3. When : At around 02:30 pM of December 31, 2023

4. Who : (Reporting Person) Charlito S Formentera, male, 41 years old,


married, Filipino, businessman and a resident of Sitio Diablo,
Brgy. Lamanoc, Merida, Leyte

Description of Motorcycle: One (1) Black HONDA Dream 100 cc single


motorcycle, Half skeleton design, bearing Plate no, IJ 8083, no documents

5. Why : Left abandoned for several days

6. How : At around 01:00 pM of December 31, 2023, above mentioned


reporting person personally appear this station to inform regarding of an abandoned
motorcycle that he discover at the above mentioned place sometimes in the morning of
December 18, 2023, that immediately he turn o-over the same to Hon. Oliver R.
Limosnero, Brgy. Chairman of Lamanoc, Merida, Leyte, for temporary safekeeping
pending to notify the identity of person who left the describe motorcycle in the area
without informing residents thereby to avoid doubts on its status. Investigation
conducted revealed that prior the reporting person turn over the motorcycle to the Brgy.
Chairman, he instructed the residents thereby to inform anyone looking the same to go
directly to the Brgy. Hall however, up to this date described motorcycle was left
unclaimed by individuals that acts as possessor/owner. Personnel believed that
described motorcycle was possibly abandoned by the driver in the area during wee
hours for still undetermined reason. If someone represent from your station as the
owner/possessor of this motorcycle, please contact the hotline number of Merida Merida
Police Station for proper coordination.

Remarks: Described motorcycle was brought to the station for proper disposition
Action Taken: Personnel of this station will coordinate to nearby LTO Office, to notify
real identity of owner/possessor.
