C. Interviews personally the head of the household member and determines if there
are household members who are engaged and operating agricultural, aqua cultural,
and fishing activities. Assigns operator serial number to all household members
indentified as agricultural, aquaculture, and /or fishing operators. If there is any 100 %
member of the household identified as operator, continues interview to record the
demographic characteristics of the household members and engagement in
agricultural,aquaculture, and fishing using CAF 2-Household Questionaire;
D. Interview each agricultural, aquacultural, and/or operator in the household using
CAF FORM 3 – Core Questionnaire on Agriculture, CAF FORM 4 Core on 100 %
Agriculture and /or CAF FORM 5 Core Questionare and Fishing.
E. Accomplishes the sticker of CAF Form II – Notice of listing/enumeration and posts it
on the door or any parts of the building/housing/unit ot institutional living quarter 100 %
SEPTEMBER 04 where it can be seen of Notice.
TO F. Informs the TS immediately about the problems encountered in the Barangay/EA 100 %
NOVEMBER 15 G. Ensures that all computer assisted personal interviewing (CAF) data files are
sent/uploaded to the server, whenever internet/data connection is available/fishable 100%
H. Ensures that all accomplished/collected and edited paper and pencil personal
interview (CAPI) questionnaire are encoded in the table/Uploaded. 100%
