JOB ORDER CONTRACT OF SERVICE

This contract is entered Between BOOR BARRIO SCHOOL represented by LIEZEL A.
ELIGUE, Head Teacher III referred to herein as “FIRST PARTY” and GELLIEJAY D.
PANGUITO, referred to herein as the “SECOND PARTY” both of legal age and resident of
Binangonan and Cardona, Rizal respectively.

WITNESSETH
That the SECOND PARTY agrees:

1. To render Administrative Staff/School Clerk at BOOR BARRIO SCHOOL and

2. Accomplish the task agreed upon by both parties and submit an accomplishment
report to the School Head.

The “FIRST PARTY” will pay the “SECOND PARTY” the amount of Thirteen Thousand
Pesos (13,000.00) per month from March 2024 to June 2024 but this term can be extended
with the consent of both parties to the agreement.

IN WITNESS WHEREOF, WE HEREUNTO affix our signatures this March 6, 2024, at

Cardona, Rizal.
