1. Objective:

SINOHYDRO (SPARK) management has thoroughly observing the dropdown month of


April CSI score graph, we analyzed the root cause and identified in this document the
corrective action required to improve the CSI and to avoid Loss Prevention Department
findings and furthermore to improve the CSI score.
2. Action:
SINOHYDRO HSE team conducted the counseling session counselled with the site
safety officers, Site Engineers, Work Permit Receivers, and foreman & emphasized the
need to improve in coming months. Weekly Joint meeting with SAPMT to analyze the
gaps to improve the CSI Score up to work in future.
SINOHYDRO HSE Department responsibility on improving the CSI score through internal
site audits and inspections and highlighted unsafe acts and conditions.
SINOHYDRO is committed to work Safely In-compliance with Saudi Aramco CSM
Standards, General instructions & Procedures to ensure the effective control and
mitigation of workplace from hazard on all the job site.

• Site Supervision and Safety Staff Trainings as per CSAR 8.6.


SINOHYDRO HSE department will arrange the Required Safety trainings as per
rd.
CSAR 8.6 section A & B through approved 3 party.

Corrective Action (Training was conducted by 3rd. party and evidence submitted
through FSIR closure certificate for the required personals).

04. Weekly Walkthrough (Company & Contractor).


SINOHYDRO Management started Weekly HSE Walkthrough with Company &
Contractor Management team to ensure and observed the areas need further
improvement on every Tuesday 8:30am to 09:30am the site safety compliance and
conducting weekly safety meetings with SAPMT.

05. Disciplinary Action and Penalties on failure to comply safety requirements.

All Site Engineers, Supervisors/Foreman, safety officers and WPR will be held
responsible and charged equal penalty and warning as per “SINOHYDRO Disciplinary
Action/ Procedure” in case of any safety violation (unsafe acts or unsafe condition) by
their crew / area, especially below listed but not limited, to the action requested by the
HSE department. whereas on good safety practices reward and appreciation certificates
will be issued by the management on monthly basis.

06. Appreciation and Incentive Program.

On the other hand, contempt of Strict disciplinary actions, an appreciation and incentive plan
will be exercised as per below and company policy. Contractor is going to encourage the
workforce who contribute to enhance safety compliance at site first week of every quarter.
Any employee/worker (despite of position) observed on site with good safety practicing
during ‘Safety campaign’ by SAPMT / SINOHYDRO management will be nominated for
appreciation letter with incentive money.
HSE team will nominate the workers / employees from their work site on monthly basis, and
every quarter, (5) best safety workers will be selected and rewarded appreciation certificate
with incentive during “Quarterly Safety Award Ceremony”.
SAPMT may also suggest the names for safety award.
Appreciation awards selection / nominees will be finalized by HSE Manager/ HSE committee
base on below important points.
1. The worker has no warning / disciplinary action previously/
2. The worker has no previous record of safety violations
3. The worker will not be nominated second time within (3) months.
4. The nominee is a good practitioner of PPE’S and safety rules and regulations.
5. The nominee is following safety guidelines.
6. The nominee is not involved or observed in unsafe acts and conditions.
7. The nominee is supporting to spread safety culture.
8. The nominee is often observed with positive safety practices.

07: Stop Work Authority.


SINOHYDRO Management have issued ‘Stop Work Authority” to empower the site safety
officers to stop the job without fear of reprimand. He shall report to his supervisor the cause
of the stop works and shall only resume upon confirmation that deficiency or unsafe
conditions has been corrected, rectified or the hazard has been minimized to a level
reasonably acceptable.

08. Areas of Improvement


There are 3 major field items need improvements.
08.1 Work permit.
08.2 Traffic and Heavy equipment safety.
08.3 PPEs.

08.1 Work permit System

a. Site Supervisors/Engineers as work-permit Issuers and Work Permit Receivers are


responsible for joint site and all equipment’s inspection before issuing permit. Permit
will not be signed by any party if there is any non-compliance.
b. NO WORK PERMIT - NO WORK policy, to be implemented strictly.

c. To improve Work Permit System, all work permit issuers and receivers need re-
evaluation / counselling meeting with all issuers and receivers.

d. Site supervisor, safety officer and WPR is responsible for proper and complete work
permit on job site, with all required documents.
e. Safety supervisor will be responsible to inspect/ audit all areas permit early in the
morning.
f. Site supervisor is responsible to conduct daily TBT and 100 % assure the all tools and
equipment’s inspection, permit and documents available before to start of any activity.
g. Site supervisor, WPR and safety officer will not leave his site (Without proper
delegation of permit).

h. Site supervisor, WPR and safety officer will be proactive to their all, job site safety
requirements and will not sit in vehicle/bus/coaster during the execution of job.
i. Site supervisor, WPR will not allow any activity until all requirements have been
completed as per approve MOS / JSA and SA safety requirements, like SA permit, pre
checklist and inspections etc.
j. Site supervisor, WPR and safety officer are responsible to assure all check lists, Fire
extinguishers inspection, 3rd party sticker, operators’ certification, training ID, PPE’S,
MOS, JSA, MSDS etc.

