NOW, THEREFORE, be it RESOLVED, as it is hereby RESOLVED by the

Sangguniang Panlungsod ng Valenzuela to AUTHORIZE Honorable WESLIE T.
GATCHALIAN, in his capacity as the City Mayor of Valenzuela, to enter into and sign
the Non-Disclosure Agreement with the PHILIPPINE HEALTH INSURANCE
CORPORATION relative to its application for accreditation of its health facilities
within Valenzuela City as Konsulta Benefit Package Provider and for other purposes.

RESOLVED, FURTHER, that the Honorable City Mayor is hereby authorized

to sign any and all cuments relative to the implementation of this Resolution
