AFFIDAVIT OF BEING SINGLE

I, RALPH IAN EGUAY MAGDAY, of legal age, Filipino, single, and a resident of
3306 9th St., UPS-5, Brgy. San Isidro, Parañaque City, after having been sworn to in
accordance with law, hereby depose and say that;

1. That I am currently applying in the Armed Forces of the Philippines as trainee;

2. That I was born on January 26, 2002 and one of the children of spouses IMEE
EGUAY MAGDAY and RENATO MARRON MAGDAY;

3. That I am still single, as I have never contracted any marriage with another
person up to the present;

4. I execute this Affidavit to attest as to the truth of the foregoing facts and for
whatever legal purposes this may serve best.
