The management failure could have contributed to this accident.

1 Non compliance with safety rules: Using mobile phones while working can lead to serious accidents.
The driver was using mobile phone during their job shows the management failed to enforced
restrictions on use of mobile during duty.

2 Poor Housekeeping: Housekeeping is the administrative and routine play activity of a running
organization. The designated walkways blockage due to goods stacking was observed which caused
workers stepping to the vehicles routes and caused accidents.

3 Unsafe Workplace provisions: Frequent near misses and collision between vehicles and workers were
happened there indicate the management failed to provide safe workplace.

4 Absence of Near misses data: The near misses have not been recorded shows absence of crucial
health and safety documents is a management failure.

5 High ratio of injuries: Many injuries over a year have been recorded indicates that accidents are not
thoroughly investigated and the root causes of that injuries accidents were not been identified and nor
have been corrected.

6 Lack of training: The breaks of FLT were applied late by the driver indicates the management failed to
ensure the understanding of driver to apply breaks on timely manners in emergency situation.

7 lack of vulnerable person managing: As the Diver of FLT was new and scenario do not have any
evidence of induction training about the new drive regarding safe driving of FLT.

8 Lack of visible leadership: The management rarely seen in the warehouse shows there was no visible
leadership to lead the workers from the front and lesson and address their concerns.
9 Inadequate Risk assessments: The warehouse risk assessment was generic and the present hazards
and risks of warehouse were not reflected in that risk assessment.

10 Unsafe Working conditions: There have been complaints made by workers regarding the conditions
of work but there is no one to show accountability to address their complaints.
