Dear Sir/Madam,
I, Dr: SIRAJ MADAYI MEETHEL, holding passport number S7222789, would
like to apply for a Schengen visa upon my intention to travel to Poland from September 06 to
September 11,2023. I would like to travel to Warsaw for appearing in EDAIC Part 2
Examination which will be held on 9th & 10th of ¹September at Warsaw.
I am a Specialist Anaesthesiology Doctor, working in Aster Hospital, Mankhool,
Dubai. I am drawing a salary of 35,000 AED along with other benefits. I am staying in Dubai
with my wife and 3 children.
The main purpose of my journey is to write EDAIC Part 2 examination
( European Diploma in Anesthesiology and Intensive Care ) which will be held at Warsaw on
9th & 10th of September 2023. Exam is conducted by European society of Anaesthesiology
and intensive care, ESAIC. Head office of the society is situated at Brussels, Belgium.
I have cleared the first part of EDAIC in 2022 and Paid the fees for the 2 nd Part of the
Examination. And I have got my exam Centre as Warsaw. I have received an invitation letter
from the EDAIC Society also. Venue of the Exam is Radisson Blu Sobeiski Hotel, Plac Artura
Zawiszy, 02-025 Warsaw.

I have been working in Aster hospital, Mankhool since October 2022 , holding the
position of Specialist Anesthesiology Doctor . My employer has already approved Leave
from 6th September to 11th September & issued a NOC letter for attending this Examination.
I will resume my duties from 12th of September.

I am the bearer of all my travel expenses. I have 2 Accounts in Emirates NBD bank –
Savings & Current account and Also have Emirates Islamic Bank Credit card also ( Credit
Limit is 35,000 AED).
I have already confirmed my to and fro flight tickets and accommodation
