Bypass i/v started to open slowly and C/V slowly closed from DCS
Bypass i/v started leaking slowly and vibration started.
It was tried to close the valve but vibration as well as leak increased
Fire caught on leaked RCO
Gradually leak as well as fire increased
Fire tender was at site , tried to put off fire.
Fire fighting continued for ½ an hour.
It is decided to cut off flow from P07ABC to F002. Firing cut off done at F002 manually to
minimum.
Furnace is bypassed for tripping at VDU pass low flow.
Fully Closed, FG and FO control valves, CV IVS isolated, by pass is closed for FG CV lines.
Physically isolated all FO and FG burners. Only pilot kept at line.
Accordingly VDU feed (RCO) diverted to VR route thru P07ABC-E127F-K/E227F-K –E126-TO VR
tank. P07ABC TO VDU furnace I/V isolated.
1. Fire did not get put off until pass flow line pressure reduces.
2. Pass flow main header depressurized thru P24-RCO line bypass (modified line), and
depressurized to V613.
3. Most Emergency steam XZV did not get opened without Instrument Maint. Intervention.
4. Fire tender approach for firefighting was inconvenient.
