The applicant asks MPS-ECAS to carry out a certification inspection in order to obtain a
certificate or continue the certificate. The applicant hereby declares that:

MPS-GAP & GRASP:

- They have taken note of the general rules of GLOBALG.A.P / MPS-GAP. with respect to the relevant scheme
(see www.globalgap.org or www.my-mps.com) and will comply with the requirements, as stated in these
regulations, among other things, compliance with the GLOBALG.A.P./ MPS-GAP norm.
- They have taken note of and agree with the GLOBALG.A.P. Foodplus sublicense and certification agreement
and GLOBALG.A.P Data Access Declaration, which can be found on the website www.globalgap.org.
- They agree to check compliance with the GLOBALG.A.P. standards by allowing the independent monitoring
organisation MPS-ECAS to acces, as well to report relevant data to Foodplus for listing on the GLOBALG.A.P.
website, and to notify MPS-ECAS immediately and in writing of any changes to the company’s details, including
the information provided on this form.
- They agree to upload the audit report in the GLOBALG.A.P. database by MPS-ECAS.

General:
- They are authorized - on behalf of the company - to enter into the obligations arising from the submission of
the application
- They will accept the application of the general term and conditions of MPS-ECAS.
- They are familiar with and agree to the relevant MPS-ECAS Certification Regulations as published on the
website of MPS-ECAS (www.ecas.nl)
- They consider themselves capable of complying with the certification scheme(s) mentioned in the application
form.
- They have a procedure for registering and handling complaints and taking corrective measures with regards to
these complaints.
- They will not hold MPS-ECAS liable for damage resulting from the application or its rejection.
- They agree to any attendance by the Dutch Accreditation Council intended to assess the audit work carried
out by the MPS-ECAS auditor
- They agree to pass the audit report to the grower association or the marketing organization of which the
grower is a member.
