D. EXPLANATION

Forms are documents that you have to fill out as a


requirement in school or in seeking for a job. These are
important because they tell the school or company about you.
There are many types of forms. For example, a biodata is
usually required when you apply for job. Another example is
a request form for a community tax certificate. This certificate
is required for many other documents as a proof that you live
in a certain locality. When you sign a contract or any legal
document, a community tax certificate number is required.
Application
Divide the class into (3) groups. Each group will select a

Directions: Each group will fill out one (1) form and share
the importance of filling out the form completely and
accurately. Present your output in the class using your
special talents or skills.
