On Sunday 27th November 2022 at approximately 4:30 PM I reached the EMCO Barzan Yard for punching out as

usual routine, when I step down from the vehicle where suddenly I showed that there were 6 people of Civil Call-
Off discipline, performing barrel cleaning and painting work then apparently I approached them to check whether
they are working under a valid permit or not as the Yard has recently been fallen-added under the work permit
system, for there we have a clear instruction from the Client Qatargas that the work permit system, now applicable
to all the yard premises which there we had conducted a special TBT Meeting to cascade down this changes among
all personnel.

Then I asked who had given you instructions to come here in the yard to do these activities, they replied that we
have been working at BFOC-2 since the morning until 3:30 then Mr Azhar, our Permit Holder instructed us to go
there in our yard we have a certain type of work to be started then the workers said that we asked PH that we have a
permit or not, as per workers, PH said that I have confirmed with our supervisor Mr. Ashok he said that we have a
permit for that specific work and area.

On the spot, I immediately called Mr. Azhar to verify whether the workers telling truth or not. The PH said Yes I
have given them instructions on that particular thing because my Supervisor Mr Ashok has told me repeatedly to
execute those activities and he aslo authorized me to use his name if in case of anybody will ask either from EMCO
or QG.

So then I called Mr. Ashok a few moments later to verify why he has given the wrong instructions to the PH to start
the activities without a valid permit and pre-task meeting, he was stubbed and said Yes I have given this instruction
to PH and you can issue me an improvement slip then I said Okay tomorrow morning we will discuss these things
and I disconnected my call.
