صيُب َس رطاُن الثدي ُثمَن النساء خالل حياتهن .وهو من أكثر أنواع السرطانات شيوعًا عند النساء أفي
العالم .ال نعرُف لماذا ُتصاب بعُض النساء بسَرطان الثدي ،غيَر أّن هناك عدٌد من َع وامل الُخ طورة .وتتضمُن
َ:ع وامل الُخ طورة التي ال يمكُن تغييرها
.الُع مر -ترتفُع ُفرصُة اإلصاَبة بسَر طان الثدي مع تقُّدم المرأة بالُعُم ر
و هما يزيدان الُخ طورة بشّد ة .وقد ترغب النساء اللواتي )BRCA2و (BRCA1الجينات -هناَك جينان
.عندهن أفراد من أسرتهن مصابات بسرطان الثدي أو الَم بيض بأن يجرين الفحص
.العوامل الشخصّية -بداية الدورات الَح يضّية قبَل عمر 12سنة أو دخول سن اليأس بعد ُع مر 55سنة
وتتضّمُن المخاِطر األخرى فرط الَو زن ،واستخدام الُم عالَج ة الهرمونَّية التعويضَّية (نسميها أيضًا المعالجة
الهرمونية اإلياسية) ،أو تناول حبوب منع الحمل ،أو تعاطي الُكحول ،أو عدم اإلنجاب ،أو إنجاب أّول طفل
بعد ُع مر 35سنة ،أو اكتناز الثديين .قد تتضمُن أعراض َس رطان الثدي وجود ُكتلة في الثدي ،أو تغير في
َح جم أو َش كل الثدي ،أو نزيز من الحلمةُ .يساعُد فحُص الثدي الذاتي والتصوير الشعاعي للثدي على
تشخيص َس رطان الثدي باِكرًا عندما يكون قابًال للعالج .قد تتألف المعالجة من المعالجة الشعاعّية
واستئصال الُكتلة واستئصال الثدي والمعالجة الكيميائية والمعالجة الهرمونّية .يمكن أن ُيصاب الرجال
.بسرطان الثدي أيضًا ،إال أن عدد الحاالت قليٌل

السرطان وأسبابه
يتألُف الجسُم من َخ اليا صغيرٌة جدًا .تنمو الَخ اليا الطبيعَّية في الجسم وتموُت بطريقٍة َم ضبوطة .تواصُل
الخاليا أحيانًا االنقسام والنمّو بصورة غير طبيعية ،ما يتسّبُب بنمو شاذ اسمه الَوَر مُ .يسمى الَو رم إذا لم
يغُز األنسجة وأجزاء الِجسم الُم جاوَر ة الوَر م الحميد ،أو نمو غير َس رطاني .ونادرًا ما تكون األورام الَح ميدة
ُم هددة للحياةُ .يسمى الورم إذا غزا ودَّم ر الخاليا الُم جاورة بالورم الَخ بيث أو السَرطان .السرطاُن ُم هدٌد
للحياة أحيانًا .كما قد تنتشر الخاليا السَرطانَّية إلى أجزاء ُم ختلفة من الجسم َع بر األوعية الدموّية
واألقنية اللمفية .اللمف هو سائٌل رائق ينتجه الجسم يصِّرف الفَضالت من الَخ اليا .وهو يتحرُك عبَر أوعية
خاّصة وبنى لها شكل حبة الفاصولياء اسمها الُع قد اللمفيةُ .تستخدم ُم عالجات السَرطان لقتل أو ضبط
الخاليا السرطانَّية النامَية بشكٍل غير طبيعي .تعطى السَرطانات في الجسم أسماء ،بحسِب المكان
الذي بدَأ السَرطان منه .يسمى السَرطان الذي يبدأ في الثديين َس َرطان الثدي دائمًا ،حتى لو انتَش َر
ألماكن أخرى مثل الَكبد أو العظام أو الدماغ .مع أن بإمكان ُم قدمي الرعاَية الصحّية تحديد مكان بدِء
السَرطان ،غير أنهم ال يستطيعون عادًة َتحديد سَبب السَر طان .تحوي الخاليا مواد َم وروَثة اسمها