08.2 Traffic and Heavy equipment safety

To improve Traffic and Heavy equipment's, PM is responsible to assign below positions.


a) Competent, Experienced Equipment Supervisor Approved by SAPMT and already
deployed at project he is responsible for all the heavy equipment.
b) Site Engr./Supervisor, WPR and safety officer are responsible for his area TRAFFIC
SAFETY rules implementation.
c) WPR and Safety will issue warning letters and inform senior management immediately
regarding any traffic and safety violation on job site.
d) Site Eng., Supervisor, WPR and safety officer Assure all drivers and passengers and
operators are using seat belt and having desert driving training.
e) Site Eng., Supervisor, WPR and safety officer Assure all vehicle and equipment’s are
using approved EXIT and ENTRY properly, notify seniors if observe any other project or
local vehicles violating SINOHYDRO exit/entry rules or area safety discipline.
f) Site Eng., Supervisor, WPR and safety officer Assur the all barricades, drums and,
blinking lights etc. is functional, and reporting if there is any deficiency.
g) Site Supervisor, WPR and safety officer Assure all vehicles and equipment’s have
checklist filled properly as per equipment/vehicle condition on daily basis and submit
accordingly.
h) Site Eng., supervisor, WPR and safety officer assure there is no lose material / chemicals
etc. in driving / operator cabin and material like sub base, base course is covered with
green mesh and secured accordingly.

08.3 PPE’s

Site engineer, supervisor, foreman, WPR and safety officer are responsible for his area to
check the compliance of mandatory/required PPEs as per the job requirements.

Strict Disciplinary Action and penalties on Failure to comply Safety requirements will be
imposed as discussed in daily TBT and standby meeting with the all employees and all the
‘Front line management’ (Site Engineers, supervisors/Foreman, Safety officers and work
permit receivers and workers in multi and understandable (Translated in their Native)
languages.

09. Responsibilities.

09.1 Site supervisor responsibilities:


Supervisors and Foremen Responsibilities

• Ensure necessary work permit from Saudi Aramco Operations has been issued prior to
start the work (if required).
• Must get counter sign from related department when required (if required).
• Conduct safety tool box talk related to activity.
• Ensure HIP and JSA for the specific work location & activity is available.
• Equipment is certified and in well working condition.
• All operators working under his responsibility are trained, experienced and certified
as per requirement.
• All workers involve in activity having and using job required PPE.
• Workers are fully aware through work related hazards.
• No any underground utilities are present in excavation area.
• Ensure safe working environment for safe work execution.
• Always ensure good housekeeping.
• Ensure all workers are fully aware about emergency response procedure.
• Ensure First Aid Kit is available on work site.
• Release workers for necessary training or meeting when required.
• Report immediately all near misses, First Aid cases, fire, heavy equipment accidents
and property damage incidents etc. to safety supervisor and senior management for
necessary Action.
• Always cooperate with safety personnel, by acting on their recommendations
• Coordinate with safety personnel prior to start any critical activity e.g., work beside the
roads, inside the plant and blasting activities etc.
• Don’t start any activity until necessary safety arrangements are completed.
• Don’t allow workers to ride over heavy equipment.
• Don’t allow workers to sit under or beside equipment.
• Don’t allow workers to sit over, inside or beside the pipes.
• Don’t allow workers to get training for heavy equipment on work site.

09.2 Responsibilities Of HSE Staff

Specific safety staff responsibilities for the project are as follows:


HSE Supervisor
The field safety supervisor assists the safety manager and is responsible for monitoring the
contractor’s performance and assist with the implementation of the safety program where
required. For day-to-day safety matters the field safety supervisor deals directly with the
contractors to resolve problems. To ensure that contractors adopt, implement and maintain a
safety process in accordance with the Project Safety Plan and Procedures as outlined in the
safety manual.

HSE Staff has the following responsibility activities:


• Administering appropriate safe work practices / procedures for the project.
• Promoting a high level of safety awareness on the project through new
employee/transfer employee, contractor/subcontractor orientation and contact with
project supervision.
• Conducting Weekly Safety Self-Assessments, tracking performance and reporting
trends to site management.
• Participates in the Weekly Area Safety Assessments/walkthrough of Owner.
• Conducting quarterly safety audits.
• Maintaining all safety related records and files associated with the project.
• Administer the first aid / medical program and maintain facility and supplies.
• Maintain pertinent information (i.e., phone number, locations) of Emergency
Response Services, physicians, and hospitals.
• Lead and assist in accident / incident investigations to ensure all accidents and
incidents are properly investigated including near miss incidents, first aid cases, all
recordable cases, chemical spillage incidents, property damage, etc.
• Complete and distribute applicable reports.
• Evaluating contractor/subcontractor safety programs.
• Attending contractor/subcontractor pre-bid and “Kick Off” meetings.
• Chair the weekly meeting with safety supervisors.
• Review the Supervisor Safety Logbooks.
• Initiate Job Bulletins concerning safety issues.
• Continuously evaluating project working conditions and safe work practices, and if
warranted, developing positive recommendations for project management.
• Monitoring the behavior of the people and provide coaching to change the attitude
and behavior positively towards working safely and instills “think before acting” and
“value of human life”.

• Participate in work-site layouts to assure adequate work areas, traffic control, parking
areas, lighting levels, receiving areas, etc. Assure that location of offices, shops,
• maintenance areas, fabrication shops, medical, and sanitation facilities reflect safety
considerations.
• Review the work schedules as they are planned to be aware of the number of
contractors and craft workers working in the various areas; anticipate hazards and
implement supporting safety activities.
• Establish personal protective-equipment requirements and recommend requisition for
purchase.
• Plan the safety publicity program and order posters, visual aids, signs, etc.
• Establish and maintain site, Emergency Response Team.

Safety Officer

The safety officer is responsible for implementing the safety program on the project. He is
responsible for monitoring and administering a proactive project safety program in
accordance with the project safety program designed to provide assistance in
recognizing, evaluating, and subsequently eliminating or controlling hazardous acts or
conditions. He works in conjunction with his project/site manager, construction manager
and site safety manager assisting in the implementation of the safety program as it
pertains to the contractor. The Safety officer plays an important role in daily field
monitoring, advice supervisors & craftsmen on safety practices and to recommend on the
spot corrective action to be taken.
