AFFIDAVIT

I, LISA JANE B. ARANAS, Filipino, of legal age, and a resident of Purok 5A, North
Poblacion, Maramag Bukidnon, after having been duly sworn to in accordance with law, do
hereby depose and say:

That I am a government employee presently connected with the Local Government

Unit of Maramag assigned at Municipal Accounting Office;

That I suffered from cough, colds and migraine last December 01, 2023 and I was not
able to report and render duty on the said date and therefore applied for one (1) day sick
leave;

That I did not secure medical certificate nor applied for hospitalization considering
that my sickness need no admission at a hospital for treatment but only requires adequate
rest and mild medications.

That I have executed this affidavit to state the truth of the foregoing facts.