الِص ْبِغ ّيات .وتضبُط هذه المادة الجينّية نمو الَخ لية .يحدُث السرطان دومًا بسبِب َتَغُّيرات تحدُث في تلك
المواد الجينية .قد تخسر المادة الجينّية قدرتها على ضبط نمو الخاليا عندما ُتصبُح شاّذة .قد تحدُث تلك
التغّيرات الُم فاِجئة في المادة الجينية بسبب عدٍد من األسباب الُم ختلفة .قد يرُث الشخُص تلك التغيرات
من والديه .كما قد تحدُث التغيرات في الماّدة الجينية بسبِب التعُّرض للعدوى أو العقاقير أو التبغ أو المواد
.الكيميائّية أو عواِمل أخرى
تشريح الثدي
يحتوي الثديان على نسيج ُد هني وُغ دد .الغدُد مسؤولٌة عن صنع الحليِب بعَد الحمل .وهما يستجيبان
للعديد من الهرمونات األنثوية ،مثل األستروجين والبروجسترون .ثّم ُيفرزان الحليب إلى الخاِرج من خالل
أقنَية خاّصة تنفتُح في الحلمتين .يصّرُف الجهاُز اللمفّي عادًة سوائل الثدي الزاِئدة إلى الُع قد اللمفية في
اإلبط .ومن هناَك ترجُع إلى َم جرى الدم .يوجُد الثديان فوَق َع ضالت ُم هّمة تسمُح بتحريِك الذراعين،
.باإلضافة لعضالت مرتبطة بعملية التنفس
سرطان الثدي
قد ينشُأ َس رطان الثدي إما من ُغ دد أو قنوات الثديُ .يطلق على السرطان اسم الَّس َرطاَنة الفصيصية إذا
نشَأ مَن الغدد .الُف صيصات هي غدد خاصة ُم نتجة للحليب .عندما يحدُث السرطان في أقنية الثديين فهو
ُيعرف باسم السَرطانة القنوّية .عندما ينتقل السرطان من المكان الذي نشأ فيه الى مكان آخر فهو ُيعرف
باسم سرطان ارتشاحي أو غازي .يكون السرطان الذي لم يعبر ألبعد من الُف صيص أو الُن بيب الُم صابين ذو
طبيعة محدودة جدًا .وُيطلق عليه اسم السرطانة الالبَد ة .قد يصيُب َس رطان الثدي أكثر من فرد في
العائَلة ،وهذا ُيعرف باسم سرطان الثدي العاِئلي عاَدًة .يمكُن أن يكون هناك بعُض األسباب الوراثية
والجينّية وراء هذا النوع من َس رطان الثدي .وَس محت التطورات الجينّية الحالية بكشِف البعض من تلَك
الجينات .يمكُن أن تستفيُد النساُء ذوات سرطان الثدي العائلي من االستشارة الجينية وربما من الفحص

األعراض والعالمات
ال يملُك َس رطان الثدي الباكر أعراضًا عادًة .وقد يتسبب السرطان فيما َبعد عندما يكبُر بُكتلٍة يمكُن
اإلحساُس بها في الثدي .ويصبُح الجلُد فوق الورم خشنًا ومتجعدًا أحيانًا .وُيعرف هذا باسم ِقشرة
البرتقال .كما قد يشكُل النزيُز من الحلَم ة عالمًة على َس رطان الثديُ .تكتشف ُم عظم حاالت سرطان
الثدي إما بواسطة صورة الثدي الشعاعية أو عن طريق فحص الثدي السريري .وفي تلك األحيان سيقوم
ُ.م قدم الرعاية الصحية بفحص الثدي .األلُم في الثدي غير شاِئع أبدًا كعرض لسَرطان الثدي
الخزعة
الَخ زَع ُة هي استئصال نسيج للبحِث عن خاليا سرطانّية .والخزعُة هي الوسيلُة الوحيَد ة للتأكد من وجود
سرطان .قد يحيُل ُم قدُم الرعاية الصحية المريضة إلى َج راح أو إلى ُم تخصص بأمراِض الثدي ألخذ الخزعة.
