Each member of the governing board is requested to inform if they have any conflict of interest before
participating in the aforesaid meeting and making decisions regarding the business of the institution.
Further, if any member of the governing board is unable to attend the ensuing Board meeting, he/she may
inform the Board Secretary or the Commissioner’s Executive Assistant before the date of the meeting by
sending the signed leave of absence application (please see attached document).

Reminder to the Board Secretary:

In line with CHED Administrative Order No. 6 s. 2014, the Board Secretary is directed to facilitate in the
preparation of all requirements for the Board Meeting following the Standard Format for Agenda of SUC
Governing Board Meetings; and submit the following documents, either in hard or electronic copy, to all
Board Members at least five working (5) days before the Scheduled Board Meeting:

a. A copy of the Meeting Agenda Folders which includes the Complete Staff Work and other pertinent
documents in reference to the Order of Business;
b. The Matrix of Implementation of Previous Resolutions; and
c. Minutes of the Previous Meeting

Any subsequent submission of documents related to the provisional agenda beyond the deadline shall not
be considered as compliance by this office, and therefore shall not be accepted as part of the Agenda Folder.
In the case of the scheduled 1st Quarter 2023 Governing Board Meeting of the following SUCs, the deadline
for the Board Secretary to submit the abovementioned items to all Board Members are:
