After the source of information I discussed with Safety Manager about health and safety
 who you spoke to; and management system in organisation and requested to share past 12 month record related to
health and safety of the organisation and the Safety Manager shares various records like training
records, accident & incident records, medical records, staff turnover records, absence records,
 how you identified: H&S policy ,copy previous risk assessment, maintenance records, meeting records, complaint
- the hazards; register, audit and investigation report previous accident etc.
- what is already being done; and
- any additional controls/actions that I went to construction site and meet with workers, site engineers, site supervisor and
may be required. Consult with them about various work activity in the site and associated hazard and understand
their feedback about health and safety in the site. I visited on the site and I made various
observation of activity on going in the site example: worker are doing welding near by the
combustible material ( Waste used cotton), I noticed existing control measure and the notes I took
during the inspection and the sources of information I have already reviewed helped me to
determine which additional control measures were needed after the inspection.

Part 3: Prioritise 3 actions with justification for the selection
Suggested word counts
Moral and financial arguments for all actions: 300 to 350 words
For EACH action:
Specific legal arguments: 100 to 150 words
Likelihood AND severity: 75 to 150 words
How effective the action is likely to be in controlling the risk: 100 to 150 words

Moral and financial arguments for ALL actions

Moral, general legal and financial Moral reason

arguments
It is a moral responsibility of employer to take health and safety or the all worker and provide safe
and healthy working condition. when health and safety is not managed properly people get killed
and injure in gruesome ways or suffer terrible diseases that have massive impact not only on them,
but also their dependent families, friends and colleagues, society as a whole considers these
events to be morally unacceptable, and injury or ill health should not that has to be paid in order for
the work to feed their family.

General legal arguments

As per ILO C155 article-16 Its a duty of the company to ensure fulfillment of legal duties defined in
national and local laws of legislation. The legal reason for managing health and safety relate to the
framework of international and national laws that govern the conduct of business and organisations.
If organisation has failure to archive legal minimum standard can lead to enforcement action by the
authorities or prosecution before the court. Successful prosecution can lead to a fine in criminal
court and imprisonment for the individual.

Financial arguments
If the health and safety is not maintained in the organisation its can lead to accident and ill heath
cost money. When accident occurs there will be direct and indirect cost associated with that event,
some of the losses can be insured but many cannot. Accident and ill health can significantly affect
the profitability of an organisation. Which impact the organisation put in loss or can put out of

Learner number: 00677003 Learner name: Arsh Wasim Raza Page 15 of 21

business. The two types of losses that organisation may face

Direct cost: Medical/ hospital treatment cost, repair cost to buildings, equipment.

Indirect cost: Production delays, loss of reputation, loss of workers morale, training for new
recruiters, and loosing clients.

Justification for action 1

Combustible and flammable materials are removed from the work area or covered by with the fire
Action
retardant blanket. ( Hazards category - Fire )

As per ILO Code of Practice Convention C167 - Safety and Health in Construction
Specific legal arguments
Convention, 1988

Article 29 Fire precautions

1. The employer shall take all appropriate measures to-

 (a) avoid the risk of fire

2. Sufficient and suitable storage shall be provided for flammable liquids, solids and gases.
As per ILO Code of Practices safety and Health in construction
3.6 Fire Prevention and fire Fighting
(3.6.3) Secure storage area should be provide for flammable liquid, solid ,gases such as liquefied
petroleum gas cylinder, paints and other such material in order to deter trespassers.

As per ILO code of Recommendation R175 - Safety and Health in Construction

Recommendation, 1988,

46. Where necessary to guard against danger, workers should be suitably trained in the action to be
taken in the event of fire, including the use of means of escape

Learner number: 00677003 Learner name: Arsh Wasim Raza Page 16 of 21

Likelihood, I set following categories
Consideration of likelihood AND severity
Certain: it will happen again and soon;
Likely: it will reoccur, but not as an everyday event;
Possible: it may occur from time to time;
Unlikely: it is not expected to happen again in the foreseeable future;
Rare: so unlikely that it is not expected to happen again.

In The justification one. Likelihood is taken – certain because naked flames or sparkles of fire
coming from the welding activity near by the flammable substance if comes in contact may catch
fire. In this work activity total 5-6 worker were engaged for 7- 8 hours of work.

Severity, I set following categories:

Minimal: no injury or damage occurred
Minor: injury requiring first-aid and/or slight damage caused to plant/equipment/buildings
Major: injury requiring hospital treatment/stay and/or significant damage caused to
plant/equipment/buildings
Catastrophic: death and/or irreparable damage to plant/equipment/buildings.

Severity is taken in this – catastrophic as worker was carried out welding activity near flammable
material (paint & solvent) which can catch fire in site which will result to personal burn injury, death,
property damage, equipment damage, and explosion.