:وسوف يستأصل نسيجًا أو يأخذ سائًال من الثدي بطريقة من الطرق التالية
خزعة الشفط باإلبرة الرفيعة :سوَف يستخدُم الطبيُب إبرة رفيعة الستئصال َخ اليا أو أخذ سوائل من
.الكتلة الثديية
.الخزعة الُلبَّية :سوَف يستخدُم الطبيُب إبرًة عريضة الستئصال عِّينة من نسيج الثدي
َخ زَع ة الجلد :قد يأخذ ُم قدم الرعاَية الصحية عّينة َصغيَر ة من الِجلد في حال كان ُه ناك تغُّيرات جلدية
.على الثدي
.الخزَع ة الجراحَّية :يستأصُل الجراُح عينة مَن النسيج
.تستخلُص الخزَع ة االقتطاعية جزءًا من الُكتلة الورمية أو المنطَقة الشاذة
.تستخلُص الخزَع ة االستئصالية كامل الُكتلة الورمية أو المنطَقة الشاذة
وسوف يتفّحُص اختصاصي الباثولوجيا النسيج أو السائل الُم ستخلص من الثدي للبحث عن الخاليا
.السَرطانية
التشخيص
عنَد اكتشاف َس َرطان الثدي ،يلي ذلك عادًة عملية إلزالة الَوَرم ولتشخيِص السَرطان .وقد يتفحُص الجراُح
الُع قد اللمفية في اإلبط أثناَء الِجراحة بحثًا عن وجود السَرطان .وسيفحُص ُم قدم الرعاية الصحية النسيَج
الُم ستأَصل وقت الِجراحة ويقرر فيما إذا كان الورُم َس رطانيًا أم ال .يمكُن إجراء ُفحوص أخرى أكثر اختصاصًا
على الُكتلة إذا كانت الُكتلة َس َرطانية .وقد يطلُب الجراح واختصاصي األورام من المريضة إجراء المزيد من
الفحوصات اإلشعاعية .يمكُن لبعض الفحوصات أن ُتحدد كم مرة تتضاعُف الخاليا السرطانية أكثر من َخ اليا
الثدي الطبيعية .وتحدد فحوٌص أخرى فيما إذا كانت الخاليا السَر طانية تحت تأثير الهرمونات األنثوية
الطبيعية مثَل األستروجين والبروجسترون .وُيعرف ذلك باسم اختبار ُم ستقبلة األستروجين البروجسترون.
على بعض أنواع الخاليا السرطانية .ويبيُن هذا االختباُر فيما HER2/NEUيوجُد بروتين HER2/NEU:اختبار
أو الكثير من ُنَس خ جيناته .إذا كان هناك الكثير من HER2/NEUإذا كان النسيج يملُك الكثير من بروتين
في الَوَر م الثديي ،عندها ُيَش ِّكل العالج الُم وَّج ه أحد خيارات المعالجة .قد يستغرُق الُح صول HER2/NEU
على نتاِئج تلك الُف حوصات عَّد ة أسابيع .وتتألُف الفحوصات اإلشعاعَّية من تصوير العظم وتصويرات مقطعية
.محوسبة مختلفة للتحّقق فيما إذا كان السَرطان قد انتشَر لخارج الثدي ومنطقة اإلبط
إلى قابٍل للجراحة (يمكن عالُجُه بالجراَح ة) وغير قابٍل للجراحة (ال Cينقسُم َس رطان الثدي من المرَح لة 3
:يمكن عالُجُه بالجراَح ة) .وسرطان المرحلة القابَلة للِجراَح ة
يوجُد في ُ 10ع َقد لمفية أو أكثر في اإلبط؛ أو
يوجُد في الُع قد اللمفية تحت عظم القص وُقرب الُع نق على نفس الجهة من الِجسم التي يوجد بها
السرطان؛ أو
.يوجُد في الُع قد اللمفية في الثدي نفسه وفي العقد اللمفية اإلبطية
