ROOT CAUSE ANALYSIS UNDER ELEVATION


CONCRETE AND REBAR R10-R50

1. Problem Definition
a. Rebar under elevation from PCO
b. Concrete Under elevation from PCO
c. Rebar and Concrete under elevation from PCO

2. Identification of Problem
a. The Rebar below the elevation of the PCO, this occurs because during process of
pouring concrete and breaking the concrete, the tremie pipe and spiral reinforcement
rebar collide and the rebar bond is not strong enough on the iron hanger above the
casing which causes the reinforcement rebar is fall.

b. The concrete casting process is always optimized for the elevation of the concrete
above the PCO. However, during process the PIT preparation process, the condition of
the concrete on the surface was still mixed with mud. So to get good PIT results,
Contractor has to pound the concrete until the finds good concrete at a certain
elevation.

All of bore pile will be tested PIT 100%, preparation will be done as below:
1. Condition of Pile may be as below:
- Bored pile consists of concrete pile, bed concrete pile and soil or mud in the
top.

2. Excavation Around Pile.


- Excavate around the pile.
- Then excavate the soil / mud on the top of pile using hoe if any.

3. Grinding the Top of Good Concrete.


- After chiseling the top of concrete may be still roughness, the parts area that
will be tested should be smoothing using grinder.

IMPROVEMENT

A. CASE 1

 Overlab rebar on the Reinforcement has been added by 30 cm, from 60 cm to 90 cm

 To add strength to the reinforcement rebar iron hanger, Contractor replace iron hanger
from D8 cm to D16 cm. Then to further strengthen the iron hangers the Contractor added
clamps on each hanger.

B. CASE 2

 At the end of te concrete pouring process the concrete elevation that we achieve is
equivalent to the ground level.