How effective the action is likely to be in

controlling the risk. This should include:
If the combustible materials and flammable substances removed from the site will reduce the
 the intended impact of the action;
chance of fire in the site which can save company from enforcement action, direct and indirect cost.

 justification for the timescale that you I consider for timescale for this action 1 week which may required for arrangement of storage area
indicated in your risk assessment; that may take 1 week to arrange in proper manner in with good ventilation with labeling.
and

 Whether you think the action will fully

control the risk. In this case I have suggested fully control measure which can prevent from fire hazards and reduce
the risk of multiple causality accident and property damage and business loss of organisation.

Learner number: 00677003 Learner name: Arsh Wasim Raza Page 17 of 21

Justification for action 2

Action Scaffolding Erection and inspection should be done by competent person (Hazards category-
Working at height)
Specific legal arguments AS per C167 - Safety and Health in Construction Convention, 1988
Article 14 scaffold and ladders
4. Scaffolds shall be inspected by a competent person in such cases and at such times as
shall be prescribed by national laws or regulations.
As per ILO, R175 - Safety and Health in Construction Recommendation, 1988 (No. 175)
13. Where workers cannot be protected against falls from heights by any other means-
 (a) adequate safety nets or safety sheets should be erected and maintained; or

 (b) Adequate safety harnesses should be provided and used.

18. The working platforms, gangways and stairways of scaffolds should be of such
dimensions and so constructed and guarded as to protect persons against falling or being
endangered by falling objects.

20. A scaffold should not be erected, substantially altered or dismantled except by or under
the supervision of a competent person.

Consideration of likelihood AND severity In the justification two. Likelihood is taken - certain because worker is working at height without
guard rail or edge protection and without full body harness above 5 meter height can be chance of
falling from height. In this activity 3-4 worker were engaged in maintenance work.
The severity is taken is – catastrophic as worker is working at height without a guard rail and
without full boy harness which will result to fatality ,death, neck injuries, spinal injuries, broken
bones or multiple fracture.

Learner number: 00677003 Learner name: Arsh Wasim Raza Page 18 of 21

How effective the action is likely to be in
If the scaffold will be erected and inspected by competent person and will help to reduce the
controlling the risk. This should include:
hazards from work at height and it also reduce the likelihood and severity from falling from height
 the intended impact of the action; which may company save money from associated with work at height accident.
 justification for the timescale that you
I have take timescale for this action is 1 month because which required hire competent scaffolder
indicated in your risk assessment;
and developed inspection procedure which may need approval by the management to set safe
and
working procedure in company.
 whether you think the action will fully
control the risk. Work at height is a high risk activity I have suggested control measure which is help in the case to
minimize and control the risk from work at height activity if the additional control measure is taken
or developed safe working procedure in the organisation.

Justification for action 3

Action Cable and plug appear to be fully insulated with no splits and maintained by competent person.
( hazards category – Electricity)
Specific legal arguments AS per C167 - Safety and Health in Construction Convention, 1988
Article 26 Electricity
1. All electrical equipment and installations shall be constructed, installed and maintained by a
competent person, and so used as to guard against danger
3. The laying and maintenance of electrical cables and apparatus on construction sites shall
be governed by the technical rules and standards applied at the national level.
Article 13 safety of workplace
. All appropriate precautions shall be taken to ensure that all workplaces are safe and without risk of
injury to the safety and health of workers.

ILO code of practice for ambient factor

6.2.5. In assessing the hazard and risk, the employer should take account of the need to prevent
cardiac accidents which may result from exposure of workers with pacemakers or similar medical
implants to electric and magnetic fields, as well as the special needs of workers for protection in

Learner number: 00677003 Learner name: Arsh Wasim Raza Page 19 of 21

relation to their health condition, for example in the case of pregnant women.

Consideration of likelihood AND severity In the justification two. Likelihood is taken – likely because Worker was performing grinding activity
grinder electrical cable and plug was found in damage condition which may cause chance of
electrocute to the worker and other staff and visitor. There are 3 workers who perform this job for 5
to 6 hours.

In this event I have taken severity is – catastrophic because if the worker are in contact with live
electrical cable and damage pug it can lead to electrocuted which may result in electric shock,
electric burn, head injury, multiple fracture electric fire & explosion,

How effective the action is likely to be in

If the Cable and plug appear to be fully insulated maintained by competent person that can
controlling the risk. This should include:
eliminate and reduce the chance to electrical shock and electrical fire that may impact on the
 the intended impact of the action; company financially and morally and prevent from the enforcement action and property loss.
I consider time scale for this action is 1 month because it’s required insulation material and
 justification for the timescale that you
competent electrical specialist for maintenance which need to approval from by the company to
indicated in your risk assessment;
follow the procedure.
and
 whether you think the action will fully The action will control and minimize the risk of electrical hazards if the suggested control measure
control the risk. is taken it can reduce the risk of electrical shock and electrical burn injury and property damage by
electrical fire.

Part 4: Review, communicate and check

Suggested word counts for each section:
 Planned review date or period and reasoning for this: 50 - 100 words
 How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words
 Follow up on the risk assessment: 100 - 150 words.

Planned review date/periodwith As per company policy risk assessment should be reviewed every 12 month. Therefore I set the
reasoning review date on 7th May 2023
However review can done if this significant changes -:
1. Change workplace environment.