الغير القابل للجراَح ة فوق عظم الَت ْر ُق َو ة وقرَب الُع نق على نفس Cيكوُن سرطان الثدي من المرحلة 3
الجهة من الِجسم التي يوجد بها السرطان .سرطان الثدي من المرحلة 4هو َس رطان انتشَر إلى أعضاء
.أخرى من الجسم ،مثل العظام أو الرئتين أو الكبد أو الدماغ في اغلب االحيان
الجراحة
ُتستأصل ُم عظم َس َر طانات الثدي ِجراحيًا .ويعتمُد امتداُد العملية الجراحية على َح جم الورم وفيما لو
أصيبت الُع َقد اللمفية اإلبطية أم ال .يوجُد هدفان رئيسيان لعمليات َس رطان الثدي .األول هَو استئصال
كامُل الَوَر م دون َترك أي ورم في منطَقة الثدي .الهدُف الثاني هو التحقُق من الُع َقد اللمفية في اإلبط
للتأكد من عدم انتشار الورم لها .وقد يرغُب الجراُح في حال انتشار السرطان إلى الُع قد اللمفية في
تحديد عدد العقد اللمفية الُم صابة بالسرطان .ألن استئصال الكثير من الُع قد اللمفية اإلبطية ِجراحيًا قد
ُيؤدي النتفاخ الذراع ،وهي حالة ُتعرف باسم الوذَم ة اللمفية ،لذلك ُطّوَر ت تقنيات جديدة الستئصال عدد
قليل فقط من الُع قد اللمفية المهّمة .يحقُن الجراُح قبَل الجراَح ة بعّد ة ساعات إما َصبغة َز رقاء خاصة أو
صبغة ُم شّعة آمنة قريبًا من الورم .يستأصل الجراُح أثناء الِجراحة السَرطان ويكوُن قادرًا على إيجاد الُع قد
اللمفية التي امتصت الصبَغةُ .تعرف أول ُع قدة لمفّية تمتّص الصبَغة باسم الُع قدة الخاِفرة .استئصاُل
الكتلة هي َع ملّية جراحّية تهدُف الستئصال الُكتلة السَر طانية فقط ،مع أخذ بعض الَخ زعات من الُع قد
اللمفية اإلبطيةُ .تعطى الُم عالجة اإلشعاعَية بعد هذا النوع من العمليات عادًة ُ .يزيُل استئصاُل الثدي
الُج زئي أو الِقطعي جزءًا من الثدي أكبر مّما في استئصال الُكتلة .ويمكُن استئصال جزء من الغطاء
العضلي السفلي أيضًاُ .تحتاج الُم عالجة اإلشعاعَية بعد هذا النوع من العمليات عادًة .وأيضًا ُتستأصُل بعض
الُع قد اللمفية للتحقق من وجوِد أي انتشار للسَرطان .يهدُف استئصال الثدي الكلي أو البسيط
الستئصال كامل الثدي ،باإلضافة لبعض العقد اللمفية اإلبطية .يهدُف استئصال الثدي الجذري الُم عَّد ل
الستئصال الثدي وبعض الغطاء العضلي السفلي ،وربما جزء من العضلة .كما ُتستأصل بعض الُع قد
اللمفية اإلبطية أثناء هذا النوع من العمليات الجراحّية .يهدُف استئصاُل الثدي الجذري الستئصال الثدي
.والعضالت السفلية وكل الُع قد اللمفية اإلبطية .ونادرًا ما ُيلجأ لهذه العملية
المعالجة اإلضافية
قد ُيوصي مقدم الرعاَية الصحّية بعَد الجراَح ة بنوع عالج أو أكثر للُم ساعدة على الوقاية من نكس
السَرطان .وتتضمُن ُم عالجات الُم تابَع ة الشائَع ة المعالجة اإلشعاعية والُم عالجة الهرمونية والُم عالجة
الكيميائية .المعالجة اإلشعاعَّية عبارٌة عن سلسلة من الُم عالجات باألشعة السينية الُم صممة إلزالة أية
خاليا َس رطانية من الثدي أو الُع قد اللمفية التي يمكن أن تظل موجوَد ة .وتحتاج المريضة عادًة نحو 5إلى
6:أسابيع من العالجات القصيرة لتنتهي منهاُ .تعطى المعالجة اإلشعاعية عادًة
.بعد استئصال الُكتلة
.بعد استئصال الثدي إذا كان الَوَر م بحجم أكبر من 5سنتمترات
.اذا تم اكتشاف إصابة عدد كبير من الُع قد اللمفية
.إذا كان الورُم قريبًا من عضالت جدار الصدر أو القفص الصدري
يستخدُم ُم قّد م الرعاية الصحية نوعين من الُم عالجة اإلشعاعية لعالِج َس َر طان الثدي .وتتلقى بعُض
المريضات كال النوعين .المعالجة اإلشعاعية الخارجَّية :وهو نوع اإلشعاع األكثر شيوعًا الذي ُيستخدم
لعالج َس رطان الثدي .ويأتي اإلشعاع من آلة كبيرة خارج الجسم .تكون المعالجات عادًة لخمسة أيام
أسبوعيًا لمدة 4إلى 6أسابيع .الُم عالجة اإلشعاعية الداخلية (المعالجة اإلشعاعية الزرعية أو الَكَثبّية):
يضع مقدم الرعاية الصحية أنبوب رفيع واِحد أو أكثر داِخل الثدي من خالل شق صغير .وتعتمُد اآلثار
الجانبية بشكِل رئيسي على الُج رعة ونوع اإلشعاع .من الشائع أن ُيصبح الجلد مكاَن المنطَقة الُم عالجة
جافًا وُم حمرًا وُم مضًا وحاّكًا .قد تحك الصدرية والمالبس الضّيقة الجلد وتسبب تقُّرحات .ومن المهم أيضًا
تطبيق عناية جلدية لطيفة .ينبغي أن تسأَل المريضة مقدم الرعاية الصحية قبَل استخدام أي ُم زيالت
روائح أو مراهم أو كريمات .يصبح الجلُد مع انتهاء المعالجة رطبًا ومتعرقًا .سوف يتعافى الجلد بعَد
الُم عالجة ببطء .من الُم رجح أن ُتصبح المريَض ُة منهكًة بشّد ة أثناء الُم عالجة الشعاعية ،السيما في أسابيع
المعالجة األخيرة .وقد ترغُب المريضة بمناقشِة اآلثار الجانبية طويلة األمد الُم مكنة على رئتيها أو قلبها
مع مقدم الرعاية الصحية .الُم عالجُة الموَّج هة :قد تتلقى بعُض مريضات َس َرطان الثدي عقاقير اسمها
الُم عالَج ة الُم وّجَه ة .تستخدُم الُم عالجة الموجهة عقاقير لمنع نمو خاليا َس رطان الثدي .حيُث تستخدُم
والذي ُيحرُض نمّو خاليا الثدي ) HER2مثل( الُم عالجة الُم وجهة عقاقير تمنع تأثير أحد البروتينات الشاّذة
الَس رطانّية .الُم عالجة الهرمونَّية والُم عالَج ة الكيميائية هي استخدام أدوَية قد يوصى بها بعَد الِجراحة
للُم ساعَد ة على الوقاية من نكس السرطان .ويمكُن إعطاؤها عبَر الوريد إلى مجرى الدم أو َفمويًا .عادًة
ما ُتعطى الُم عالجة الهرمونية إذا ما ُو ِجد مستقبالت أستروجين أو بروجسترون أو كالهما على السرطان.
فوجوُد تلَك الُم ستقبالت يعني ُع مومًا بأّن تلك الهرمونات تحرُض نمو السرطان .يمكُن للُم عالجة الهرمونية
باستخدام التاموكسيفين أو العقاقير الُم ماثلة أن يمنَع تأثيرات البروجسترون واألستروجين .وقد ُتنصح
النساء في عمر قبل سن اليأس بأن يجرين استئصال المبيض كي يتوّقف إنتاج األستروجين
والبروجسترون بعد ِجراَح ة َس رطان الثديُ .تعطى الُم عالجة الكيميائَّية عادًة في الحاالت التي يكوُن ورم
الثدي فيها كبيرًا جدًا أو إذا كان قد انتشَر ألعضاء أخرى في الجسم .وُتعطى الُم عالجة الكيميائّية أحيانًا
لتقليل ُخ طورة النكس حتى ولو كان السَرطان لم ينتشر خارج الثدي .يوجُد الكثير من األحوال الُم ختلفة
التي قد يوصى فيها بالُم عالجة الكيميائَّية .يناقُش اختصاصي األورام االحتماالت مع المريضة وما يظن أنه
أفضل خيار بالنسبة لها .التجاِرب السريرّية هي وسائل عالج أخرى تكون ُم توفرة عادًة ؛ وهي وسائل
الختبار الخيارات الِعالجّية الجديدة .وُينظر لتلَك التجارب عادًة بأنها بمثل فعالية أفضل العالجات المتوافرة

Breast Cancer
Breast cancer affects one in eight women during their lives. Breast cancer kills more women in
the United States than any cancer except lung cancer. No one knows why some women get
breast cancer, but there are a number of risk factors. Risks that you cannot change include
Age - the chance of getting breast cancer rises as a woman gets older.
Genes - there are two genes, BRCA1 and BRCA2, that greatly increase the risk. Women who
have family members with breast or ovarian cancer may wish to be tested.
Personal factors - beginning periods before age 12 or going through menopause after age 55.
Other risks include being overweight, using hormone replacement therapy (also called
menopausal hormone therapy), taking birth control pills, drinking alcohol, not having children
or having your first child after age 35 or having dense breasts. Symptoms of breast cancer
may include a lump in the breast, a change in size or shape of the breast or discharge from a
nipple. Breast self-exam and mammography can help find breast cancer early when it is most
treatable. Treatment may consist of radiation, lumpectomy, mastectomy, chemotherapy and
hormone therapy. Men can have breast cancer, too, but the number of cases is small.
INTRODUCTION
Cancer of the breast is the most common form of cancer that affects women but is no longer
the leading cause of cancer deaths. About 1 in 8 women develop breast cancer. This health
information will help you understand the diagnosis and treatment options of breast cancer.
BREAST CANCER
Breast cancer may originate from either the glands or the ducts of the breast. If cancer
originates from the glands, it is called lobular carcinoma. The lobules are the special milk-
producing glands. When cancer happens in the ducts of the breast it is known as ductal
carcinoma. When the cancer moves beyond the place where it began, it is known as
“infiltrating” or “invasive” cancer. Cancer that has not crossed beyond the involved lobule or
tubule is very limited in nature. It is called “in-situ” carcinoma. Breast cancer may involve more
than one member of a family; this is usually called familial breast cancer. There may be some
hereditary and genetic cause for this type of breast cancer. Recent genetic advances have
allowed the detection of some of these genes. Women with familial breast cancer can benefit
from genetic counseling and possibly genetic testing.
BIOPSY
A biopsy is the removal of tissue to look for cancer cells. A biopsy is the only way to tell for
sure if cancer is present. Your health care provider may refer you to a surgeon or breast
disease specialist for a biopsy and they will remove tissue or fluid from your breast in one of
several ways:
Fine needle aspiration biopsy: Your doctor uses a thin needle to remove cells or fluid from a
breast lump.
Core biopsy: Your health care provider uses a wide needle to remove a sample of breast
tissue.
Skin biopsy: If there are skin changes on your breast, your health care provider may take a
small sample of skin.
Surgical biopsy: Your surgeon removes a sample of tissue.
An incisional biopsy takes a part of the lump or abnormal area.
An excisional biopsy takes the entire lump or abnormal area.
A pathologist will check the tissue or fluid removed from your breast for cancer cells.
DIAGNOSIS
When breast cancer has been detected, an operation to take the tumor out and to diagnose
the cancer usually follows. During such an operation, the surgeon may check the lymph nodes
in the axilla for the presence of cancer. A health care provider looks at the tissue taken out at
the time of surgery and determines whether or not the tumor is cancerous. If the lump is
cancerous, other more specialized tests may be done on the tissue. Your surgeon and
oncologist may ask you to have more radiological tests. Some of the tests can determine how
much faster the cancer cells are multiplying than normal breast cells. Other tests will
determine whether the cancer cells are under the influence of normal female hormones such
as estrogen and progesterone. This is known as the estrogen progesterone receptor test.
HER2/NEU test: HER2/NEU protein is found on some types of cancer cells. This test shows
whether the tissues either have too much HER2/NEU protein or too many copies of its gene. If
the breast tumor has too much HER2/NEU, then targeted therapy may be a treatment option.
It may take several weeks to get the results of these tests. Radiological tests may include a
bone scan and different CT scans to check whether the cancer has spread outside the breast
and the axilla area.
STAGING
A stage is an indication of how widely spread the cancer is. Staging involves a surgical
procedure to determine the type of cancer and whether it has spread to the lymph nodes.
Treatment can be recommended based on the stage of the cancer. Stages are usually
described using the numbers 0 - 4. A lower number indicates an earlier stage of cancer. Some
stages may be divided into sub-stages. These sub-stages are given a letter designation. For
example there is a stage 3A and a stage 3B. A stage 3B is more advanced than a stage 3A.
Stage 0 breast cancer is ductal carcinoma in-situ, which means the cancer is only in the ducts.
This is called DCIS, which stands for ductal carcinoma in-situ. Abnormal cells growing in a
gland used to be known as lobular carcinoma in-situ, or LCIS. However, these abnormal cells
are no longer thought of as cancer. Abnormal cells growing in a gland are considered to be a
risk factor for breast cancer, meaning that the woman is at high risk of developing breast
cancer. This means that she may need closer follow-up and possibly tamoxifen therapy. Stage
1 breast cancer measures less than 1 inch in size and has not spread outside the breast. Stage
2A breast cancer can be one of the following:
no tumor is found in the breast, but cancer is found in the axillary lymph nodes (the lymph
nodes under the arm); or
the tumor is 1 inch or smaller and has spread to the axillary lymph nodes; or
the tumor is between 1 and 2 inches but has not spread to the axillary lymph nodes.
Stage 3C breast cancer is divided into operable (can be treated with surgery) and inoperable
(cannot be treated by surgery) stage 3C. In operable stage 3C, the cancer:
is found in 10 or more of the lymph nodes under the arm; or
is found in the lymph nodes beneath the collarbone and near the neck on the same side of
the body as the breast with cancer; or
is found in lymph nodes within the breast itself and in lymph nodes under the arm.
In inoperable stage 3C breast cancer, the cancer has spread to the lymph nodes above the
collarbone and near the neck on the same side of the body as the breast with cancer. In stage
4 breast cancer the cancer has spread to other organs of the body, most often the bones,
lungs, liver, or brain.
SURGERY
Most breast cancers are taken out surgically. The extent of the operation depends on the size
of the tumor and whether or not the lymph nodes in the axilla are involved. Breast cancer
operations have two main goals. The first is to take the whole tumor out without leaving any
tumor behind in the breast area. The second goal is to check the lymph nodes of the axilla to
make sure the tumor has not spread to them. If it has spread to the lymph nodes, the surgeon
may want to determine how many lymph nodes are involved with the tumor. Since taking a
lot of axilla lymph nodes out surgically can lead to swelling of the arm, known as
lymphedema, new techniques are being developed to take out only a few important lymph
nodes. Several hours before the surgery, the surgeon injects either a special blue dye or a safe
radioactive dye close to the tumor. During the surgery, the surgeon takes the cancer out and
is able to find the lymph nodes that have picked up the dye. The first lymph node to pick up
the dye is known as the "sentinel node." A lumpectomy is an operation aimed at taking only
the cancerous lump, with some biopsies of the lymph nodes of the axilla. Radiation therapy is
usually given after this type of operation. A partial or segmental mastectomy takes more of
the breast than a lumpectomy. Part of the covering of the underlying muscle may also be
taken out. Radiation therapy is usually needed after this type of operation. Again, some of the
lymph nodes are taken out to check for any spread of the cancer. A total or simple
mastectomy aims at taking the whole breast out, along with some of the lymph nodes in the
axilla. A modified radical mastectomy aims at removing the breast, some of the underlying
covering of the muscles, and possibly part of the muscle. Some of the lymph nodes of the
axilla are also taken out during this type of operation. A radical mastectomy aims at removing
the breast, the underlying muscles and all of the lymph nodes of the axilla. This operation is
rarely done.
