, safety shoes, with steel toe caps, Safety glasses,
gloves, ear plugs, and coverall are mandatory. Additional equipment may be
required according to a nature of work.
Issue of Safety Helmets is restricted by color as follows;
• White – Project Manager, Engineers, Foreman, Site Staff, Charge hand, Safety
and all visitors
• Red – Rigger
• Yellow – Workforces

10.1 EMERGENCY RESPONSE CAPABILITIES
• At commencement of work on site an Emergency Evacuation Procedure will
be explained for all Employees and this will be developed as the work
Progress. This will include Escape Routes, Area of Site Operation, and
assembly Areas off-site for Employees Accounting.
• Before any Major Operation commences on site engineer together with
the Safety Officer/Site Engineer will ensure that the safe access for the work
location is planned and maintained throughout the operation.

10.2 EMERGENCY EVACUATION PLAN / PROCEDURE


In case of any Emergency, the following should be followed a)
Stop all work!
b) Shut down all equipment.
c) Inform the supervisor and colleagues in a loud voice to alert them for the
hazard
d) Siren will give a long-repeated horn to alert the workers.
e) Evacuation of workers by means of Man-Basket if necessary.
f) f) If in the high level:
• Use the proper access way
• Never ever think to jump
g) Move to the Emergency Assembly Point
h) Do not run!
i) Stay in the assembly point until further instruction addressed.
j) The senior person available on site should control the situation and must
report the case immediately to the top management.

QUALITY CONTROLS
QA/QC Engineer
The Quality assurance is based on the PQP for this operation. The use of the correct
tools / equipment and material in a suitable environment is an integral part of the
quality assurance plan that will be implemented.

Reports shall be generated incorporating with the following major elements of


information
- Mentioning the type of construction also mentioning the phase of the feature
of work (preparatory, initial or follow up).
- Location and time at which control activities and /or check test were made.
The result of these control activities should be stated in detail. If deficiencies
are found, it should be listed along corrective actions that have been or will be
taken.
- Results of all tests, the results of all tests should be attached along the
procedure used to conduct the test. The result should be included the details
and corrective action taken. If results can't be submitted on the same day with
the QA/QC report, the fact that tests were performed should be reported with
an expected due date for the test results. Delayed test results are then
submitted with the QA/QC report on the date of receiving the results.
- Submittal Information, information should include submittal approval and
delays or predict covered by incorrect submittal.
- Material and Equipment, QA/QC report should be used as a monitoring tool
for materials and equipment. Accordingly, the following process should be
considered:
 Inspect material and equipment for integral part of payment work.
 Prepare material inspection for records material and equipment and
submit to the Engineer.
 Report information on construction compliance of material and
submittal approval for damaged and storage information for each
piece of equipment or type of material received.
- Off- Site Surveillance Information  The status of Work.
 A list of control action
 Estimating the date of the next action.
- Verification, the report must be verified and signed by the supplier and /or
subcontractor’s representative. The verification must contain the statement
that, except as noted all suppliers of material and equipment incorporated in
the work comply with contract requirements.

Plan. The overall supervision and direction of the work will be carried out by the
Construction Manager or designated Project Engineer

Fences, Hoardings, Handrails, etc., to protect members of the public, as may

unauthorized persons out of construction areas.
LOCAL REQUIREMENTS
• All local Authority Requirements in terms of Safety and security will be comply
with project requirement and Sub contractor and Suppliers.
• The Rules and Regulations of the Ministry of Electricity and water will be
strictly observed, particularly in relation to Electrical Installation and
Connection to Government Utilities
