1. On or about April 14, 2022 around 2:45 PM, a shooting incident transpired in front of Lapuz
National High School. The victim was identified as one Ronilo Toquiro Raquil, 20 years old,
single, a college student of South East Asian Institute of Technology Inc. (SEAIT) and a resident of
barangay Lapuz Norala, South Cotabato.
2. Initial investigation revealed that the victim was walking along the national highway when a
lone unidentified gunman aboard a Sniper 125, color yellow, of unknown plate number suddenly
stopped and shot the victim 3 times in the torso. The identified suspect fled northward towards
jurisdiction, Norala thereafter.

3.The victim Ronilo Toquiro Raquil who sustained multiple gunshot wounds was rushed to
Norala District Hospitsl but was declared dead upon arrival.

4.The motive behind the incident is still unknown.

SUBJECT :INITIAL INVISTIGATION REPORT RE: Alleged Simple Imprudence Causing Damages to

Property that inspired Limpin Subdivision, Población Norala, South Cotabato

III. FACTS OF THE CASE:

7. On or about 10:30 in the evening of January 22, 2021 Brgy. Tanod of Población Norala
South Cotabato reported and seek police assistance on the allege vehicular accident at Ledisma
street, Poblacion Norala, South Cotabato. PO2 Johnrey Legaspi was dispatched and returned
station together with the alleged involved parties.
Upon confrontation NSCPS-CPA 2 office, Leonard Morales M. 34 y/o married, a resident
of Barangay San Jose, Norala, South Cotabato with temporary rented apartment along Limpin
Subdivision, Población Norala, South Cotabato (referred to as V1 driver) requested to put into
record that while his vehicle Chevrolet trail blazer, color auburn, bearing plate no. WOA-536
was park unattended at the side of the road fronting his apartment he heard an impact of
collision and upon verification a color Red SUV Hit his vehicle and shouted to the driver not to
left, he then chased said vehicle by Banga drive, Norala District , until he reached Ledisma
Street. Old Norala District, Norala, South Cotabato. Then and there he found a color red vehicle
and flagged it down and asked the driver where they came from, however Tarhata M. Cablo 30
y/o, married resident of Brgy. Simsiman, Norala, South Cotabato (referred to as V2 driver),
holder of LTO NPDL no. NO2-98-005689, with restriction of code 1 & 2 valid until 2027-06-16.
The driver of Toyota Revo Wagon, color black, bearing plate no. VNX-147alleged that while
driving his vehicle coming from B. imperial street, fronting Norala Police Station to fetched her
daughter and her daughter’s friend, they proceed toward Norala District traversing alternate
road and turned left to Isulan drive, Norala South Cotabato, on board in the said vehicle were
Ana Cablo, Rose Cablo, 17 y/o minor and Bryne Dordas and upon reaching Ledisma St., Norala
District Ramon saw a vehicle from his side mirror and suddenly overtook him near Joel Padilla
store and shop in front of them. On that juncture he drove backward his vehicle and step down
and walk with his wife towards the fruit stand nearby. The driver of V-1 alighted, came nearer
and confronted them and asking in bikol dilect “ sahi kamu hali?” which he did not replied and
averred “ Buyong-Buyong” not interested to other diver. It was witness by Grace Ambon 23y/o,
single a resident of Brgy BS Aquino Norala South Cotabato. Few minutes lapse, the responding
police officer arrive together with the Brgy. Tanod nino Arabia and they were brought at the
Police Station at Norala District, South Cotabato. (Hereto attached the copy of excerpt from
NDPS-CPO blotter entry no. 0627 & 0628 dated January 21, 2021)
IV. DSCUSSION/FINDINGS
8. In this instant case under investigation and both parties have a respective allegation
related with the incident. In the statement of V1 driver he only saw a Red SUV hit and damages
his vehicle while identifying the LTO plate number and the model of the suspected vehicle,
upon chased he chance upon another vehicle that he wasn’t sure. Upon confrontation, the first
question of the V1 driver was “sain ka,o hali?” (Where do you come from?) the V1 driver is not
certain. PO2 Arnold Gonzales responded and brought them in this station. Ocular inspection
was made by Pinsp Reynaldo Casuera and personnel of CPAC2 and they recovered a par tide
left by the other vehicle attach to the bumpers as cause by strong impact of damages. in the
statement of V2 driver fetch his daughter and daughter’s friend who were both minors at B.
imperial St. Bitano Norala thereafter proceed a Norala district traversing alternate road and F.
Sto.Nino drive and upon reaching Ledisma street he saw a vehicle at the side mirror, that
overtook and stop in front of them, V2 driver prevented from being hit, indicating that he
cautiously drive his vehicle in favor to other road user, instead of flaring up he move backward
and alighted in his driven vehicle. Both drivers were advised to undergo medical examination at
BIRTH by personnel of CPAC2 due to suspicious smelling of alcoholic breath but V1 driver
refused while V2 voluntary submitted himself with hospital no. 0245622.
V. CONCLUSION
After a careful perusal surroundings this particular incident, a fact of comparison related
with the damage incurred by V1 were not appropriate/matched as to the vertical height,
structure of the vehicle and to damages of V2. The V1 driver conducted cause by the color of
the route traversing by V2. That a fact V2 were enroute to his residence on board a friend of his
daughter. In the position of CPAC2 personnel related with the drivers suspected smelling
alcoholic breath they submitted both drivers but only the V2 driver voluntarily submitted
himself for medical attention while V1 defensively refused the procedure.

SUBJECT : Progress Report on Shooting Incident resulted to the Death of Alvin C. Dahay

DATE : March 17, 2021

1. Reference: Spot report on shooting incident dated March, 17, 2021.


2. This pertains to the shooting Incident in Malipayon, Brgy. San Miguel, Norala South Cotabato a
around 09:00 in the morning on March, 17, 2021. Be informed that the victim Alvin C. Dahay, 25
years old, single, laborer, and a resident of Malipayon, San Miguel, Norala, South Cotabato, was
shot by suspect Alfred M. Fontillon, 30 years old, married, farmer, and a resident of the same
place. Saic victim brought to Norala District Hospital for Medical treatment but declared dead on
arrival.
3. The cadaver of the victim was moved to Regional Crime Laboratory Office, Camp Martin
Delgado, Koronadal City for autopsy examination. The immediate cause of his death was
gunshot wound on the neck, head and trunk.
4. The suspect was arrested at his house during hot-pursuit operation conducted, but the weapon
used in the commission of the crime was not recovered and he is now under the custody of this
station for investigation and proper disposition.
5. Further development will be sent to that office as soon as obtained.