ADDITIONAL TREATMENT
After surgery, your health care provider may recommend one or more types of therapy to
help prevent the cancer from coming back. Common follow-up therapies include radiation
therapy, hormonal therapy, and chemotherapy. Radiation therapy is a series of x-ray
treatments that are intended to free the breast or lymph nodes of any cancer cells that might
still be present. It usually takes about 5 to 6 weeks of brief treatments to complete. Radiation
therapy is usually given:
After a lumpectomy.
After a mastectomy if the tumor was larger than 2 inches.
If a high number of involved lymph nodes were found.
If the tumor was close to the chest wall muscles or rib cage.
Health care providers use two types of radiation therapy to treat breast cancer. Some women
receive both types. External radiation therapy: The most common type of radiation used for
breast cancer. The radiation comes from a large machine outside the body. Treatments are
usually 5 days a week for 4 to 6 weeks. Internal radiation therapy (implant radiation therapy or
brachytherapy): The health care provider places one or more thin tubes inside the breast
through a tiny incision. Side effects depend mainly on the dose and type of radiation. It is
common for the skin in the treated area to become dry, red, tender and itchy. Bras and tight
clothes may rub your skin and cause soreness. Gentle skin care also is important. You should
check with your health care provider before using any deodorants, lotions, or creams. Towards
the end of treatment, your skin may become moist and weepy. After treatment is over the skin
will slowly heal. You’re likely to become very tired during radiation therapy, especially in the
later weeks of treatment. You may wish to discuss the possible long term effects to your lungs
or heart with your health care provider. Targeted therapy: Some women with breast cancer
may receive drugs called targeted therapy. Targeted therapy uses drugs that block the growth
of breast cancer cells. Targeted therapy uses drugs that may block the action of an abnormal
protein (such as HER2) that stimulates the growth of breast cancer cells. Hormonal therapy
and chemotherapy are medications that may be recommended after surgery to help prevent
the cancer from coming back. They may be given through an IV into the bloodstream or
orally. Hormonal therapy is usually given if the cancer was found to have estrogen and/or
progesterone receptors. The presence of these receptors generally means that these
hormones promote the growth of the cancer. Hormone therapy with Tamoxifen or similar
drugs can block the effect of progesterone and estrogen. It may be recommended that pre-
menopausal women have their ovaries removed so that they stop producing estrogen and
progesterone after breast cancer surgery. Chemotherapy is usually given in cases where the
breast tumor was very large or it had already spread to other organs of the body. Sometimes
chemotherapy is given to decrease the risk of recurrence even if the cancer has not spread
beyond the breast. There are many different situations where chemotherapy may be
recommended. Your oncologist can discuss the possibilities with you and what he or she
thinks is the most appropriate option for you. Clinical trials are another means of treatment
that are usually available; they are a way to test new treatment options. These trials are usually
thought to be at least as effective as the best available treatment. They may be a combination
of all the above-mentioned treatments.
AFTER THE DIAGNOSIS
After the diagnosis of breast cancer it is normal for most patients to worry about the
prospects and effects of treatment options such as surgery and chemotherapy. Fortunately,
there are several options available to help patients look and feel their best. Plastic surgery and
prostheses are available to make the change in the patient’s appearance as unnoticeable as
possible. In some cases, chemotherapy can lead to temporary loss of hair. Excellent wigs are
available. Choosing one that matches your hair and hairstyle ahead of time is an excellent
idea. There are very helpful networks of patient support groups with people who have all
experienced similar procedures. Breast cancer survivors in support groups and patient
networks are glad to share their experiences and support. It is often a positive step toward
recovery to join a support group and meet people who can understand your feelings.
SUMMARY
Breast cancer is one of the more common cancers. Early detection improves the overall cure
rate and survival. A clinical breast exam (your health care provider checks your breast) and
mammograms are very important. Treatment options, including breast-sparing surgery, are
now available. The outlook for women with breast cancer is now better than it has ever been.
